Don't worry about the blood. The Straw Hats were facing a tough battle and Yuna decided that she'd have to resort to turning into her monstrous shark form to give them an advantage. Sanji strongly objects at first, remembering what happened last time and is afraid that he'll lose her again. But Yuna reassures him, having trained the past two years and gained more control over her powers. He relents, deciding to trust her, and after their enemies are taken care of, Yuna returns to her human form, still breathing but exhausted, her blood turning into a field of amaryllis flowers. To his relief, she opens her eyes briefly and asks if she can rest, to which Sanji tells her to go ahead, that he will take care of her.

Note: I didn't actually watch the Z film. I just thought he looked really good in the outfit so that's what I chose 😅 Now I'm picturing this as one of those Fabio romance novel covers LOL again, thank you for that scrumptious chest view 👀
