If you want to have some idea of how things are going in our household, here is the Saga of getting my child his medication.
My youngest child has reactive airway disease (aka it's asthma but he's too young for them to perform the tests to give him an official diagnosis). He has been to the emergency room at least once a year since he was 1, and was hospitalized at 14 months old because he couldn't breathe.
We've determined that his asthma is activated when he gets respiratory illnesses, so we try to keep him from getting sick. But with my spouse working in the office 4-5 days a week, my oldest child in kindergarten, and my youngest now in preschool, we get sick all the time. So it's critically important that he takes his daily asthma medication, Flovent (fluticasone). When he's sick, he has to take additional doses of it every day in addition to albuterol.
Earlier in 2023, when I went to pick up his medication at our local CVS, they had some bad news for me. Our insurance only covered the brand name Flovent inhaler, not the generic fluticasone. Okay, I said. That's a little silly because I assume it costs them more money to cover the brand name than the generic, but it's their decision. But CVS policy is that if there exists a generic version of a medication, they can only fill the generic version. So, my options were 1) pay out of pocket for the generic fluticasone inhaler or 2) get a new prescription from our pediatrician that stated "brand name only" and then come back and pick it up and have it be covered.
We had enough puffs left on the inhaler that we opted for the second choice. Within a day, our pediatrician had sent over the new script, CVS filled it, and we were all set.
A couple of months ago, however, we got a letter from our insurance company. Starting January 1, 2024, they would no longer be covering brand name Flovent. Instead, they "suggested" we switch to the generic fluticasone, which they would now cover. GSK, the manufacturer of Flovent, would be discontinuing the product, hence the switch. So I assumed this meant in January, we would need a new prescription for the inhaler that did allow for the generic. There was nothing in the letter that suggested I needed to do anything else.
Earlier in December, we were running out of Flovent. I called in the prescription to CVS and went to pick it up the next day. Sorry, they said, we don't have the inhaler in stock. I was told that it was on the way and that they would fill it when it came in. Okay, fine, we have puffs left, we can wait. But a week later, it still wasn't in. The day after that, it still wasn't in. What was going on?
On my most recent visit to the pharmacy, I lucked out. One of the pharmacists overheard the conversation I was having with the technician. He interrupted to explain that there would be no restock. The reason they had no Flovent was because the manufacturer had already discontinued the drug. If the pharmacy was out of the drug, it was out of the drug forever.
So here I am, out of Flovent for my asthmatic child who already had spent Thanksgiving in the ER. I have a prescription for it, but it's for the brand name only; even if the generic is already covered, the prescription is for brand name only, so I can't just get the generic. Tomorrow is Saturday, Christmas Eve.
It's New Year's Eve tomorrow. My pediatrician has sent the prescription over to CVS twice in that time. They haven't gotten it. She sent it to the clinic's pharmacy as well. THEY haven't gotten it.

Day 12 of @creativepromptsforwriting 30-Day Writing Challenge instructs us to write about our MC’s motivation. I found the question fascinating when looking at it from a general perspective, and I will discuss all three of my MCs: Casey, Kaycee, and Zoe. As those familiar with my writing know, Casey & Kaycee are essentially the same MC. They exist in two different universes with different LIs. They have different characteristics that make each better suited for their “world” (especially as they enter adulthood), but their childhood/early years are the same, so they share the same motivating factors. As such, I will address them as one and deviate when needed. I will also discuss my newest MC, Zoe, at the end.

Casey MacTavish Carrick / Kaycee MacClennan Ramsey
I am writing about Casey/Kaycee’s motivation to enter the medical field, but it goes beyond that. The life experiences that led them to pursue a career as a physician are also motivating factors in other areas of their lives. These experiences helped to shape their overall life philosophy. Had they not entered the medical field, I am confident both would have chosen professions where they worked toward solutions and for the betterment of others.
Casey/Kaycee grew up in a small, loving nuclear family; it was just her and her parents, David & Rose. She saw both sides of her extended family frequently as a child, and while she was close with both, she did have issues with some members of her mother’s family. They were a very traditional Italian Catholic family, and, even from a young age, some of their ideas were in direct conflict with Casey/Kaycee’s. Her father’s family was of Scottish & English descent. While they tended to be old-fashioned, they were more open-minded and accepting of differences/changes.
Casey/Kaycee and her parents lived in a lower-middle-income area of Philadelphia, and she loved growing up in an urban environment. She grew up in a community made up of people from different races, nationalities, and religions, and she credits that experience with making her who she is today. Additionally, her paternal grandparents lived on a farm in rural Pennsylvania, and she spent a good portion of her childhood summers there. This gave her even more perspective on different ways of life.
