Hey saw your post about weight loss and I'm a big fan and it makes total sense. Just curious how being overweight affects pregnancy. I feel like because I'm overweight I'm going to have a high risk pregnancy. Not pregnant at all right now but it's been on my mind lately
Oh boy. Weight in pregnancy is a WHOLE other topic, and not in my wheelhouse--I haven't delivered a baby in over two years now. (They love to show up at 3am and I love sleeping.) The short answer is that if you gain more than the recommended amount of weight during pregnancy, baby is also likely to be bigger, and that's a problem because your pelvis is not capable of expanding, so you run a higher risk of shoulder dystocia, which is where the baby actually jams in the pelvis on the way out, which is typically fatal to the baby within 5 minutes if we can't get them out because they can't breathe and their umbilical blood supply is cut off so the oxygen is not available. There is also a higher risk for gestational diabetes, aka high blood sugars in pregnancy, which also raises the risk of a larger baby. HOWEVER, a lot of this raises questions current research doesn't really answer--are fatter people at more risk in pregnancy because they're fat, or are they fat because they're high-risk? And a lot of people who deliver babies, whether they're OBs or family medicine or midwives or, God help us, lay midwives, will go full-on raging fatphobia, which makes it difficult to parse out genuinely helpful advice from terrible bullshit. Obstetrics in general is the least evidence-based medical field, because no one wants to be the asshole who ran a study that somehow hurt babies, so instead we just fumble around blindly in the dark a lot of the time going "well this SHOULD work" and hoping. Also, racism! Huge thing in obstetrics. Black women have 4 times the risk of dying in pregnancy/childbirth as white women, and of course racism and fatphobia dovetail, where fat Black women get treated particularly badly by the medical establishment.
Long story short, the best way to prepare for possible pregnancy is to take the best possible care of YOU--that means exercise and eating lots of veggies, not because that will make you thin, but because it improves your blood vessel and heart health, your bone health, your stash of nutrients that baby will raid during development, your muscle strength, and so on. And take the time BEFORE you want to get pregnant to find a doctor who delivers babies (whether that's an OB/GYN or a family doctor) who treats you like a human being.
And, controversially, I do think it's best to have a doctor for most pregnancies. Not because midwives can't provide excellent care, but because obstetrics is all about the situations where something goes wrong, and we are very bad at predicting who will have something go wrong. The worst delivery I was ever in looked fine on paper. First-time mom, dilated fine, pushing went great, no problems on the fetal heart monitor, baby descended just fine. Then baby crowned, got shoulder dystocia, my attending had to take over for me and do the maneuvers to free the baby, baby comes out and has to go straight to resuscitation, and I look back over at mom to realize more blood is coming out of her vagina than should immediately after delivery, by A LOT, and we almost had to call a massive tranfusion protocol for her. She hemorrhaged 2 liters of blood in minutes. Then she developed post-partum preeclampsia, which can be fatal if you don't have medical management right there. We had to put her on a magnesium drip to prevent seizures. That was the worst delivery I ever did, and I'm counting the 17-year-old primipara (first-time mom) I worked with while a med student, who delivered in the back-ass of nowhere, spoke Spanish only with an attending doctor who spoke English only in a hospital that didn't bother with interpreters until active labor, and needed forceps, a technique fewer and fewer doctors even practice. No epidural. Forceps without an epidural is, no joke, something that scares me to the point where I simply would not do an unblocked birth.
One of the major causes of maternal death is hemorrhage. If you have an IV in someone's arm pouring in fluid, you can survive with as little as about 1/7th of your original blood. Red blood cells can do a LOT of heavy lifting. And that buys you time for someone to get up to Labor & Delivery from the first floor with the on-hand packed red blood cells that then go into the IV and save your life. Blood transfusions are life-saving, and they are life-saving every single day in the US.
And hemorrhage risk goes up with subsequent babies. Someone giving birth for the first time is at higher risk for a lot of things, but someone giving birth for the 5th time who figures they're OK with a home birth because their other births went fine can bleed out in minutes.
I don't like or trust the medical establishment, and I think OB care has a LONG way to go in the US, because we create PTSD in about 11% of people who give birth. Maternal PTSD is far more prevalent in the US than combat PTSD. I would still have a medical doctor be my obstetrical care provider, because I don't want to die like that.

