so I keep learning about The Horrors™️ of American healthcare and I keep hearing Americans talk abt this shit like it’s totally fine and let me tell y’all. It’s not. Apparently and ADHD or Autism test costs like 500$ for you guys? And then when you do get diagnosed you gotta think abt shit like “okay but do I actually need meds?” Bc apparently that’s a financial decision? But it,,, it shouldn’t be right? Right okay bc
I live in Europe, specifically Austria, and this is how healthcare works here: if you have a job you have healthcare. Your employer pays for it. They have to. It’s absolutely illegal and unheard of not to. They do take about 20% out of your pay-check every month to pay for that and other legal things that I don’t quite understand (my dad explained it to me once but I’m 20 so I still don’t get it bc I don’t really have to but every cent they take out benefits me I think so it’s fine).
Now irl this is a lot more complicated but basically, you have public healthcare and private healthcare and then just stuff that’s up to the government (like ambulance rides, I think. Even if you don’t have insurance u don’t pay for that. I know this bc an American friend of mine didn’t and had to go to the hospital and called a cab and when they told me I was like “??? BRO WHY NO AMBULANCE” this is also how I found out that Americans have to pay for ambulance rides. wtf.)
Basically your insurance covers all medical expenses from doctors who are in your network (public practice). This is your GP, hospitals, and pretty much all specialised docs (dentist, internists, cardiologists, etc.). However, the public offices are much busier than the private ones (obvi) so if you’re in a rush, want special treatment, or just be a little fancy you can CHOOSE to pay for private doctors. And even then you can hand in those bills to your insurance and you get part of the money back (abt 30% which you receive abt 4 months after handing the bill in).
If you keep your bills for private care doctors you can hand them in to,,, someone somewhere somehow (idk man) and get a bigger tax return. (bc we don’t calculate our taxes ourselves. it just gets taken out of your pay-check by your employer every month and then at the end of the,,, fiscal year??? I guess?? Idk man idk how shit works — you get any money that was like,,, extra (????) back)
Now you might ask yourself: “phi, why would I pay for doc if I can go to free doc?” Which is an excellent question. Again, for most people it’s about waiting times but there is one field where it just really makes more sense: mental health services. Private practitioners have less clients and more high quality care than public ones because they choose their own funding, which, surprise surprise, is better than public funding.
I will now take you through my own personal journey so you can have a practical example of how good healthcare CAN AND SHOULD BE (keep in mind here I have parents who after messing me up and being terrible but learning their lessen once I became an adult and told them how horrible they were are very supportive of me pursing my mental health)
When I was about to turn 20 I went to my GP and had a conversation with her about how I think I might have ADHD. She asked me about my complaints and agreed there definitely are symptoms indicating it. She wrote me a referral to a Psychiatrist. When I got to my Psychiatrist she tested me and because of the referral she could tell the insurance company that this test was necessary and not elective, meaning insurance payed for it in full and I never even saw that bill.
Now I had a diagnosis all but a week later. I spoke to my parents who agreed to support me in finding private care. My Psychiatrist charges 160€ (insurance returns 48€) per appointment which my mum graciously pays for. I see him once every couple of months to check in on my use of medication and how I’m responding and to see if my dosis needs adjustment. If and when I need a refill, I only have to write him an email. Before he could put me on medication I needed a blood draw and cardiac test, to ensure that I qualified for a stimulant, since they can be harmful in some cases. He wrote me referrals and I went to my GP and a public cardiologist for those tests, which were all free of ANY charge.
I was then cleared to take my meds within a few weeks (cardiologist had a bit of a waiting list). I was prescribed Ritalin and some sleeping pills by my psychiatrist. He gives me the prescriptions and a letter to the insurance company asking them to cover the cost, as I have a diagnosis which he confirmed again in his initial assessment of me. Now that I’ve gotten approved, all I pay for is the prescription, not the actual medication. Which is about 6-7€ for the Ritalin and 4-5€ for the sleeping pills (monthly supply each).
My Therapist is also a private doctor. She charges 120€ per session and I go twice a month, which is 240€ per month. My dad contributes 100€ and my mum contributes 30€, which means I’m down to 110€ per month. Insurance pays 30€ per session (so 60€ per month) so I pay a grand total of 50€ or 25€ per session.
So my medical treatment each month costs me just about 62€ AT MOST (since I don’t always need my sleeping pills refilled).
Now, just for fun, let’s assume my parents don’t support me and I see both my therapist and my psychiatrist in the same month and need both my meds refilled. This would come to just about 304€ (because of insurance deductions).
But you have to consider that I DON’T see my psychiatrist every month, so the more realistic monthly cost would be 187€. And should I decide I can’t pay for that I can still get on a waiting list for public mental health practitioners, in which case the only thing I’d ever need to pay for are the prescription costs for my medication which would still be up to 7€ for the Ritalin and up to 5€ for the sleeping pills putting the grand total at 12€ PER MONTH.
My family and I simply chose the quicker, more efficient option, but no one twisted our arm into paying this amount of money FOR BASIC CARE.
And yes, it’s a lot of bureaucracy (it took me WAY too long to spell that) and paperwork, but I’d much rather that than have to decide between mental well-being or being broke.
So, my conclusion: Cancel America, move to Europe, OR just tell the American People In Charge Men that it is that easy and they really should try a little harder and you citizens shouldn’t settle for the shit you have to put up with right now because wtf
if you wanna cry a little it’s okay lemme know I’ll come sit with you and offer a hug but you can say no if that’s not your thing

