I really enjoy the moment where Mikey lists the rules of the family snowball fight during the episode 'Snow Day' because it slightly shows bits of Leo, Raph's & April's characters when we watch their reactions to Mikey listing the rules to them.

Leo is taking notes on the rules; he's carefully making sure he knows what is & isn't allowed most likely so he can either manoeuvre around the rules or know how to bend them later on. Raph is jumping up & down in excitement, showing that he wants to get into the action of the snowball fight & April looks amused with April possibly remembering the incidents that lead to most of those rules being made in the first place.

Who do you think is most likely to have found out about Leo's psychosis? (If they made an episode about it)

Most likely it would be either Donnie, Raph or April.

In canon, Donnie was the first and only one to walk in on Leo during his conversation with Splinter's spirit in the first episode of season 5.

Donnie walks in on Leo during this supposed moment of psychosis, asking Leo who he was talking to and Leo responds by saying "no one" and quickly changing the subject by focusing on the current mission. Donnie shows some concern before Leo leaves and the scene ends. It was small moments like this I wish were explored more. Donnie of all people I feel would notice the signs/symptoms because of his expanded knowledge.

Raph and April are my other two candidates because they are the ones who have shown the most concern for Leo. Raph mentions to Mikey in the second episode of season 5 that he worries for Leo, how much he knows Leo misses Splinter and wishes there was a way he could help him. Then in the final episode of the Kavaxas arc, April makes a comment about Leo to the others saying "I feel so bad for him. The weight of the world is always on his shoulders."
