Vaccines are typically injected into an arm muscle, including those recently developed against SARS-CoV-2, the virus responsible for COVID-19. Yet coronaviruses first infect the mucous membranes in our respiratory tract, in the nose, mouth, throat and lungs. Administering vaccines intranasally, to target those frontline areas, could allow us to respond to infection faster. Nasal vaccines against SARS-CoV-2 have already been developed in India and China, but researchers are now testing a new intranasal live-attenuated vaccine, containing a modified virus. Tests in hamsters found it highly effective: very few SARS-CoV-2 particles (in brown) penetrated their nasal mucous membranes after double vaccination with the intranasal vaccine (top left) or even a combination of one intranasal dose with an existing mRNA vaccine (bottom left), compared to poor results after intramuscular vaccination only (right-hand panels). Following up these encouraging results with safety and efficacy trials in humans could provide new options for future vaccination.

met my new doctor yesterday, learned that I can in fact get vaccines on this new medication as long as they're not specifically live attenuated, and went oh yay I've been worried about that, let me get both the flu and covid shots while I'm here.
did not think that through at all. woke up with The Devil's Migraine. just awoke from dark slumber feeling mostly normal which I fucking better because I have fucking black friday tomorrow. did NOT think this through.

Many people think the only ingredients in vaccines are a little bit of dead or "attenuated" (deadened) virus and some saline, but they are gravely mistaken (pun intended). If someone were to actually read the vaccine package inserts, and then add up the amounts of toxic ingredients included in the 70 vaccines kids get before age 7, meaning the whole CDC-recommended vaccine schedule, they would find that each child ends up being injected with the following:
600 micrograms of mercury
Over 13,000 micrograms of aluminum - plus, aluminum helps mercury cross the blood-brain barrier
Over 200 different chemicals
Genetically modified and adulterated additives, preservatives, adjuvants, emulsifiers, and stabilizers, including formaldehyde (embalming fluid), monosodium glutamate (MSG), sorbitol, industrial-based dyes, polysorbate 80, alcohol, acetone, and castor oil
Residuals of peanut oil, latex, lactose, eggs, dairy, antibiotics, genetically modified spike proteins -- the reason for so many severe childhood allergies
Blood (albumin) and infected cells from African Green monkeys, eagles, dogs, pigs, cows (fetal bovine serum), and cells from human abortions (MRC-5)
Wild, live viruses from other animals, including deadly circovirus from wild pigs in China, coronavirus (with gain-of-function), etc.

A quick guide to vaccines for scifi writers
(can you tell I've seen too many wildly innacurate bits of fiction this week?)
What is a vaccine for?
A vaccine is a fake disease. Its job is to pretend to be one very specific disease and con your body into fighting it, without risking you getting dangerously unwell. I compare it to fire drills when I'm talking to children. You have fire drills at school/work so that if there's a fire, everyone knows what to do and everyone can be safe. You have vaccines so that, if you ever meet this disease, your body knows what to do, and your body can be safe.
Important: Vaccines prevent. Vaccines very rarely cure. In almost all circumstances, you have to have had the vaccine you come in to contact with the disease, sometimes several weeks before. In a very small number of cases, in diseases with very long incubation periods (the time gap between catching it and getting sick), vaccines can be given in that long time gap and still work. Once you have symptoms, vaccines will not help you.
How long does a vaccine take to work?
It varies. The fastest one I've ever seen boasted 72 hours to onset of immunity, five to fifteen days is more normal
What is a vaccine made of?
Dead disease (killed vaccines), maimed disease (live or attenuated vaccines) fake disease (mRNA vaccines), or part of a disease (subunit vaccines). Dead viruses or bacteria, obviously, can't actually make you ill. But immune systems are suspicious gits, they'll still practice killing dead bugs most of the time. Subunit vaccines also are very safe, but small mutations (variations) in the bugs can stop the vaccines from working. Live vaccines tend to be very, very effective, but might be able to make you sick if your immune system is very weak. I could spend a long time explaining why mRNA vaccines are so, so clever (I thought they were likely to be impossible when I was an undergrad - like teleportation or universal translation software), but here is a nice readable explanation if you're curious.
But the disease in my story is a virus, so antibiotics won't work on it, so I need a vaccine, right?
Okay, yes, but also no. In face of a viral epidemic or pandemic, a vaccine is pretty much your only hope. But, as COVID showed us, vaccines are bespoke, slow to make, and don't work perfectly - vaccinated people still got COVID and, occasionally, still died. But there are a whole lot of other disease on which antibiotics won't work. Fungal infections (like in The Last of Us, or murcomycosis in real life) are scary and we don't have many treatment options for them. In many cases, you have to cut the infected tissue out like it's a tumour. Parasitic disease is a big group of weird and wonderful stuff which (mostly) won't respond to antibiotics. There's some nightmare fuel in parasitic diseases! Malaria is a parasitic disease, toxoplasmosis is a parasitic disease (and it rewires your brain a little), lungworms and heartworms are parasites. Some parasites even have the fun catch that, if you kill them quickly/in the wrong way, they release stuff that kills the host anyway. Stargate SG-1 had a go at this concept. And, certainly these days, there are bacterial infections that are almost impossible to cure with antibiotics. Clostridium difficile, Pseudomonas aerogiosa...
Bottom line: Don't use vaccines as a ex machina fix for a disease in scifi. They only work for prevention, and take at least days to work, which is far too slow for most narrative purposes. Gross yourself out reading about unusual parasites or fungal diseases and come up with something more original and more horrifying than a virus.
And vaccinate your kids. And yourself