10 hours of therapy
This time of the year does bring back some strange vague memories. It takes me back to fall of 2013. That was 10 years ago?!
I don't remember much from that whole year. But that is nothing new. I do remember that I ended up in therapy. It was 10 hours and they were paid by the insurance and there was no need for a diagnosis on their behalf. It was easy to get a spot there because they are a team of psychologists and stuff and they are specialized in taking in clients who struggle with addiction.
I can barley tell you how everything went because I barley remember. I guess I always went there hungover. I do remember going there very drunk once but no one stopped me. It was an extra appointment with their psychiatrist. She discussed with me what meds I should take and prescribed me some. I don't do well with any type of chemical “stuff” in general (an OD thing when I was 16). I threw the prescription away.
I also don't remember talking much in there because stuff like that triggers me so I dissociate. My memory loss when it comes to these sessions could be a combination of being hungover and also dissociated? Why did no ever ask me about dissociation?! I was to unaware of it at that time to bring it up myself.
What else do I remember....having to fill out stupid questionnaires. The ones where you can already see what they are aiming at with the question.
As you can see I really enjoyed all of this.
Idk what else to say. I'm hesitant on calling it useless because knowing what you don't need and what does not help is also something vital to learn. It was good for something.
Yeah idk.

I had more therapy but I paid cash for it. That opens a up a different world. But I will write about that some other time.

everyone be proud of me, i got my ass out of bed BEFORE 3 pm so i could deal with my health insurance problem, because there's no motivation quite like "if i don't fix this i can't afford my medication, if i can't afford medication that means PERMANENT ZOMBIE MODE"
like. i'm out of all of my pain meds except the one i never take (and am now taking to be merciful to my body) and i just ran out of my vyvanse, aka the only thing to get me out of bed every day.
the health insurance problem itself is incredibly stupid. i know for a fact that i have an account for my state's medicaid website. i have a case number, a client ID, a medicaid ID, and also this is the website i used to get medicaid in the first place

I have Colorado medicaid. You know what we have covered? Dental (cause they realized it was cheaper to give dental than pay for complications) and eye exams (no glasses, but better than nothing)
People act like, and I wish I was joking, the idea with socialized medicine is to hold doctors at gunpoint (I figured we could just... pay them, but you do you rightwingers)
It's actually just really good insurance. I get into places as fast or faster than when I had actual insurance in the past (despite all the places that don't take medicaid, which might change if everyone had it). I've gotten way more care since I don't have copays. I pretty much never have had anything denied... it's like normal shitty insurance, but a bit better
Also here's a great bonus story, my mom's on disability. Had medicaid before she was able to get on disability even. Well she gets old enough to qualify for medicare. She's forced to pay like $125 a month for the privilege of worse insurance
By that I mean medicaid covered her thyroid medication. She gets medicare, they don't want to cover that med despite it being an old one and one she doesn't have a bad reaction to. Well medicaid won't cover it now cause she has other insurance. She literally pays a fee to be forced to pay out of pocket for a med that was previously covered (and as far as I know basically can't drop it now that she has it without messing things up with medicaid)
So... yeah. Just... just every time I mention like going to the dentist and having people say lucky, and me realizing oh shit I am lucky, and my luck being that I was broke enough to qualify... what the fuck?
Really really wish everyone just had at least as good as Colorado medicaid, even fucking bezos so we're not worrying about enforcement nonsense
Really wish we had universal healthcare cause... basically I just have kickass insurance, and it sucks hearing that you, someone working, are less well covered than I am
(And private insurance could still exist, it just might be forced to... you know... actually provide a benefit at a reasonable price to give people a reason to not just go with their medicaid)
Honestly the way we handle health care in this country is a goddamn nightmare, and we literally already know and have known for years that it kills people.
I will say that often even good medicaids are uhhhhhhhh not so good on the provider side. They "cover" a lot of claims that don't actually get paid, ya know? I work in healthcare, and have generally been a big fan of the medicaid in my catchment area because it actually seems to be pretty comperable to how you describe CO's. That said, as a provider, I would say a solid half of any "non-urgent" claims get denied, and at least one of the medicaid providers has denied every single claim the practice has submitted to them even though they keep telling us we're in network with them and following their instructions.
So. I do sort of get why it can seem (depending on which side of the insurance you're on) like holding providers at gunpoint. On the patient end you've got people receiving reasonably quality/comprehensive care with no fee at the point of service, which is phenomenal! On the provider side though, it often looks like losing literally thousands of dollars a month on denied claims for necessary services with little to no recourse.
We do it anyway of course, because the practice I work for is strongly oriented towards "serve the clients best needs even if it causes operating at a loss" but admittedly not every practice can survive under a premise like that for long. I think one of my biggest frustrations is that they always reject crisis care billable hours, even though they claim to cover them, and even when I am literally talking someone through a total breakdown in functionality. It's very frustrating to know that one of the most basic elements of mental health care (emergency support in crisis) will never be paid for (keep in mind that often when insurancr denies a claim, a practice won't pay a provider for that time) which neither the practice nor I can really afford.
Basically, we desperately need universal quality healthcare coverage, as well as (frankly) incentives for providers to actually register with the providers of this insurance rather than providing OOP care only. And that universal coverage needs to understand that "protecting patients from overcharging" is not mutually exclusive from "paying claims for emergency/out of norm care".

