For World Poetry Day I want to share with you one of the best songs ever (songs....poems...sue me!).
It´s old, it´s rare, it´s dramatic.
The Egidius Song
Y´all have been sleeping on this one!
This is an elegy, a song of sorrow for a friend who died. Unfortunately its author is unknown. It had been theorized to be Jan Moritoen, a Bruges fur trader and poet whose paper trail ends in 1418. The song was found in the so-called Gruuthuuse-manuscript, dating from the 14th Century (written between 1395 and 1408). It was not written by the name-giver of the manuscript, Lodewijk van Gruuthuuse, this Flemish diplomat and patrician collected manuscripts in a certain illuminated style. His collection – this manuscript –, however, is an important source for Dutch literature. It holds no less than 147 songs, 18 poems and 7 prayers. Today the Gruuthuuse museum can be visited in Bruges, the headquarter of the Gruuthuuse family. The sale of the OG manuscript (which had been passed down privately until 2007) to the Netherlands caused a lot of uproar in Belgium from people considering it a scandal and ´theft´ of Belgo-Flemish heritage. None of the Dutch teachers I´ve had seems to be over it.
It is important because it is one of the oldest songs we know the full lyrics and tune and it has some of the most timeless and emotional messages in the whole manuscript. The music is writte down with a dot-line system. This means there are several variations possible (length of note 2b song etc) but the general melody is fixed. You can see the lyrics and texts below, plus some translations:

The book was probs made as guide for people who already knew the songs. These poets and elites would come together and recite poetry and make merry (a la dead poets society). Most of the songs and poems are not found in any other sources which makes one think on how many must have been lost through time.
Who was Egidius is uncertain. Some think him to be Gillis Honin (Egidius being the latin form of Gillis) who died unexpectedly at a young age on 8th october 1385 and is buried in the Saint-Walburga church in Bruges. His death had an unfortunate timing for his business had eloped and he was in dire finacial (and reputational?) straits. If this is true and the poet is indeed Jan Moritoen they could have known each other for they resided in the same elite social circles.
Some interpret the song as an ode to friendship or a bromance cut short too soon, other see it as a tragic romantic song. The use of companion (geshelle) instead of lover or friend makes it difficult to interpret. The ´choosing death above life´ does most likely not mean suicide since the song places Egidius in Heaven and they did not believe this to be possible for people who comitted suicide at that time.
Regardless of who they were, the depth of their feelings and the cause of death, the song is relatable and dramatic: you choose death, left me behind to live and suffer/ keep a seat free for me next to you in the beyond/our bond was good but apparently had to die/ I still have suffer pain in this world!/...

WIERINGERWERF – Afgelopen week zijn de sgraffito’s in de Cultuurschuur bijgewerkt door Ted Verhulst, Anneke Lont en Janneke de Regt. Op de balustrade zijn een tiental sgraffito’s van de hand van kunstenaar Jan Goeting en stammen uit 1955. Deze werken verbeelden de ontwikkeling van de Wieringermeer van 1930 tot en met 1955.

168. Van Sint Jans cruyt
Sint Janskruid, Hypericum perforatum
Als je een blaadje van het Sint Janskruid plukt en het naar de zon houdt, zie je waarom het 'perforatum' wordt genoemd. Het lijkt vol kleine gaatjes te zitten. Op die plekjes stapelt de plant zijn oliën op! En laat die nu een wondermiddel zijn. Bovendien bloeit dit bijna heilige plantje rond de naamdag van Johannes de Doper, of Sint-Jan. Voila!
Dodoens schrijft het volgende: 'Die bladeren ghestooten/ zijn goet gheleyt op die verbrantheyt/ Inderdaad maak je van Sint-Janskruid een geweldige after-sun-olie. Gewoon het kruid in fijne olie macereren. Je vind zulke oliën en zalven tegenwoordig ook makkelijk in de bio-winkel. Ook andere huidaandoeningen en wonden genezen ermee.
Je vindt het echter nog meer in pilvorm. Sint-Janskruid bevat namelijk hypericinen. Die stoffen remmen de heropname van serotonine, dopamine en noradrenaline. Zo blijven die oppeppende hormoontjes langer in je bloedbaan circuleren. Een anti-depressivum dus! Bij milde depressie en slapeloosheid werkt het kruid goed.
Wel opletten, de plant bevat ook oliën die de huid extreem lichtgevoelig maken. Voor een winterdepressie vormt dat geen probleem, met de huidige zomertoestand wordt het toch opletten! Ik herinner me een zeilvakantie tijdens een dipje... dat vergeet ik nooit meer!
Dodoens heeft nog meer nuttige toepassingen die wij intussen niet meer gebruiken: koortswering en reumatische heup- en rugpijn.
Heilige Sint-Jansolie maak je zo:
Vul een glazen pot met Sint-Janskruid. giet hierop zonnebloemolie tot het onderstaat. Bind een doekje over de opening. Laat 4 weken in de zon staan tot de olie rood wordt. (Dat wijst volgens sommige gelovigen op het verhaal dat het kruid zou zijn ontstaan uit het bloed van Johannes de Doper!)
Klaar voor gebruik! Je kan eventueel het kruid er nadien uit zeven.

