Today, Amazon announced it has agreed to acquire US-based, tech-centered primary care organization One Medical for $3.9 billion. This could potentially help the Big Tech firm expand its healthcare footprint.
One Medical is a human-centered, technology-powered national primary care organization on a mission to make quality care more affordable, accessible, and enjoyable through a seamless combination of in-person, digital, and virtual care services that are convenient to where people work, shop, and live.
Amazon will acquire One Medical for $18 per share in an all-cash transaction valued at approximately $3.9 billion, including One Medical’s net debt. Completion of the transaction is subject to customary closing conditions, including approval by One Medical's shareholders and regulatory approval. On completion, Amir Dan Rubin will remain as CEO of One Medical.

on the topic of diy hrt as ive seen it going around the dash a few times, i want to offer what i can in terms of help as i got on hrt in secret while still living with my parents. i cant give advice for paths i didnt take (getting the hrt itself online) but i can detail the process i experienced so you know what to expect if you pursue.
i highly recommend seeing if there is a planned parenthood in your area and if it offers gender affirming hormone therapy. i had a couple things that made this an option for me that may be barriers for others - i was over 18, and they do not offer informed consent to minors. i also had a job that allowed me to pay for the visit and the prescription. lastly i forced myself to learn how to drive so i could get to the planned parenthood and the pharmacy.
for cost, i recommend checking beforehand if planned parenthood accepts your insurance. They did not accept mine so i had to pay out of pocket for the appointment. Luckily they have a sliding scale system that charges based on your monthly income. The amount changes (and this may vary by region as well), so you can call them for a cost estimate. As someone going for testosterone, my visits have cost between $39 and $63 out of pocket.
For me getting HRT occurred in 2 phases: A telehealth call where they told me about the effects of HRT and then had me digitally sign an informed consent form. And then an in-person visit to my local PP office to get my blood pressure and hemoglobin (finger prick) checked. I then walked out with a T prescription.
For bloodwork, planned parenthood offers this service as well but it was a “strongly recommended but not required” option for me. I did eventually get my levels tested through PP but it cost me ~$40 because of lack of insurance. i later learned i could get my bloodwork done for free at quest diagnostics, for which PP sent in an order that automatically went into quest’s system and all i had to do was make an appointment online filed under “all other tests” … in my area quest can be found in grocery stores. I recommend looking into quest for bloodwork if your insurance would cover it.
Estrogen and progesterone are not controlled substances (often used in birth control), but testosterone is since it can be abused as a steroid. so if you are pursuing T this may present challenges - in my experience it makes it hard for me to change pharmacies without calling planned parenthood first, and you need to present ID when picking it up.
At first i swore by gel and not injectable because i was afraid of accessing needles, and i used goodrx coupons to get gel at a reasonable price. I do not recommend this as it is not sustainable, goodrx coupons are great but always changing. Unfortunately if you do diy hrt you are probably going to be doing injections. i tried for several months to get gel in a sustainable way but the only way i was able to do this was when i had insurance that would cover it (which i since lost access to).
You can get needles and syringes in bulk (100 for like $10) on amazon. Your prescription will probably include an order of needles and syringes but pharmacies are generally not awesome to get them through, because they tend to cost more and be out of stock…. for subq i use 25g 5/8” needles to INJECT, and 18g 1.5” needles to DRAW. i use 1 ml syringes with a LUER LOCK. make sure the needles and syringes have the same locking mechanism so they can connect to each other. the alternative to a luer lock is a slip tip, which you do not want.
Even though on the bottle it says INTRAMUSCULAR USE ONLY, i and many other trans people do subcutaneous as the needles are smaller and less painful. “subq” injects into the layer of fat beneath the skin and releases the hormones slower over time. I personally inject into my stomach in the area below my belly button - i pinch the fat and inject at a 90° angle. there are detailed videos on how to do a subcutaneous self injection on youtube, and here is planned parenthood's guide.
if you are scared of needles, i am too - i used to black out when i had to get a shot. i can say that subcutaneous rarely hurts and its definitely tolerable to me. i know its freaky to inject into your stomach but the fat layers there make it pretty painless. if you pinch your fat your nerves focus on the pressure your fingers are making, and as long as you hold the needle like a dart and inject quickly you barely notice it.
For questions i recommend looking on reddit. there are many subreddits dedicated to DIY hrt as well as logistical questions in the context of navigating pharmacies and doctors. And, i am always happy to offer what help i can.