Hello from COVID isolation
Where there's nothing to do but post... except work I suppose.
So here's what happened: On Wednesday I was about to head out to see friends. I had a little tickle in my throat, nothing I would even take meds for, but I took a rapid test just in case... wherupon I got the very faint positive I already posted.
For safety's sake, I did not go see friends and I ran the apartment isolation plan we used when my spouse got it. I immediately took a second test of a different brand, and that was negative. I have not tested positive since then. Thursday I felt more tired than usual, but that could have been from canceling all the things, notifying everyone who was a close contact and stressing about these outcomes (also adjusting to sleeping on an air mattress, which is fine... but not my bed). Today I feel quite good actually.
As far as I can figure there are one of two equally unlikely options:
a) the test was borked
b) I had COVID, fought it off enough that I didn't notice, and only caught it at the tail end. (I don't regularly take tests so I hadn't taken one in weeks... probably just before we babysat my sweet nephew.)
I guess b) is a little more likely because last week I was at a client conference where I know people tested positive after (plus it was in a hotel so a lot of in and out traffic), although I don't know if I was their close contact. I was masked 95% of the time when I was inside, the remainder being drinking water and iced coffee indoors but away from everybody. Most people were not masked, of course. On the other hand, cooking a baby and whatnot, I am very aware of my symptoms and how I'm feeling—probably more than ever before. It's hard for me to believe that it would be so mild that it wouldn't even occur to me to test.
Post-test I felt really sad and guilty that I had failed to protect my sweet decembaby by maybe/likely getting COVID. But as my sisters remind me, I did as much as I could—it's society and the prevailing YOLO COVID attitude that failed me. Yes, it would have been safer to not drink anything or at least go outside and try to find something—but dehydration and exhaustion are real! (And I don't even know if it was the conference—the next most likely vector is public transit, which I really can't avoid because I have to go to work, I can't walk there and I don't have a car.) I played the hand I was dealt. I am glad my OB got back to me right away and gave me sound guidance on how to deal.
I'm going to isolate through Monday (5 days) and get a PCR test that will hopefully fully clear me. Props to my insurance's 24-hour nurse line for talking me through this and also complimenting me on my COVID preparedness (sad laugh). Cruel irony, I was supposed to get my updated booster on Saturday and now I either have to put it off for a few weeks or a few months if I actually did have it (which I hope an antibody test will establish, but I can't get that within 2 weeks of positivity). (Update: An antibody test, at least the common consumer type, cannot distinguish between vaccine antibodies and infection antibodies. Sorry about the misinfo.) So close, yet so far!

