People can abstract from feeling too much pain if it breaks their mind from it being too much. Characters feel pain from things, but not as intensely as they would in real life.

Okay so *Notable Pikmin 4 Spoiler* but--
I was so happy to learn about this little guy. His name is Yorke and he's Alph's oldest brother! He seems like a swanky dude and for whatever reason I like drawing his eyes ~Like That~.
You may notice I did some experimenting on shapes but the biggest things I have to make sure when drawing Yorke are that his nose is small, his eyes are also small and closer together (like Alph).

Not a theory or analysis here, just a "wouldn't it be funny if..."
I've entertained for a while the almost completely unsubstantiated idea that the Medicine Seller could be descended from or otherwise related to Tamamo no Mae, the most powerful and evil kitsune in Japanese folklore. According to Yokai.com, Tamamo no Mae's fatal influence over the emperor is said to have led to the end of Heian period and the rise of the shogunate.
The shogunate fell in 1868, which was then followed by the Meiji period, a time of radical change and increased prosperity for Japan.
For most of Mononoke, the Medicine Seller appears to be living around this time; that is, in the late Edo period, mid- to late-1800s. In any case, we know for sure that the shogunate is still in power in Karakasa.
Based on the time period, the presence of a mononoke inside the palace, and Mononoke's general attitude toward the social structure of the period and those who held power in it, I kind of expect there to be at least an implied connection between the events of Karakasa and the eventual fall of the shogunate.
It wouldn't be the first time the presence of a mononoke and the Medicine Seller brought down a powerful but faltering lineage (see the entire Sakai family being shredded like cheese in "Bakeneko" #1). Nor would it be the first time there was an implied connection between the Medicine Seller's actions and the shift forward from the Edo period (see the jump from the mid- to late-19th to the early 20th century in "Bakeneko" #2).
My thought then was, wouldn't it be poetic if the Medicine Seller, a kitsune who magicked his way into the shogun's palace and helped to destabilize the shogunate, were related to Tamamo no Mae, a kitsune who magicked her way into the emperor's palace and helped bring about its rise?