So, in Brazilian healthcare news, the Bolsonaro government has made budget cuts of 60% to the Health Ministry, affecting programs of HIV/AIDS treatment, resources for medical residencies, indigenous healthcare, and other programs.

Implementation and Operation of Digital Health and Telehealth in the Unified Health System (the country's universal healthcare network) lost R$ 26 million (~US$ 5 million);
Health Care for Riverside Populations and Remote Areas in the Amazon Region lost R$ 10 million (~US$ 1.92 million);
Implementation of Policies for Stork Network and Implementation of Policies for the Maternal and Child Care Network lost R$ 28 million (~US$ 5.38 million).
The cuts have been implemented in order to save money for the Secret Budget, priority expenses for a select group of lawmakers, included by way of a little-understood mechanism known as “rapporteur-designated budgetary grants” (in simplified terms, it works as a "purchase" of legislative support by the executive branch with money for amendments).

Soggy Solstice
Good thing I didn’t pan to celebrate the summer solstice by dancing nekkid in a meadow or anything because it rained all day.  I’m not complaining about that one bit because we really needed the rain.  It was a lovely, steady, restful rain - more than a drizzle less than a deluge.  I know the gardens were slurping up the water. It almost seems like the very best way to welcome summer, by helping green things flourish. I like it. Because it was a rainy day, I spent most of my time inside.  I did do a little porch sittin’ with a good book for a while, but most of my time was in the house.  B-O-R-I-N-G. All of that time on my hands and I didn’t get a whole lot done. I puttered. I did make a pot of vegetable soup, did one load of laundry, tidied a bit...and then wasted the day. Oh wait! It wasn’t all wasted - I did a physical therapy session!  My insurance offers telehealth PT so I thought I’d take advantage of it for my issues.  I signed up, filled out a long form with my ailments, then had a phone call with a physical therapist who set up my program, and they shipped me a kit.  Inside the kit was a tablet, some motion sensors that I strap on my chest and my hips, and chargers for all of it.  Today was my first session and it was great, I could feel the difference, but....I can’t keep the damn cat from batting at the screen and moving me on to the next exercise.  Phoebe was fascinated with the lady on the screen and no matter where I moved it the floor the counter, whatever, she was right there.

I guess I’ll have to do it in a bedroom with the door locked.  Still, it felt great and if it works I’ll be very happy.  Even better, my insurance considers this preventative for chronic pain management so it’s FREE.  Yippee! Just wait, I’ll be as good as new in no time. Look for me on the streets.

I don’t wear prescription glasses, but my old lady eyes do get tired by the end of the day.  I read in bed so I have a wardrobe of cheap eyeglasses nearby. The last time I had my peepers checked the doctor sort of rolled his eyes at that, but didn’t scold me.  I’m terrified of needing glasses. They’re expensive and I know I’d sit on them, drop them, or lose them.  I mean, I guess the idea is to keep them on your face so none of that happens, but I’m not taking any chances. I’ll keep buying cute specs at Target and Amazon until I can’t anymore.

I’ll tell you what I did see - some cute crafts at Artisans on the Choptank last week. I was on sort of a scouting mission to see if anyone was selling wares similar to what I make.  Nope.  The next event is in mid-September so I might give it a whirl.  It’s a really cool chance for local makers to show and sell their stuff.  It’s down by the water at Wharves of the Choptank Visitor Center and there’s usually live music and a couple food trucks.  I really liked what I saw there -  a nice variety and all of it really interesting and well-made.  I met a super fun lady who had a booth and we chatted a lot.  I wanted to take her home with me, but she probably just wanted to make a sale.  She did.  I bought a keychain made of wood beads and a tassel. The disk it what sold it for me.