I feel scared. My gut is churning because I don't feel stable or safe. My world is upside down. I drove through the streets wailing, crying, yelling my distress in my car, through tears. Through endless cold concrete roads. Desperate for connection. Unsure of where I was going, just that I needed to get out. Had to figure out a destination in my gps and just go there and sit in the car. Then figure out some game plan. Who do I call. Who makes me feel safe. Who can I rely on. Who won't try to manipulate my decisions and emotions based on what's happened. I'm glad I called my friend, she's always reliable, responsible, caring. She let me stay over. I just felt homeless or like a domestic abuse victim. And part of me was legitimately thinking.. am I a victim? Or did I choose this? Am I the problem? Am I the financial leech? If I do all the house chores, work part time, pay what I can in rent, look after the pet, the only one who drives (and generally is a taxi when needed), pays for petrol/car insurance/rego, always collects groceries (pays for whenever I go alone), the only one that cooks, i have the mental load of admin of when bills are due or birthdays or events or even reminding partner to take meds or not spend as much money, I have mental inventory of everything in the house and when its low so I can prepare before it runs out, I don't pay for takeout, I pay for vet bills/medicine, I can barely afford my own necessities so I'm always in poverty mode. I study as well and I'm sporadically all over the place with the amount of mental/emotional work in my head for each client's support needs, and driving to each physical context can be frazzling. My neurodivergence means I get burned out quicker. My trauma means it's taken me longer to learn life skills. My lack of family social supports means I'm pretty isolated. Theres no safe place to go immediately thats family, my mum is interstate. Everyone else is overseas. My mum basically inserted herself into men's lives/houses and stayed in dysfunctional relationships for survival. I met my current partner while essentially homeless and just did the same thing. And I'm just wondering was it dysfunctional from the start. I was seeking someone who could be relied upon in every way, just as I try to reciprocate this back in every single way. I don't want both of our traumas to be triggered in every single conflict. And for it to spiral out of control. I don't want to feel resentment but I do. I don't want to be doing so much of the mental load, house admin, life admin, and I don't want to feel like I'm asking for much when I expect this just to be a basic pillar of support. It's this vicious cycle of both of us being stubborn. I'm told I still need to be communicating my needs, even when it comes to basic house upkeep. I am the only one that wants to decorate the house and make it feel livable. I also have to tidy and unbox my partners boxes full of sentimental and tech gadgets and excess that is hard for him to rid of. I don't think it's fair it should fall on me. I don't think it's fair I have to do more of this type of mental labour. I don't think it's fair to turn around and manipulate me with words like "I got adhd meds for you, otherwise I wouldn't care" or "I didn't want to rent but I did it cuz you wanted to" or "you think I'm a burden and you just don't want me in the house" or "whens the last time you showed me affection" and trying to make data out of human complex memories which are obviously hazy and tainted by emotional bias. I shouldn't have to coddle someone's extreme self-deprecating thoughts. It's so confusing. So it makes me feel like I'm the villain. Am I being manipulated?
It's just a bit absurd that I don't really have a choice. And the only thing I keep thinking now is get full time job and just find a different living situation. Live with ladies who just reciprocate in harmony and don't have to be micromanaged.
So I screamed and screamed through tears. So much pain and trauma and distress was released. So much panic. I have no idea what I'm supposed to do. I don't understand what comes next. I have anxiety because my abandonment trigger is being hit. I feel like I'm codependent with my partner. I feel like it's no healthy. I don't know what we should do.

Get a new PCP. Look for other primary care providers who are in-network with your Medicaid plan and see if they’re taking new clients, schedule an appointment, call your insurance plan, and request a change to the PCP they have on file to your new provider.
I appreciate the sentiment but medicaid is a broken site, the reps give me doctors by name who take medicaid only for it to turn out to be for kids only, and I've been doing this dance since I got dumped into medicaid the first of the year. I only found the pcp I have, an MD because they have to be to prescribe my adhd meds, through my mom. The number of MDs that take medicaid is pretty close to zero and the ones that do are underfunded and deeply suspicious of anyone needing a schedule 2 drug.
It's not this simple. It's very likely I will have to give up access to my adhd meds to get trans care. And not only have I done this myself, I've worked the insurance and billing side. I know the tricks, I know what to ask, medicaid in this state is just a broken system and anyone in it who is genuinely trying to help (the reps are always nice) is stopped by the system itself.