I recently received an ask containing some concerns the asker had about medical fatphobia. That ask is not posted, because the asker requested that it be answered privately, but I thought it was worth making a list of advice for those of you who are facing fatphobia and are trying to access medical care.
Some of you may feel too triggered, scared, or frustrated to speak up for yourselves when you feel your doctor isn't taking care of your needs. If you're concerned about your ability to do this throughout your appointment, consider bringing along a trusted individual who can serve as an advocate. A family member or friend might be able to do this for you if you have anybody you trust enough. Make a plan with this person beforehand, keeping your specific concerns and triggers in mind. Doctors may be more concerned about showing you respect if they're aware that their interactions with you are being observed.
If you've come in to be seen about a problem and you're concerned it's not related to your weight, but your doctor assumes that it is, try asking them what advice they would give to a thin patient who was struggling with the same symptoms.
Doctors may suggest weight loss as a first course of treatment, and assume that failure to lose weight means you haven't tried. This means you can wait and wait to get your problem addressed, only to have them refuse to address it at all. Some health problems cannot afford to wait that long. Get a second opinion. And if they refuse to pursue further testing, say you want it noted in your chart that your doctor chose not to test further.
I know it's so frustrating and triggering to go through the effort of getting an appointment, only to have your concerns dismissed. This piece of advice is going to be frustrating, because the last thing you'll feel like doing is attempting to speak with more doctors, but keep trying. The financial and emotional impact, not to mention the wasted time, may put you off medical services in the future, but you deserve a practitioner who takes you seriously.
If you feel triggered by being weighed, you can ask them not to weigh you, or not to tell you what your weight is.
Use your community! Ask other fat people who've had success getting real medical treatment how they found their doctor and what techniques they use in medical settings to ensure they get results. Ask them how they manage their triggers in medical settings. If you are diagnosed with a chronic health condition, seek advice from others who have the same condition. Learn what works for them, and how they got diagnosed.
You are not obligated to prioritize losing weight, but if you're fat, you might have better luck with your doctors if you offer a justification for that. Yes, that sucks and no, you shouldn't have to, but if you really need answers/support unrelated to weight loss issues, try saying "I am not prioritizing weight loss at this time. The issue I came in for is impacting my life more acutely just now, and I want to focus entirely on addressing that." (Again, if they insist on adding weight loss into the conversation, or if they refuse to test until weight loss has happened and not fixed the problem, find a different doctor.

We've all heard of seasonal depression, but did you know it is a real medical condition? The Cleveland Clinic states, "Seasonal depression, also called seasonal affective disorder (SAD), is a type of depression. It’s triggered by the change of seasons and most commonly begins in late fall. Symptoms include feelings of sadness, lack of energy, loss of interest in usual activities, oversleeping, and weight gain." Preparing for fall to prevent or alleviate seasonal depression, also known as Seasonal Affective Disorder (SAD), involves proactive steps to maintain your mental and emotional well-being as the days get shorter and the weather becomes colder.
Here are some strategies to help you prepare for fall and mitigate the symptoms of SAD:
Light Therapy: Consider investing in a light therapy box, also known as a lightbox. These devices mimic natural sunlight and can help alleviate the symptoms of SAD. Use the lightbox in the morning for about 20-30 minutes to regulate your circadian rhythm and boost your mood.
Maintain a Regular Schedule: Stick to a consistent daily routine, including regular wake-up and sleep times. This can help regulate your body's internal clock and improve your mood.

Tsuyuri Kikyo(🔥)
Who is Tsuyuri Kikyo? She is the former Flower Hashira, and the adoptive mother of Kanao in my Age/Fate Swap AU. Let's dive into her character a little, shall we?
Kanji: 栗花落 桔梗
Age: mid-late 40's
Status: Alive(retired)
Appearance:
Hair: black with a pink streak, worn in a knot with a flower hairpin
Eyes: blue, both eyes are blind
Clothes: purple kimono with blue embroidered flowers on the sleeves, black hakama pants, purple-laced zori sandals
Notable Features: scars of varying shapes and sizes on her arms, chest, and back, a rather large scar on her right leg, slight limp(more weight on left foot), smile lines and crow's feet
Personality:
No nonsense: Kikyo will take exactly 0% of your BS. And despite some believing her use of Flower Breathing means she's weak, she packs a mean wallop. Even if you are her favourite person on this earth, she will take none of your BS. And her blindness has made her strikes no less true. She can and will knock you on your a**.