Yeah, I’m too old for an Eminem joke but it still makes me giggle. I think the fact that I have a loyalty tag for my local grocery and my library card on that keychain makes me a total gangsta.

My jokes aren’t even current. How old is that song?  Fifteen, twenty? Yikes. It’s time for me to go upstairs and take a bath.  I’ll grab a pair of my basket glasses and read a good book.  I picked up two at the library yesterday, so I have high hopes.
Take care of yourselves, sweethearts. Do something that’s good for your soul every day, even if it’s just for ten minutes. Those minutes add up. I read poetry. Sometimes I come across a poem that hits right in the heart and I think about it all day. For you it might be turning your face to the sunshine, taking a dance break, or making a quick sketch of something.  Okay, sure, it could also be exercise,  a quick ten minutes walk used to revive me mid-afternoon at work.  Sometimes just a lap around the building is enough to recharge.  You get the idea - take care of yourself, body and spirit.   Off to a bubble bath I’ll see you soon. I am in trouble, financially. This is the first time I've clearly expressed that (I think). I've always known, since my business failed, that I was in trouble. I saw the trouble coming 6 years ago. But I was in denial. I thought I could recover from it even though I had no plans.
Well, I can't say I was totally in denial because I knew I would have to start working a 9-to-5 again. But I was in denial about the fact that I would actually do it if it came down to that.
I've been able to hold off from working a 9-to-5 because I have savings. But my savings has been steadily dwindling. The time to get a 9-to-5 has come and gone. But still, I can't bring myself to do it...
I want to hold on to my freedom but I may risk losing all my savings to do that. It could take up to 2 years to grow my small businesses and side hustles to where they would make enough to cover my monthly expenses and credit card payments. By that time, my savings will be gone.
I hate this position I'm in. Nearly everything I worked for will be gone except for the my primary residence, rental property, and the mutual fund.
There are so many reasons why I'm trying to hold off on not working a 9-to-5. I want to make the right decision for myself...
I'm not at all a successful sex worker. I hardly get any bookings. The bookings that I get from my 2 regulars (and a 3rd I can hardly call a regular because he can only afford once or twice a year) make up only enough for groceries or a few bills in the grand scheme of things. But the effort I put into sex work is still worth it because it's better than having no income.
I'm in a difficult position. I keep waiting and hoping that something new and big will come up that will save me from this predicament. I can't keep betting on something I'm not sure will happen (for instance, I'm fairly certain a wealthy benefactor will come my way... somehow)...
Meanwhile, my brother hiked up his britches and took a job at an Amazon fulfillment because he knows he needs to make rent. Those assholes have their employees on their feet for 9.5 hrs a day, not including two 30-minute breaks for $15.00 an hour. But my brother is doing what he needs to do. The good news is that he only needs the fulfillment job until he starts a new part-time job he got, working from home. He's quitting AMZN in about a month...
But my health isn't there yet. If I took an Amazon fulfillment job, I would be slowing down the progress I've been making with my health. I don't even have the strength to wake up at 6:30AM every day, stand on my feet and then come back home at 6:30PM... F*ck nah. I'm definitely not there yet.
I just don't want to take up anything that would thwart my plans for financial independence. And it's very difficult to be financially independent unless you're building something of your own no matter what kind of hustle it is...
God, I hate this life... I wish I could share all of these troubles with Nigerian boy, Dexter. He understands would it means to be a failed entrepreneur. But I can never speak to him again. And I've gone through about four trash telehealth therapists. I don't feel like searching for one again. Maybe at a later time...
This post was good therapy itself. Shout out to all the women and black women that are doing their damn best to survive in a world that hates women and hates the working class. I know we can work for and get everything that we deserve.