A pivotal event was her mother becoming critically ill when she was just four years old. Rose had been her primary caregiver; her dad was a schoolteacher who attended graduate school part-time. Rose worked part-time to help with family finances but took a step back after Casey/Kaycee’s birth because she wanted to spend more time with her daughter until she was in school. When Rose became ill, she was in the hospital for extended periods, and when she was home, she often couldn’t care for her daughter. 
Casey watched her father struggle to keep their family afloat. He couldn’t take time off work because he was now the family’s sole income and source of medical insurance. He had to care for his ailing wife and be a father to their little girl. They were lucky enough to have extended family to step in and help, but it was not the same. This instability, coupled with Casey/Kaycee’s fear of losing her mother took its toll, and it led to a lifelong struggle with anxiety. 
Once her mother was on the road to recovery, her family faced another obstacle. The medical bills they incurred to save Rose’s life were astronomical. With Rose still too ill to work for some time, David postponed graduate school. He worked as a teacher, a tutor, and a line cook at a local restaurant on weekends. He also took on odd jobs to keep his family afloat. Even at a young age, Casey/Kaycee knew it was wrong for a family to be thrown into such chaos and poverty because of an illness.  
In her hometown of Philly, she saw many people who became very ill or died because they could not afford adequate healthcare. She also witnessed the lack of medical access available to people in her grandparent’s rural community. She was familiar with the inequity in all areas of American life, including healthcare, which she saw as a fundamental right. These things made her angry, but she was also determined to do something to fix it. Both Casey & Kaycee did just that, albeit in different ways.
After completing their residencies at Edenbrook Hospital in Boston, they established successful medical careers before incorporating ways to help the underserved. Kaycee left Edenbrook and founded the Dolores Hudson Family Clinic to meet the needs of uninsured and underinsured families in Boston. Eventually, she had several locations in Boston, one in her hometown of Philadelphia and one in her husband’s hometown of Providence, Rhode Island.
Casey also remained at Edenbrook for several years but eventually moved with her family to Washington, DC, where she was an attending at George Washington University Hospital. She was also a registered federal lobbyist and worked tirelessly for healthcare reform in America. She focused on healthcare access, addressing gender and racial inequity, and mental health reform. She did this until after the birth of her third child when post-partum complications nearly cost her life. After that, she and her husband started a family practice in Washington, DC, so they could spend more time together and with their three young daughters. Their practice was dedicated to serving those who could not otherwise access healthcare. They created a model emulated by many others.
Zoe Rivera
Zoe is a young woman who has grown up in a post-apocalyptic world. But even before she was born, her life was marked by challenges. Shortly before learning she was pregnant with Zoe, her mother informed her father, Daniel, that she was filing for divorce. When she realized she was pregnant, she agreed to carry the child to term but did not want custody of her, or her first daughter, Ana, who was three years old.  
Her distraught father met the love of his life, Samuel Connors, shortly before Zoe’s birth. In time, they were married, and Samuel adopted both girls. Daniel and his new husband were loving, doting fathers, and the young girls had a wonderful family life. However, Ana was traumatized by her mother’s abandonment. While Zoe had no memory of her, she didn’t realize how much knowing her mother didn’t want her impacted her until much later in life. Still, during the brief period the family had before the outbreak, they enjoyed a very happy existence.
That came crashing to an end with a parasitic infection spreading worldwide, causing many of the Earth’s population to die off. Many who didn’t succumb to the ailment turned into zombies. Samuel was bitten in the early days, but Daniel refused to “put his husband down” after he became ill, insisting the man he loved was still inside. Unfortunately, he turned and attacked Daniel in front of the girls, and Daniel instructed Ana to take Zoe and run. That and the horror the girls endured for days after impacted them greatly though Zoe was young enough that she didn’t retain many memories.
Ana’s motivation was Zoe. She essentially lived for her little sister from that point on, doing all she could to ensure her safety and well-being. Zoe was always bright and strong. She knew right from wrong and wouldn’t shut up when she saw injustice – a true liability since they ended up residing in a totalitarian colony, The Tower. But between Ana’s protection and Zoe’s fighting abilities, which were viewed as an asset, Zoe flew below the radar… for a while.
