Bottom Surgery Update
It's been a while since I've had bottom surgery, but I know many people don't follow my Tumblr, so I wasn't super worried about posting. Below I mention anatomy terms, so if that triggers you, please stop reading.
In July 2021, I got metodioplasty, with vaginectomy, full hysterectomy, and urethral lengthening.
In December 2021, I had stage 2, which was the testicular implants.
Overall, I'm fairly happy with my results but there's definitely a few things I'd change.
For one, I guess the method my surgeon used causes a fluid build up of vaginal fluid. This can cause immense amounts of pain. It happened about 5 times now and it leaks out of my urethra eventually, but man, it sucks.
Due to my *ahem* size, I also probably would not have opted for the testicular implants. My junk is basically buried and very difficult to access. I was expecting to not be able to STP due to my growth, but I didn't realize how much of a hindrance the implants would be. Even if I don't get phalloplasty, I don't think I'd get them removed. As annoying as they are, it is nice to have balls.
I'm looking at getting phalloplasty still, but at this point I still feel like the potential risks don't outweigh the pros. Also I'm a giant baby and am scared of surgery and using skin donors.

Bottom surgery journal 231107
There is way too little talk about bottom surgery, so I thought I am going to add a bit of my own ongoing procedure. I had the first stage of a metodioplasty done a little over a month ago. If I can be honest, I thought the whole thing was a single surgery, if things went well. I guess it really depends on the country and surgeon, and considering how nice results this surgeon makes I don't really mind. But it is a bit of a bummer to get this info on the same day after years of anticipation and whatnot.
Anyway. I had a urethral lengthening and on the upside; no graft from my mouth or anywhere else that's not the crotch area. On the downside; catheter. I had mine for 6 weeks and it was hell from start to finish. The pain. The accidents. The hindered hygiene. Oh god the UTI. I finally got it removed last week, but then next nightmare began; dilation. That shit is scary. Not only does it hurt, but if you go too far you might poke through the urethra and enter the vaginal cavity. And this needs to be done every day for three months...

for real!!! i think like. its just so beyond rude to need to spread negativity onto ppls results like im sorry ur bitter that u cant have a cis man’s penis magicaly zapped onto you but like. phallo doesnt look bad at all, and i dont mean to objectify my fellow tboys out there but. god damn, yknow! i do love a man made penis!! putting phallo or metodioplasty up against cis penisises isnt rlly the point? and even then- it is in fact, possible to make a ‘cis passing’ penis, have u seen medical tattooing? like i just. do not get why so many esp. young tguys feel like they need to comment on how ugly phallo is when its a very hard decision to make in the first place. its a crazy expensive surgery that takes several stages over several years and is, very physically taxing- only a very select amount of guys can even get it due to how huge a surgery it is and how much you have to devote to it. im like. always back and forth on what type of bottom surgery im gonna get. im leaning towards metoidoplasty so if i want, i can still get phallo later on but i remember as a barely out tboy at like. age 13 being so scared to admit i wanted bottom surgery bc all my trans friends just shit on it all the time. its def internalized transphobia above anything else but its just a rlly nasty attitude to have and these days i block anyonw who says shit abt how phallo just isnt there yet when its taken. a long time for the surgery to get to this point and its saved so many lives, its just not fair to say that it’s not an effective or succesful surgery or that is ugly or unrealistic or doesnt look like a ‘real’ penis

Hello. I know that you are a bit cautious regarding surgeries (hysterectomy, mastectomy etc.). But the recent rise in anti-trans movements that advocate for a change of laws is terrifying. I already had the mastectomy (last October) and I recovered very well. I'm also on the waiting list for Hysterectomy and Metaidoioplasty (already got it insurance covered as well).
I am a bit scared of the surgery, because of possible complications but I do know that someday I definitely want to take that risk. But if I get the call tomorrow for an appointment I would only take it because I'm scared there won't be a chance to get it otherwise (for example if the laws change (I live in Germany)).
What do you think about that? Do you think it's a rational fear to have? Because if that really would happen, like in some parts of the US, that would mean a forced detransition.
Did you ever think about that? To "get it done" just so the progress (HRT) so far cannot be taken away?
To be fair I'm cautious of any surgery. I think in general doctors kind of downplay it a bit, but maybe it's also because I've had a very bad experience.
I think it's rational to fear your rights disappearing because there is a trend. At the same time, for example for hysterectomy, while I fully understand and support the idea of having metodioplasty (which I think is the best bottom surgery atm available to us), I also wonder what would happen if, say, testosterone was no available anymore. There is a shortage of T these days here and so it's already pretty stressful, I wouldn't want to know how stressful it would be if the alternative was no hormone at all.
I plan to eventually get those surgeries too though, but I decided to lose weight beforehand so that I would reduce the stress from it on my body and eventual risks.
However I'm not sure progress regarding HRT can really be reversed. We already know that hair and voice would probably still be similar, the rest could change a bit but most detrans women still struggle to pass as women while trying. So... I'm not really worried about forced detransition.
I would caution you not to rush it even if you are scared.

is it true that to get a hysterectomy on the nhs for gender reasons it must be part of a metodioplasty or phalloplasty?
i really really want a hysterectomy but am not sure about bottom surgery yet
Hi Anon,
You can be referred for a hysto without having to have bottom surgery.
If you were having bottom surgery it is easier to just have your hysto done at the same time, but if you are not sure, you can have your hysto done then wait until you know if you want bottom surgery and then go ahead at a later date.

