Horrifying Theory:
This wasn't an accident.
Manga spoilers below.
I say it wasn't an accident because we find out in a later chapter that All For One was lurking around on the mountain. Since I don’t think he’s the type to be out for a casual stroll just because, this would imply heavily that someone, either himself or one of his underlings, was keeping tabs on the situation in the Todoroki house. Given that he has his 'friends' everywhere, this is not beyond the realm of believability.
However...he just happened to show up on the very night Touya’s Quirk went out of control and nearly killed him? Sure, we can be allowed a coincidence for plot convenience but…
Also recall that All For One did (and still does?) have a Quirk that forced other people to use their Quirks. He used it on Kurogiri and Magne back in the Kamino Ward Arc so the LoV could make their escape.
He could have started Touya’s fire. He had the means.
And he has a motive.
Because in Touya's flashback after waking up from the coma, AFO essentially tells him his body was destroyed and his Quirk is significantly weaker, but they may be able to restore him and asks if he's interested in 'joining their family.'
May be able to restore him?
Okay, we have seen AFO and Ujiko accomplish some pretty terrible but, scientifically speaking, impressive feats in biology and modifying Quirks. I'm calling bs on them not being able to put a physically broken teenager back together in the three years he was in a coma. That 'join the family' offer was a caveat and a manipulation of the ‘I will hurt you so I can help you,’ variety. It was supposed to foster a sense of gratitude and loyalty in Touya so as to make him a potential candidate for the whole vessel thing if Shigaraki didn't work out.
Except that plan went awry when AFO seriously underestimated Touya's obsession for his family/father's attention. Touya rejected the offer and ended up fleeing, and they let him go as a lost cause because they believed he'd be dead in a month anyway.
Touya didn't burn himself. All For One did.

IDEA!!!!
AFO steals an immortality quirk while he is young. More specifically a quirk that prevents aging. So OFA is instead AFO's babysitter because a superpowerful child needs a superpowerful minder
A much happier AU! Yoichi is horrified when he first realizes his brother doesn't age. He has no idea what a bullet he just dodged.
Let's say that AFO got stuck around age five because he has to be very young to predate when he went evil. AFO gives Yoichi many more powerful quirks since now Yoichi is the one protecting and providing for them both. Since he's still an idealist, Yoichi joins the vigilantes as a youth. Second and Third adore little All for One. (His ego is much cuter when he's five.)
I'm sure they tried to persuade All for One to pass on the quirk so that he could grow up. But All for One discovered he liked everyone doting on him, and he'd have to get a job if he grew up, so he pretended he couldn't.
Without any supervillains to ruin everything, the trio lived long and happy lives. They asked their adopted kid Hikage Shinomori to look after little All for One after they passed. He's still a bit of a terror so they need to give Hikage One for All so that he has enough strong quirks to control a superpowered child. For plot convenience, One for All includes a body strengthening power so the holders don't die premmaturely.
After Yoichi's death, All for One was inconsolable but calmed down when he realized Yoichi's vestige was inside One for All. He's been clingy with all the holders ever since, viewing them as extensions of his brother.
All for One keeps getting passed down between the holders. In Toshinori's era, there was some trouble with the HPSC trying to kidnap him. But overall society is more peaceful. Nana is still alive by canon time to see Toshinori become number one hero.
After being selected as the Ninth, Izuku becomes close to little All for One and actually persuades him it would be fun to grow older and attend school. He finally lets go of the quirk. I'm sure it will fine, All for One had a few centuries of character development.

Hi, I already made a post about this but I decided to make a more elaborate post on the subject
as we know after failing to steal One For All from Banjo and En, All for one realized that he would need a greater will than the user and his predecessors to steal the OFA, as he could not, he proceeded to find a Suitable Successor/Receptacle with enough will to steal the One for all
Due to the Connection with All Might, the Shimura family was the main sieve of All for one, but it was not the only one, as we know Garaki had several Orphanages and All for one probably Evaluated and tested possible Vessels there, including Number Six
This shows that even though he was working at Tenko, he was never the Only Possibility, because that doesn't suit someone who is always planning all the variables.
but he didn't limit it to Garaki or Shimuras, as shown with his experiment with Touya Todoroki
as All for one was coincidentally already at the mountain that day, it is possible that he used "Forced Activation" to cause the Fire, These three Shigaraki, Dabi and Number Six all have their hatred shaped based on some kind of heroic obsession.
What if this was also the case for Izuku Midoriya, but perhaps this would be a slightly more complex Experiment because of this because of this he was secondary, it would be an even more ambitious Vessel, since All for one would play a little with the Quirk Singularity, and your own genetics by having a child with someone from the fourth generation, the first generation that demonstrated the concrete Signs
the goal was not only to create an All for one with the ability to Hold the complex Quirks of the future, but also combined with Inko's Quirk it would be able to Steal Quirks without the need for direct touch, as it is already difficult to copy the All for one in a perfect and functional way, it is impossible to create something even more powerful in the laboratory

interestingly Koga (AFO Construction Site) Musutafu and Jakku Hospital then connected in a triangle close to each other, for easy accessibility to AFO and Garaki
and both families knew each other, little Tsubasa played with Izuku and Katsuki, and Izuku's mother knew Tsubasa's mother, probably that's why they went to Garaki since the families were already friends
Tsubasa was eventually transformed into Nomu for reasons currently unknown.
However, the Bullying did not have the expected effect and did not generate in Izuku the hatred and resentment that AFO wanted and neededIzuku was too good, he didn't create a grudge, he didn't get that hatred for society that Tomura got, which made Shigaraki Tomura the suitable Vessel for All for one, which made all for one leave this family adventure aside and focus on Tomura, leaving an automatic network sending money under the banner of a "Hisashi working overseas"
Maybe the reason AFO categorizes Izuku as Useless, and criticizes Yoichi for trusting him with the OFA is because he didn't become a Suitable Vessel and didn't live up to expectations, almost a parallel to Dabi, right?
Maybe he left it to use Izuku's Quirk when it was really necessary in the future