When her outspoken nature got her on the wrong side of several Tower Guards, she was selected to be “sacrificed” to the zombies. Unbeknownst to Zoe, Ana and her friends got wind of this and planned an escape. There was one thing Ana knew she couldn’t survive, and that would be the loss of her baby sister. She would do anything to protect her, and in the end, she lost her life the night they escaped. Zoe was distraught, and Ana’s death and the events that followed haunted her for the remainder of her days; however, she could move on and became the leader of a colony she founded with others. Despite all the tragedy she had endured, she found love and lived a fulfilling life. Sometimes I have reflected on this because… how? After losing so much, enduring so much trauma, and living in a world filled with horror… how?
To me, Zoe had a natural optimism from the time she was a child. Her sister mentions how she was always headstrong and a natural leader; she truly believed Zoe was destined for great things. I think part of the reason she felt that way was that Zoe could find sunshine in the middle of hell. She found a reason to smile when none seemed to exist. Though she felt pain profoundly, and at times, the darkness took over, her ability to move on – with purpose and joy – inspired many.
But how? What motivated her?
As a child, I think it was her coping mechanism, and it’s a direct contrast to Eli’s coping mechanism of withdrawal. Zoe needed to know that there was something more. She looked for ways to find kinship and joy with others; she had just had to. Ana kept the story of their father’s deaths from her until the very end of her own life. Deep down, I believe Zoe knew they were gone. Still, a small part of her had held onto hope that she’d find them again one day, and that motivated her to become a fearless fighter, to see the world outside of the Tower. Ironically, when she finally gets to do so, she learns they’re long gone and loses her sister too. But her core personality was already formed, and she was determined to find reasons to live and others to love. Ana’s death was a further motivator. She knew her sister lived and died for her. Though it was sometimes almost too great a burden to bear, she refused to let all of her sister’s sacrifices be for naught. She promised Ana she’d be brave, she’d help others, she’d live… and she intended to keep that promise.

Wisdom in a new world. The creation of Britain’s Welfare State

Britain’s National Health Service was established on the 5th of July 1948. Its founding father and chief architect was the Minister of Health and Welshman Aneurin Bevan.
Before the NHS was set up, healthcare was provided on a patchwork basis with many people having to pay directly for primary and hospital care services. The NHS was to be funded out of general taxation and based on the principal of being "free at the point of delivery".
The aim of the NHS was to promote: "the establishment of a comprehensive health service designed to secure improvement in the physical and mental health of the people of England and Wales and the prevention, diagnosis and treatment of illness".

It was not until after the Second World War that the British Welfare state took its mature form. In a climate of relief after the war, a climate diffused with an idealism for a new and just society. There was a clear sense of rebuilding a better Britain.
The period before World War Two had seen long-running debates about the lack of co-ordination of services. There was also concern to learn from existing experiences of a health insurance scheme for medical treatment for some of the population. There were many criticisms of the Poor Law including the indignities of means-tested payments and fear among the elderly, of ending life in the workhouse.
But the Labour Party landslide victory in 1945 was about creating a new deal for "the boys back from the front", giving them a sense that their country had been worth fighting for and would support and care for them in peacetime, by offering them and their families jobs, homes, education, health and a standard of living of which they could be proud.
The centerpiece was a state-run system of compulsory insurance. Every worker, by contributing to a scheme of "national insurance" - deducted through wages - would help to build up a fund that would pay out benefits to those who were sick, unemployed, or suffered from war or industrial injury. The scheme would also pay pensions at the end of a working life to employees and the self-employed.
The idea was support the worker and family. Benefits were set at a level that enabled a man, his wife and child to survive. There would be benefits for widows and an allowance for guardians of children without parents. A system of "family allowance" for the second child and subsequent children was intended to ensure, that those with large families were not penalized.
There was also to be a marriage grant, maternity grant and some specific training grants and even a death grant. The key feature was that people were eligible to receive these benefits because they had contributed to the Welfare State. Rich and poor 'paid the stamp’ and could claim as a right because of their National Insurance contributions.
Alongside these financial provisions for all, there would be universal access to education and to health services - the NHS. These would be funded through taxation and would be "free at the point of access". Everyone in work would pay, but in this case, since taxation increased with income, the rich would pay more.
The welfare reforms gave meaning to the proud boast that the welfare state provided for everyone -  'from cradle to grave’. But for it to happen there had to be employment. The post war government would give top priority to the rebuilding of a strong, "peacetime economy" and the redeployment of British troops into civilian work.