So M2...
He's great. Really great. He's a good guy. He went through some struggles and changed his life around and became a therapist and turns out he doesn't take insurance -- originally I told him he was an ass expecting people to pay out of pocket. Nope, he has a sliding scale as low as $25 an hour. And his clients can text or call if ever they need on his private cell.
He adores his daughters. He so good with animals he has the two best cats I've ever seen. He plans fun nights out and cooks delicious dinners in. He has good media taste. The sex is good. He is gainfully employed, he has a car, his apartment is so shocklying beautiful (the apartment, he has no furnishings lol... my eyeballs itch to fill the space all the time because it's just got such great architecture and I'm a sucker for architecture.)
He's open and honest... I think. I mean, I always worried he's just being polite, like me coming over was because we can't come to my place and me sleeping over was just to be polite. Especially the second time because at like midnight or one he said how he could have called me an Uber any time if I didn't want to stay.
He says in like 3.5 years I'm only the third woman he's let into his apartment and the first he's slept next to in years. He said he just felt comfortable with me -- my head doesn't even hurt his shoulder like usual.
He made an off hand comment how Boy Thing would probably love his girls -- I had said how he wanted to steal the cats and no, I do not think either of us were thinking of them meeting, just the fact that it's even occurring to him struck me. (No one meets the girls, which is kinda my policy too since... well, you all know when.)
He asked me after I slept over the second night why I hadn't brought my meds and I said it seemed presumptive to bring overnight stuff (especially because he had an importabt day the next day), and he said if we were near his place he was assuming I was staying over, unless he specifically said so. So to see if he just meant toss my meds in my purse or full overnight bag, I mentioned buying a travel set for shampoo and stuff, and he seemed approving.
So far the downsides are he isolates when he's upset about something unrelated to me and he snores. (I never list sex drive as a downside unless it's like never, because expecting anyone to keep up with me is unreasonable and it's not like I'm not satisfied and he's happy to get me off evened if he's not in the mood, so yeah... even the not perfect is still pretty damn close.)
And the thing is...
He's too good for me. He went to great schools and a great education -- I slacked my way through something I wasn't that into just to be mildly employable. He makes a ridiculous amount of money, I will never, because I just don't care enough for that level of work. He's smarter than me, which is something I don't generally encounter in the wild. I've even gained weight so I don't even bring decoration to this partnership.
I just feel so incredibly self conscious and insecure because I feel like I'm just waiting for him to realize it. And if he did have one actual flaw, he's pretty slow on the commitment front, which isn't bad, but I'm not really sure where I stand -- I tend to know, if not how serious I'll be, how serious I COULD be with someone. And I don't know what he thinks or feels.
And I can't expect everybody to announce every feeling or thought. That's not fair and I know that.
It's just... scary. I don't do vulnerable well. I don't like games or believe in then but I generally like to know I don't have the losing hand even if there isn't an upper one. And here I just feel lost.

So my job is doing case management for disability services. The county contracts out to companies like the one I work for to manage shit. So one thing I'm really familiar with is state healthcare coverage for MN. I also am familiar with it because I've been on it for over a decade. But I am familiar with an important part of it: The income limits. My partner currently doesn't get paid fuck all. His current job is structured in a way where he only gets benefits if he's full time, but they won't give him a full time caseload for almost a year. He's also a full time student. He also only gets paid for the amount of hours he actually bills. So if his clients cancel, he doesn't get paid. Which sucks because when I worked the same exact type of job in the past, I was salaried and had production expectations. Anyway. Last night he's on the phone with his dad and I'm in the room and he's panicking because he needs to find a doctor to prescribe him his ADHD meds because he can't function without them and he's been only taking them on school days because he doesn't want to run out. His dad is sitting there going "Look into an Obamacare program and we'll pay for it." The entire fucking time I'm going "No. He can apply for the state program. He is under the income limit." This back and forth went on until I went "It's my job to know if he qualifies for these programs. Trust me. He can get state insurance." I did his application for him today because he was busy with school and work. Guess what? He's eligible for straight up medical assistance. Which is BETTER than what I get as a disabled worker. He will literally not pay for any healthcare related thing as long as he has MA. Like fucking hell. I need to nap or something. I also need men to listen to me when I say something important.

So it's obviously been a while.. Posts will probably be more sporatic and be general summaries now.
March 1st-15th
I had my MRI and dentist appointments. It's been 2 years since I've had any insurance and 5 years since I've had good insurance. I'm hoping to get a lot done before I lose it again.
Despite having a decent insurance, they declined to cover one of the meds for my migraines. So rather than pay $2000+ for a med I just have the less effective one.
I finished my first classes and took my spring break. I also quit one if my clients, cutting the hours for that job from 18/week to 10/week. I plan on using the extra time for schoolwork.
I started to paint my white bookshelf, but only have 1 side done so far.
I finished reading Lies and Plague from Gone. And with that I am successfully reading most mornings.
I have been outlining project tasks and paying my brother to do them. So the garage is nearly clean and usable.
My brother had his 21st birthday. He had friends over, drinks, and played some DnD. I got him a zweihander.
I've had some very serious conversations with my friend to reevaluate our relationship and some aspects. We are hoping that makes everything smoother and less conflict heavy.
Soccer is a mess as we keep having coaches drop out and complain. The first uniform order was submitted, but another one will need to be done this week.