Hmm, is cowpox still a thing and is that dangerous for kids?
I know the oldest way of vaccinating for smallpox was just getting cowpox so, maybe vaccines from or just straight up exposure to cowpox could help protect kids?
Actually do we still have any smallpox vaccines or was smallpox so thoroughly destroyed that nothing useful is left?
I suppose you mean to replicate the cowpox experiment by Edward Jenner which gave us our first vaccine?

Cowpox does still exist rarely in Europe, typically contracted by domestic cats that roam freely and come in contact with rodents. This experiment however was quite dangerous, and the ethics of it are debated regarding the fact the child likely did not give informed consent and may have been a poor boy taken advantage of for the sake of science. Had the experiment failed, I imagine we would look at it a lot less fondly. It was fortunate for all involved that things went smoothly.
Cowpox, though far less dangerous than smallpox or even monkeypox as it tends to remain in an isolated region of the body where contact was made, is still a dangerous virus. Those who are immunocompromised, indeed including small children, can absolutely still die of infection.
The smallpox vaccine, in a rare example of foresight, does indeed still exist and was stockpiled in the event the virus returned. However, smallpox vaccines were made using live forms of the virus that were weakened or made inert. This has many pros and cons, but the largest con being that your body may overreact fatally or you may receive smallpox from the vaccine in extremely rare circumstances. Less severe but still relevant: even those with proper immunities often experienced adverse effects from the smallpox vaccine. They outweighed the risk of smallpox yes, but there is a reason these vaccines stopped being administered once the virus was eradicated from the wild.
Variations of the vaccine were proposed and created over the years to be safer, but use of a attenuated virus in a vaccine was criticized as it is possible this would not grant strong enough immunity. I believe Canada is the only country to have approved reemergence of the smallpox vaccine, and is currently only administered to certain demographics to my best knowledge. I have not been keeping up to date on the matter.

I'm watching the OnlineMedEd video about TORCH infections and Dustyn said to use "TORCH," not "TORCHeS" because not everything in the latter causes TORCH infections. Whatever. Just remember Toxoplasmosis, Other (i.e., syphilis), RSV, CMV, HSV.
TORCH syndrome causes the baby to have destruction of heme, so they need to make more RBCs outside of bone marrow-> hepatosplenomegaly. Baby will have hemolysis-> jaundice. Baby will have low platelets (thrombocytopenia)-> petechiae.
Usually if a pregnant woman is exposed to a TORCH virus/bug in the first 20 weeks, it causes spontaneous abortion. If it's after 20 weeks, baby probably won't be affected. If baby is affected, they'll present with jaundice, petechiae (also called a "blueberry muffin rash," but it only looks like that if baby is caucasian).
Toxoplasma is a protozoan found in raw meat and cat litter. TVUS shows ventriculomegaly (hydrocephalus) and calcifications in the brain.
MRI shows ring enhancing lesions. Toxoplasmosis tx is pyrimethamine and sulfadiazine.
All pregnant women get screened for syphilis. Get RPR and if that's positive, FTA. Tx for syphilis is penicillin. Even if penicillin allergic, you tx with penicillin. Can do desensitization therapy. If RPR is ever positive, it will always be positive. If the pt has been treated, there will be a small number on the other side of the colon (I believe he was talking about titers).
Congenital syphilis: Saber shins, saddle nose, Hutchinson teeth, snuffles.
Rubella - mom has fever and a rash that starts on the face then goes to the trunk and the limbs. Dx with high affinity IgG. Congenital rubella-> cardiac defects.
Obviously you can't give MMRV vaccine in pregnancy because it's a live attenuated vaccine. A woman should wait 4 weeks at least after getting the vaccine before trying to get pregnant.
CMV shows up on 2nd trimester ultrasound as periventricular calcifications. You can dx with IgG Abs.
HSV primary infection can cause TORCH syndrome. Secondary infection can cause baby to get infected coming through birth canal. You see vesicles and you can diagnose with PCR of the fluid in the vesicles.
If mom has HSV and there's a flare during pregnancy, give acyclovir from week 36 to term. If active lesion at delivery, C-section is warranted. If no C-section, give mom IV acyclovir.