1. obviously T n stuff, and utero-begone-o, and i think maybe metodioplasty is Required for some other steps here???, but after that,
2. phalloplasty where they use the skin from my chest, because, like, it really seems logical, youre gonna be cutting it off anyway
2. but an addendum: they are giving me a foreskin im gonna make them do this. ive always wanted one. i think theyre neat
3. THEN removal of the chesticals. i dont care if the scar's kind of wonky cuz im planning on a big chest tattoo anyway

ya’ll.... i want a metodioplasty i’m pretty sure. i keep researching it and getting excited about it. the simplest version of the procedure (simply separating the penile tissue from the inner labia) seems so attainable, and like it would make such a difference and would also be very affordable and relatively easy to heal from, speaking as someone who has never had real surgery before... i’m excited and also scared to be at least mentally going down this path. i think i want to get a consultation

I originally had a blogspot account where I was documenting my lower surgery experience but I found the formatting clunky and uncomfortable, and I eventually lost interest and felt too fatigued to continue updating so I got rid of it. But since then, lately I’ve been feeling weird about not having that blog anymore since there’s so little information out there regarding lower surgery for transsexual men - so I figured, why not, maybe using a format I’m more accustomed to, I can actually document my experiences here. For my own reference and others.
I’m 21 at the time of writing this, and I had full metodioplasty (meta) w/ UL, scrotoplasty w/ testicular implants, and vaginectomy with Dr. Meltzer in Scottsdale, AZ. My first stage was slighly over one year ago, May 23, 2016, where I had the initial meta done with UL and vaginectomy, where vaginal mucosa tissue was used for the primary urethral lengthening. My second stage took place on December 15, 2016, where I had scrotoplasty completed with testicular expanders, which remained for the following six months until I had my expanders taken out and permanent implants placed, on June 7th, 2017.
I am more than happy to share my experiences and hopefully documenting them will also be useful for me, so when the time comes I’ll be updating this soon.

I hope you guys can give me some advice on this topic, because I'm having quite a hard time finding some more recent information myself. What is the better option, meta or phalloplasty? I know, it depends... but being able to penetrate my partner would be really important to me, and also to still feel something and be able to orgasm. But I've read that in the reconstructed penis you wouldn't have any erotic sensation, it wouldn't be any more intense than the feeling of someone touching your arm.
Zak: As you mention, it depends. There are pros and cons to each option and what is best for you depends on what factors are most important to you (this video gives a good overview of the differences between the procedures). There’s some great information about bottom surgery out there, for instance this collection of resources on metoidioplasty and this one on phalloplasty. There are also people who have blogged about their bottom surgery experiences (these links are NSFW), for instance pansy-boy has blogged about their experiences with the meta, while this person has chronicled their experience with the phallo. Some people, like kahild, have had the meta first and then the phallo and have discussed both procedures on their blogs. There are also blogs like FTMphalloplasty that provide more general information. You can also see pictures of results on transbucket and on surgery yahoo groups. There’s also an excellent book out there called Hung Jury that I’d encourage anyone considering bottom surgery to read. 
Unfortunately, there’s also a lot of misinformation out there as well. For instance, erotic sensation is possible with the phalloplasty. For most people, erotic sensation and ability to orgasm stay the same, though for some people it decreases and for other people it actually increases. This summer at the Philly Trans Health Conference I attended the phalloplasty session and not only did the results I saw look phenomenal but the people who shared their results reported that they were incredibly satisfied with their surgeries. 
I know this isn’t a really clear answer to your question, but the fact is that this is an incredibly personal decision that depends a lot on what you would be most comfortable with and want. There are a lot of factors to consider: cost, scarring, size, appearance, etc. I’d recommend looking at results and talking to people who have undergone the different types of surgery (beyond just phallo and meta, there are different methods for each that produce different results).

Heey. Hope this ask is okay for you. It's 2021 and I'm wondering where our penis-grow pills, creams or whatever are hidden. Surgery is so scary man 😭 Do you think we'll ever get some alternative to arm-tissue-phallo? I'm already fearing the metoidioplasty but from there I wish we'd had some enlarging methods... Is that realistic or just wishful thinking? And do you think the metoidioplasty is worth the risks? I heard they are very minimal but some complications can be serious.

Yes this ask is okay for me but I think asking a doctor about it would be more interesting. Please research and ask doctors. :)
I can't tell you which is worthwhile or which isn't. I am very scared of surgery too so I haven't done anything and I'm not sure I would. It's a lot of trauma for the body so it has to be thought thoroughly and it's better to have a support system around you to help you in recovery.
As for metodioplasty, I think you can do a phalloplasty after getting a meta if you want to. So options are open, probably will be open in the future when they come up with better ways to construct a penis.
Complications aren't really common for meta except if you do a urethral lengthening which is what would make you able to pee from your peen.

Haven't checked it for a while but the dude working on it has a prototype. And i don't know if it's still the case but at some point he said we could probably put little magnet under the skin to magnetically attach the prothesis and stuff. It's all very smart, so I'd recommend checking that out!

I'm 2 years on T and soon to have my first GIC appointment. I'm hoping to be referred for top surgery and then metodioplasty. Roughly how many extra gender therapy appointments does it take to get meta and how much longer after top surgery it would take? Are there any specific requirements before they will refer you for meta?
At least two independent clinicians must be in agreement before a referral for lower surgery.  Depending on the GIC these might simply be two difference clinicians at the GIC, they might refer you for an appointment at a nearby GIC, or make a referral to someone outside of the GIC system for the second opinion.  Depending on the GIC they may wish to wait for you to have had your top surgery before starting this process, or they might be happy to begin getting your lower surgery opinions while you’re waiting for top surgery.  This might also depend on the waiting time for your top surgeon as lower surgery referrals are typically only valid for a year and you will also need to have recovered completely from top surgery first.
In terms of other requirements, they are only that you have been living as male for at least one to two years and have been on testosterone for at least 12 months.  From your message it sounds as though you’ve already met these requirements.
