I have this headcannon that when they were kids, a bit after Katsuki started being really mean, Izuku had the sad realization that maybe Katsuki doesn't want to be called Kacchan anymore. So he calls him Katsuki-kun like the other kids.

Katsuki fucking hates it. He doesn't know why but Izuku calling him that is way more irritating than him saying Kacchan. It makes him so confused and angry, he explodes at Izuku and yells that he doesn't get to call him that and storms off.

Secretly happy about it, Izuku switches back to Kacchan and doesn't call him anything else for years.
