You gotta wonder what comic Ozai was thinking when his dad just goes up to him and basically says he has to marry some girl he's never met for the sake of the bloodline and have children with her even though he won't get the throne anyway.
I think it depends on certain factors. According to the ATLA comics, Ozai is at least a decade Ursa's senior. It is possible that Ozai already had a lover who was closer to his age and possibly had to abandon that relationship due to the Fire Sage's prophecy and Fire Lord Azulon's demand. Regardless of Ozai’s own wishes and desires regarding his own marriage and love life.
However, the comics try to make Ozai an irredeemable monster and caricature of a villain. So this interpretation is too sympathetic. Even though the original ATLA creators and OG ATLA Wiki on Nickelodeon stated that Ursa was raised as royalty and was the perfect betrothed and possible wife for Fire Prince Ozai.
However, Zuko’s flashbacks showed that Ozai did make an effort to try to be a good father and husband to Ursa and their children. The good memories of the Fire Nation Royal Family on Ember Island hurt Zuko and Azula during the Beach episode. This is why Zuko desired Ozai's love, respect, and admiration from him. If Ozai was only a cruel bastard to his family and son why would Zuko want to go back to his father and homeland to be abused again?
The Search Comics by Yang just took the most stereotypical, uninteresting, and non-complex lazy route regarding the Urzai relationship. Ursa is a good woman! Ozai is a bad man! She must have been kidnapped and physically, mentally, emotionally, and sexually abused! This is why Yang goes out of his way to demonize Ozai and bastardize his characteristics for his own personal fanfic OOC named Ikem. Yang has a horrible habit of adding new uninteresting characters into other established characters' lives. Yang destroyed Urzai with Ikem and also Maiko with Kei Lo.
Even though I don't think Ozai was Prince Charming regarding Ursa initially due to her being the granddaughter of Avatar Roku and possibly being a non-bender. Ursa possibly could have been seen as a blood traitor by the royal court and Fire Nation nobility and royalty due to Avatar Roku attacking and stopping Fire Lord Sozin during his early stages of imperial conquest until he died on Ember Island giving Sozin the chance to end the Avatar Cycle by killing the Air Nomads. However, I do believe that Ozai eventually warmed up to Ursa and bonded with her by the time they had Zuko and Azula. We even see Ozai be quiet and somber regarding her favorite spot by the Turtleduck pond after her banishment and disappearance.

Thoughts on Ikea and Kiyi possibly being his child?
If Ursa says the child is Ikea's, then she's definitely not Ikea's. It's the same Ursa who once wrote a letter announcing that Zuko himself also was Ikea's. The same Ursa who spent a week saying that Azula also was Ikea's (somehow) when things started to get difficult between us. That was always her petty way of messing with me.
The girl melted a steel door without any prior training. I'll let you draw your own conclusions.

Ursa: Oh no! He's sick! Ozai he's sick!
Ozai, feeling Zuko's forehead: He's feverish. Great. Every time one twin gets sick the other goes down too.
Ursa: Let's not panic. Maybe they've grown out of it. I saw Azula this morning. She's fine.
Azula, from the other room: *sneezes*
Ozai: Is she, woman?!
[Rushes into the other room and sees Azula lying on the couch surrounded by used tissue]
Azula, feverish: Mommy I'm sicky. *sneezes* So cold... I want my blanky!!!
Ursa: I-I can't get sick! *Nudges blanket on Azula with her foot* There! All better!

Ursa, sighing: Looks like I'm on plague patrol. I can kiss Dragon Empress leading role goodbye.
Ozai: Wait a minute firelily, I can take care of the twins.

Honestly Ursa is just lucky Azulon didn't find out about those letters because he would incinerate Zuko, her and probably Azula just to be safe and beat Ozai's ass for being 'incompetent' enough to let his wife supposedly sneak out and get laid behind his back.
Yeah the comics make her moronic because she legit endangered everyone just to see if Ozai was reading her stuff.
Funny in a morbid way…
Ursa? Killed by her daddy-in-law! Zuko? Killed by his granddaddy! Azula? Killed by her granddaddy! Ozai? Ass beaten by his daddy! Iroh? Heartbroken and furious because of his daddy!
I affirmatively agree, that if Fire Lord Azulon found out and read about those letters then Zuko and Ursa definitely would be killed for treason if Ozai and Iroh had not stepped up in Ursa's defense. Fire Lord Azulon is extremely old and as Azula stated not as powerful as he used to be. Ozai most definitely would not let any harm come to his favorite child and daughter Azula. Probably would fight an Agni Kai in her defense. Ursa probably would have to beg Iroh and Ozai for protection for herself and Zuko. Ursa is extremely lucky that Ozai knows the truth that both Zuko and Azula are his children. Along with Ozai being willing to defend Azula at least and Iroh willing to defend Zuko.
To be frank and brutally honest. Ursa was extremely stupid for writing that the Fire Prince of the Fire Nation and fourth-in-line to the throne Zuko is an illegitimate bastard that she had with her lover Ikem during her marriage with Fire Prince Ozai and she is trying to pass him as Fire Lord Azulon's rightful grandson and Fire Prince Ozai’s rightful son. It would be very hard to convince Ozai to defend Ursa after she wrote some hateful, untrustworthy, unfaithful, and cruel lies about their child; claiming that he is another man’s son. If Ozai were to side with Azulon and probably the Fire Sages then Ursa and Zuko are roasted and toasted LITERALLY!
I am completely against and appalled by how the ATLA comic The Search made Ozai a completely inhuman monster and Ursa a complete moron. Ozai and Ursa are a heartless and idiotic himbo and bimbo couple in the comics, unfortunately…