The claim stems from the fact that the strain detected in New York is what is medically termed a "vaccine-derived polio" virus, which has led some to claim that the vaccine for polio is actually what's making people sick in the first place. That is not what the term actually means, though, said British vaccine expert and medical anthropologist Heidi Larson.
"The United States, and many wealthy countries, use a polio injection that contains zero, zilch, nary a drop of live polio virus," wrote Joanne Kenen. "Many lower-income countries, for a variety of economic and logistic reasons, use an oral vaccine that contains a teeny, tiny trace of a weakened, or 'attenuated,' form of the virus. People who are immunized that way (usually babies and young children) are not 'deriving' polio from it. They are getting protection from it. The oral vaccine is extremely safe, Larson said."
"But people who get the oral version do excrete minute traces of the virus, which can reach the water supply and sometimes mutate. Exposure to that mutated version is how people 'derive' polio," said the report. "Those water-borne traces 'don’t infect anyone when people are vaccinated,' Larson told Nightly. 'Where it thrives … is where there’s low vaccination.' During Covid, vaccination programs lagged across the world, for polio and other childhood diseases. The spread of anti-vax sentiments isn’t helping."

I was vaccinated as a child, but if I lived in an area with an outbreak I would get tested to see if I still had antibodies for it.

Last month, German Health Minister and renowned virus pest Karl Lauterbach gave a remarkable interview in which he denounced “exorbitant” pharmaceutical profits, deplored “dismaying” vaccine injuries, and called for the manufacturers to set aside funds for those who have been harmed.
He did so amid a growing wave of reporting on vaccine injuries in the German press – a wave which his statements have now turned into a tsunami. In the weeks since, vaccine injuries and side effects have become the dominant theme of German press coverage on the injections, from local papers to national media.

It’s been a serious shift, the likes of which I’m not sure has unfolded in any other country. To give you a taste of it, I’ve assembled a representative selection of stories from the last eight weeks or so, in roughly reverse chronological order. As you read through them, remember that these are all links to publications read by ordinary people; I’ve excluded all media with overt covid-sceptic associations.

The trend is so dominant that it colours all other reporting on covid and the vaccines. It’s hard to miss the subtle anxiety at work in pieces like THIS one from the Vienna-based Standard, asking whether we’ll have to vaccinate ourselves against corona every year from here on out, or the not-so-veiled notes of hope in ZDF reporting on successful Phase 1 trials of the German nasal vaccine. What’s the big deal about regular vaccination and why should we care about new live attenuated vaccines, if the mRNA jabs were God’s gift to man?
There is clear, encouraging patterns here. The reporting originally surrounded lawsuits brought against the vaccine manufacturers, but has steadily assumed a more general focus. Regional and local papers are carrying a big share of these stories, with major state media playing a supporting role. The publications most popular with German biens pensants, meanwhile, like Süddeutsche Zeitung and Die Zeit and even Frankfurter Allgemeine, are pointedly underrepresented. This is a trend driven from the bottom up by popular interest, and in that it is the opposite of much covid reporting since 2020.
Of course, these stories have always been out there, but until the last few months, enthusiasm for the vaccines was sufficient to suppress them. As with all pandemic policies, mass vaccination exhibits qualities of inertia. Obsession steadily grew through the summer months of 2021, as the jab failed to eradicate covid, and achieved a frenzied peak around December 2021. There was nothing to do about the insane mania and its manifold irrationality back then, and the steady disenchantment with these products will prove just as inexorable.
This isn’t the repudiation I would have chosen. The focus on isolated stories allows the very same press outlets to recycle World Health Organisation propaganda that the vaccines have saved a million European lives, and to repeat uncritically the claims of foolish regulators that “vaccination was the decisive factor” in ending the pandemic. I also have reservations about the emerging discussion of “Post-Vac Syndrome” – not because I doubt that the vaccines have made people sick, but because it seems to be built from the same ill-defined grab-bag of fibromyalgia-adjacent symptoms as long covid. This is a continuation of the media-supported myth that vaccine injuries are merely a subset of the long-term sequelae from covid itself, and it’s a not-so-subtle way to pre-empt any kind of cost-benefit analysis. But, for a pandemic that was also built largely on innumerate anecdotes, and a media that has proven chronically unable to notice basic patterns or count things, perhaps this is the only repudiation that was ever possible.
The pharmaceuticals have made a lot of money, but their mRNA vaccines have failed. The Robert Koch Institut have stopped updating their vaccine dashboard, after almost four months of totally flatlined uptake; one of the foremost mRNA promoters, Bill Gates, has called the jabs a disappointment and compared them unfavourably to masks, of all things; and BioNTech, facing a revenue decline of 70 per cent, have announced an impending return to mRNA-based cancer therapies – their original focus upon their founding in 2008, where they’ve never enjoyed particular success. Perhaps if the vaccinators had proceeded cautiously, limiting their promises and injecting only the most vulnerable on a strictly voluntary basis, they could’ve preserved some future for their doubtful products. Instead, they oversold and over-administered their snake oil, and two years later most people have decided they don’t like it very much.