Texas Mass Shooter All-too-Familiar
LOS ANGELES (OnlineColumnist.com), May 25, 2022.--Slaughtering 19 children and two adults at Robb Elementary School in Uvalde, Texas May 24, some 80 miles west of San Antonio, 18-year-old Salvador Rolando Ramos showed all the same traits of school shooters, reclusive, temperamental and non-communicative, all the hallmarks found in teenage killers.  While there are plenty of misanthropes that don’t commit mass shootings, Ramos fits the profile to a T, showing the kind a Autistic Spectrum disorder often overlooked and untreated by school officials.  Whether Ramos was ever treated for autism or other behavior problems isn’t know but it’s clear he had all the features of a ballistic killer.  Ramos was shot dead by an off-duty immigration and border inspection officer but only after he had gunned down 19 children and two adults.  Ramos’ mass-shooting comes only 10 days after a racist teenager murdered 10 Black people at a Buffalo supermarket.    Before Ramos went on his rampage at Robb Elementary School, he shot his grandmother, who apparently remains in critical condition at an Uvalde hospital.  Imitating the Dec. 14, 2012 Sandy Hook Elementary mass shooting in Newtown, Conn., Ramos, like 20-year-old  Sandy Hook shooter Adam Lanza,  shot his grandmother before moving on to his rampage at Robb Elemenary.  Lanza shot his mother between-the-eyes before going on to Sandy Hook where he killed 20 children and six adults.  Law enforcement officials no doubt look for a motive or precipitating cause for Ramos’ shooting rampage but glaring similarities exist between most childhood shooters.  Like 18-year-old Payton S. Gendron who shot 10 Black people May 24 at a Buffalo supermarket, Ramos followed the same script of a teenage killer equipped with AR-15 assault rifles and body armor.    Debate has started again over the Second Amendment and the need for better gun control legislation.  When the National Rifle Association [NRA] meets in Houston May 27, speakers, including former President Donald Trump and Sen. Ted Cruz (R-Tx.), will no doubt blame the Uvalde incident on mental illness or some other type of depravity.  “He [Ramost] was kind of a weird one.  I never got along with him.  I never socialized with him.  He doesn’t talk to nobody,” said Juan Alvarez, 62, boyfriend of Ramos’ biological mother. “When you try to talk to him he’d just sit there and walk a way,” speaking volumes about the reclusive, socially inept personality that often goes with child mass shooters.  What’s disturbing is the fact that Uvalde police, two months earlier, came to the Ramos home for a domestic dispute when he left to live with his grandmother. ��Uvalde police did not assess Ramos for dangerousness.    Police and other mental health professionals often miss signs of dangerousness, failing to get appropriate treatment for suspicious individuals.  With today’s medical insurance not covering all mental health, it’s difficult for law enforcement or school counselors to refer to appropriate treatment.  Police cannot intervene early because it violates civil rights, unless a violent incident has already occurred.  Ramos had no  history with the Uvalde police or his high school with violent behavior.  Reports of Ramos telling a friend that he might do “something” didn’t result in police notification.  Ramos grandfather, Rolando Reyes, said they were not aware that Ramos possessed assault rifles and ammunition. But whatever the holes in the current mental health system, elected officials must pass some type of gun legislation to prevent teenagers from getting assault weapons.    Ramos apparently bough the assault rifles legally close to his 18th birthday.  Where he got the cash for the purchases in anyone’s guess.  Elected officials, including Second Amendment advocates, must accept existing research on brain development, knowing that the adult brain doesn’t mature until at least 25-year-of-age, maybe later.  So when it comes to purchasing firearms at retail stores, elected officials can surely agree that teenagers aren’t ready to assume responsibilities of gun ownership.  Much talk in the gun debate focuses of longer waiting periods to complete gun purchases or extending more comprehensive background checks.  While all of that can’t hurt, elected officials must keep guns out of the hands of teenagers, no matter what.  Until the teenage brain evolves by age 25 into a responsible adult, the government must prevent teens from buying lethal weapons.    Salvador Rolando Ramos was another teen killer, someone with a disturbed upbringing, but, more importantly, just not mature enough to buy lethal weapons.  When 49-year-old Texas Democrat governor candidate Beto O’Rourke interrupted Texas Gov. Greg Abbott at a press conference today in Uvalde’s central square, it said more about politics than any practical ways to prevent future mass shootings.  “This one is on you,” O-Rourke screamed at Abbott, blaming the pro-Second Amendment governor for the horrific mass shooting.  But instead of hurling insults, elected officials must work together across the aisle to come up with a fix that works.  Since it’s impossible, at this point, to assure comprehensive mental health services, elected officials could still change the age at which lethal gun purchases take place.  Raising the legal age to 25 would have saved at least 19 children and two adults.
