Seit Ende letzten Jahres gibt es hierzulande deutlich mehr Infektionen mit A-Streptokokken. Die Bakterien können unter anderem Scharlach auslösen. Infektionen durch Streptokokken der Gruppe A haben europaweit aber auch in Deutschland seit Ende letzten Jahres zugenommen. Das trifft auch für Scharlach zu. Bis Mitte März 2023 sollen bereits doppelt so viele Scharlach-Fälle gemeldet worden sein wie im gesamten letzten Jahr, berichtet der Bundesverband für Kinder- und Jugendärzte e.V. Scharlach ist keine schwerwiegende Erkrankung und wird in aller Regel nicht im im Krankenhaus behandelt. Deswegen sehen wir im stationären Bereich praktisch keinen Scharlach – egal, ob die Fallzahlen gerade hoch sind oder nicht. Im Gesundheitsamt auch nicht, denn Scharlach ist nicht meldepflichtig. Momentan sind sie aber schon wieder am Abklingen. Was wir in der Klinik sehen konnten, waren vor allem schwere invasive Infektionen durch A-Streptokokken. Die werden durch den gleichen Erreger wie Scharlach ausgelöst, nur äußern sie sich als Blutvergiftung, als schwere Lungenentzündung oder als Knochen- und Gelenksentzündung. Da hatten wir auch einen gewissen Gipfel – vor allem im Dezember und Januar. Aber dieser ist nun vorbei. Scharlach wird durch Streptokokken hervorgerufen. Sobald die Temperaturen steigen, beginnt die Zecken-Zeit – und das teilweise schon recht früh im Jahr. Es gibt bereits Phasen, wo Zecken schon stellenweise aktiv waren. Erst Anfang des Monates hatte das Robert Koch-Institut (RKI) drei neue deutsche FSME-Risikogebiete in Bayern und Sachsen ausgewiesen. Die Frühsommer-Meningoenzephalitis (FSME) ist eine Hirnentzündung. Zecken können die Viren übertragen. Die meisten Infektionen verlaufen ohne Symptome. Das Risiko einer schweren Erkrankung ist bei Menschen über 60 Jahren deutlich erhöht. In der ersten Phase gibt es dabei häufig grippeähnliche Symptome, später kann eine Entzündung des Gehirns, der Hirnhäute oder des Rückenmarks folgen. Risikogebiete vor allem in Süddeutschland. Ein Infektionsrisiko besteht laut RKI vor allem in Bayern und Baden-Württemberg, in Südhessen, im südöstlichen Thüringen, in Sachsen und seit dem Vorjahr auch im südöstlichen Brandenburg. Hinzu kommen einzelne Risikogebiete in anderen Bundesländern. In den Risikogebieten empfiehlt die Ständige Impfkommission (Stiko) eine FSME-Impfung. Zecken können außerdem die Erreger der Lyme-Borreliose übertragen, was oft erst Stunden nach Beginn des Saugakts erfolgt. Sie ist laut RKI wesentlich häufiger und kommt deutschlandweit vor.