Caring: Kikyo is both a sisterly and maternal figure to her juniors and fellow ex-Hashira. She truly considers Kanao the child she never had, and only has a different consideration for Aoi out of respect for the girl's parents. Her care can be harsh, but you'll feel better afterwards... eventually
Perceptive: As a former Hashira and current medic and trainer, Kikyo has a perceptiveness only rivalled by the Hashira with special senses(including her adopted daughter, Kanao). Even without sight, she can readily provide a challenge to the Demon Slayers in her estate.
Stubborn: Kikyo is exceedingly stubborn. The only people to outmatch her stubbornness have been Tanjurou, Kie, and their kids. The only reason she retired was because she sustained damage that would prove life-threatening in the field.
Meiji Era Rumours/Secrets(This isn't ALL the interesting stuff, but the rest is very big spoilers for this AU, so they'll be left out):
The kanji for Kikyo's given name means "bellflower".
Kikyo is a Marechi. While not as rare as Sanemi Shinazugawa's, her blood is still considered a delicacy by demon standards. She became a Demon Slayer to better protect herself and those around her, after a demon attacked her for her Marechi blood and killed her boyfriend.
She actually beat up Tanjurou(Tanjirou and Nezuko's dad) one time. She gave the Kamado patriarch time to grieve the loss of Kie and the younger four kids, even taking in Tanjirou and Nezuko as needed, but decided an intervention was needed before her best friend drowned in the grief. While she loved the Kamado kids, she strongly felt the family needed to stay together.
During their run as Hashira, Kikyo, Tanjurou, Urokodaki, and Kuwajima were considered the quartet to beat. Even after retirement, they all have a reputation that keeps their juniors from getting too cocky around them.
Out of the four, Kikyo was the third to retire(after Urokodaki and Kuwajima, but before Tanjurou). Overuse of the Equinoctial Vermillion Eyes technique cost her her eyes, and very nearly her leg, so she retired so her girls wouldn't have to fend for themselves before they were adults.

Healer of Jedi
I see an awful lot of fics where the Clones have to forcibly take care of Obi Wan, with the implication that he is an idiot who can’t take care of his own needs. Here’s an inversion of that. (I have to admit I may be projecting just a bit, as I have been the adult others thought they needed to take care of, even when I didn’t.)
It opens in a small room in  the Halls of Healing. Sitting off to one side, half hidden is a member of the corrie guard (perhaps they are there to visit Vos, who’s done something stupid. Perhaps they are delivering or picking up something or an escort.), he has been sitting quietly off to the side for long enough that everyone has kind of forgotten he was there. In comes Bant Eerin, Mace Windu, and Obi Wan Kenobi, discussing medical supplies. Videos of General Windu or General Kenobi are a premium item on the clone black market, so the trooper goes to record (he figured if it was truly private he would have been asked to leave. This would be true, if they had been paying attention). By accident, instead of the record, he hits the transmit button. Every bucket in the GAR is suddenly live streaming this small room. 
Bant turns to Mace and Obi Wan going ‘ok, both of you can drop the glamor. I’ll see you first Obi, Mace go ahead and take a seat’. Suddenly Obi Wan Kenobi and Mace Windu look horrible, like death warmed over. They are both emaciated and exhausted, gaunt. Bant asks some questions that have some frankly horrifying answers (How long since you last slept: “Two months”; How Long since you last ate: “Three Ten day''; Current Pain level: “6 out of 10” ) .  What is even more terrible is that Bant is clearly not reacting as if those answers were as concerning as they are.  She gets through her questions and exam for both and tells them that they are right where they should be, tells them that their month long coma was scheduled for Four months from that point, and reminds them that they were about Four ten day out from consumable Force Suppressants, as opposed to collars or binders, being a death sentence.  Mace and Obi Wan go their separate ways after putting the glamor back on. 
By pure chance Obi Wan makes it back to his quarters on the Negotiator without meeting any of the clones, most of whom are still reeling from the way that Obi Wan looked and how he was obviously not taking care of himself (What the kark is their jedi doing to himself?). Cody manages to intercept the crowd of clones that want to storm the room and forcibly put their idiot Jedi to bed (the medics at the forefront). He points out, rightly, that dog piling their general to make him take care of himself is just going to make him clam up more.  Cody would go in first and do some recon. 
Cody goes in to see his General, doing flimsi work like nothing was wrong. He tells Obi Wan about what he had seen, what the entire GAR had seen, and asks about it.  Obi Wan tells him about the Force Technique the entire council is using (as it really wasn't a secret, though there is some concern about his private medical conversation being broadcasted to the entire GAR) where all of their physical needs (sleep, sustenance, healing) is taken care of by the Force for up to eight months, with the immediate repercussions being that once this technique is dropped (after being held for at least a week) the Jedi in question has to be put in a very specifically calibrated coma for a month in order to regain what they lost.  It does not prevent immediate physical effects of those things (weight loss, scaring,  the visual signs of exhaustion) but does prevent long term damage and collapse.
 The High Council has an agreement with the healers that they would use this technique for a total of six months at a time (to ensure that there is a cushion of time) before being put into the coma to recover, then start the process all over again. The Masters of the High Council were on a rotating schedule for the month-long ‘mission’  that was cover for the coma. It is an all or nothing technique, any attempt to meet those needs above and beyond what the Force provides (sleep: one night every four months; Food: A meal every 4 ten day) will release the technique in its entirety.  If it releases without the aforementioned highly specific coma, all of his organs will shut down and kill him.  For injuries, either the injury will kill him before the medics can get to him or The Force will heal him. There is no in between.  There are long term consequences to using this particular technique as it does significantly shorten the lifespan of anyone who uses it. Most of the council figure they have about another five years of use before Yoda is forced to step down, maybe another ten years before there are significant effects for the less long lived species. 
The glamor is because people get uncomfortable when they start showing signs that any of them are less than well rested. Morale matters after all.
Cody can’t quite keep the hurt from showing when he asks why his general didn’t tell them what was going on. Because if nothing else it is dangerous when the medics aren't aware how to treat someone.   
Obi Wan looks at him, surprised, and tells him he tried. At the beginning of the war Obi Wan tried to tell Cody, as well as a number of the medics, about his unique requirements. Each of them, including Cody, told him to ‘stop with the Force Osik’. Cody gapes a little, because he does remember several conversations where he said that to his general (There is more than a little self recrimination going through his head). At some point Obi Wan tells Cody that the medics have had permission to access/request his full medical file from the Jedi temple since the beginning of the war and that they haven’t yet (His medical file would have all the requirements for how to best to work around the restrictions of the technique they were using).
After a few minutes Cody manages to ask why the council needed to use this technique. Obi Wan tells him that the High Council has much too much work to do, between the War, the work given by the Senate, and normal duties of the High Council. They had already delegated everything that they were able to other councils, but it is still too much. 
Obi Wan shows Cody a Pad with a to-do list on it with something like 1000 items on it. Cody agrees that this is a bit of a long list if he wants to be done before his month long coma.  Obi Wan sighs and tells Cody that this is everything he has to do before the next day. Then he tells him everything on the list that is highlighted, about 60%, are things that a member of the Senate has insisted that a High Councilor of the Jedi Order handles personally (Among them are absolutely stupid things like researching some obscure fact of a separtist planet, to see if that fact could be used to bring them back into the fold; the kind of thing that could have been assigned to a junior padawan). When Cody questions why they haven’t tossed some of these things back at the Senate as something they don’t have time for, Obi Wan tells him that if the Council admits to being overwhelmed or unable to do whatever the Senate has assigned them then the Senate will take away their battalions rather than the stupid busy work (and the Clones represent one of the only points of happiness in any of the council’s lives). Cody clocks that one of the items is checking Anakin Skywalker’s paperwork (It was a special request by the Supreme Chancellor, who justified it with the fact that Obi Wan was hesitant about Anakin being ready to be knighted). 
Any more discussion is interrupted by a Comm from Mace marked urgent. Both think that it is likely about the recent revelation. Cody steps from Obi Wan’s quarter to a crowd of clone troopers, with the head medic furiously leading the charge.  Cody has to forcefully pull rank to make them listen to what is going on and why (“Every time we have said the words ‘Force Osik’ we have been telling the general that our perception matters more than his actual needs”).  Then he tells the Head Medic to get in touch with the Temple healers, get their generals actual file, and figure out what they can do to actually support their general (and see if they can get whatever is needed for the month-long coma, just in case something happened). He also says that they need to find someone from the 501st to get Skywalker up to speed with his “Force Damned Paperwork”, so that their general doesn’t have to grade it any more. Cody will work with the general, the council, and the other commanders to see if there are any workarounds for lightening their collective workload.