Η αποφρακτική υπνική άπνοια είναι μια συχνή πάθηση που συναντάται στο 13% των ανδρών και στο 6% των γυναικών στις Ηνωμένες Πολιτείες της Αμερικής.1 Η θεραπεία εκλογής θεωρείται η αναπνευστική συσκευή CPAP (Continuous Positive Airway Pressure), η οποία όμως εμφανίζει χαμηλή συμμόρφωση που κυμαίνεται από 50 έως 70%.2 Για τους ασθενείς με αποφρακτική υπνική άπνοια οι οποίοι αδυνατούν να χρησιμοποιήσουν ��ην συσκευή CPAP έχει προκ��ψει την τελευταία πενταετία μια καινούργια θεραπεία: η βηματοδότηση του υπογλώσσιου νεύρου με το σύστημα Inspire UAS.3
Η προσπάθεια για την δημιουργία ενός βηματοδότη του υπογλώσσιου νεύρου ξεκινά από τα τέλη της δεκαετίας του 1990 οπότε εμφυτεύθη��αν οι πρώτοι 8 ασθενείς.4 Με την τεχνολογική εξέλιξη του συστήματος διέγερσης του υπογλώσσιου νεύρου φτάνουμε μερικά χρόνια αργότερα, στην πολυκεντρική κλινική μελέτη STAR (Stimulation Therapy for Apnea Reduction). Σε αυτήν την μελέτη και συγκεκριμένα στο Sint Lucas Andreas Ziekenhuis στο Άμστερνταμ όπου εμφυτεύθηκαν 25 ασθενείς συμμετείχε ο ιατρός ΩΡΛ Κουτσουρελάκης Ιωάννης το 2011. Η δημοσίευση των αποτελεσμάτων της κλινικής μελέτης στο New England Journal of Medicine το 2014 απέδειξαν την αποτελεσματικότητα της χρήσης του βηματοδότη της εταιρείας Inspire σε ασθενείς με μέτριας προς σοβαρής βαρύτητας σύνδρομο αποφρακτικής υπνικής άπνοιας.3 Πράγματι, σε ένα σύνολο 126 ασθενών στους οποίους εμφυτεύθηκε ο βηματοδότης του υπογλώσσιου νεύρου, η διάμεση τιμή του δείκτη απνοιών/υποπνοιών μειώθηκε από 29.3 σε 9 επεισόδια την ώρα και η διάμεση τιμή του δείκτη αποκορεσμών μειώθηκε από 25.4 σε 7.4 επεισόδια την ώρα. Εξίσου σημαντική ήταν η βελτίωση στον υποκειμενικό δείκτη υπνηλίας (Epworth Sleepiness Scale Score) του οποίου η διάμεση τιμή μειώθηκε από το 11.0 σε 6.0, καθώς και στην κλίμακα ποιότητας ζωής (Functional Outcomes of Sleep Questionnaire) της οποίας η διάμεση τιμή αυξήθηκε από 14.6 σε 18.2.3 Παρενέργειες ή επιπλοκές από την θεραπεία ήταν ασυνήθεις.
Με οδηγό τα ισχυρά δεδομένα της δημοσίευσης του New England Journal of Medicine, ο Αμερικάνικος Φορέας Τροφίμων και Φαρμάκων (FDA) ενέκρινε προς κλινική χρήση το βηματοδότη Inspire II UAS τον Απρίλιο του 2014 (https://aasm.org/fda-approves-inspire-upper-airway-stimulation-therapy-for-sleep-apnea/).
Πρόσφατα, δημοσιεύθηκαν τα αποτελέσματα της 5-ετούς παρακολούθησης των ασθενών της μελέτης STAR, τα οποία αποδεικνύουν ότι τα ευεργετικά αποτελέσματα διατηρούνται στο πέρασμα του χρόνου τόσο ως προς τον δείκτη απνοιών/υποπνοιών όσο κι ως προς τους δείκτες ESS και FOSQ.5
Όπως εξηγεί ο ιατρός ΩΡΛ Κουτσουρελάκης Ιωάννης, ο αριθμός των ασθενών που έχουν εμφυτευθεί παγκοσμίως έφτασε τον Απρίλιο του 2019 τις 5000. Τα χαρακτηριστικά τους καταχωρούνται στην πολυκεντρική μελέτη ADHERE, της οποίας τα πρώτα αποτελέσματα δημοσιεύτηκαν στο European Respiratory Journal τον Ιανουάριο του 2019.6 Έτσι, σε 508 ασθενείς στους οποίους εμφυτεύθηκε ο βηματοδότης του υπογλώσσιου νεύρου στις Ηνωμένες Πολιτείες της Αμερικής και την Γερμανία, η διάμεση τιμή του δείκτη των απνοιών/υποπνοιών μειώθηκε στους 12 μήνες από 34 σε 7 επεισόδια ανά ώρα ύπνου ενώ η διάμεση τιμή της κλίμακας υπνηλίας ESS μειώθηκε στους 12 μήνες από 12 σε 7. Είναι ενδιαφέρον ότι η αύξηση της ηλικίας και η μείωση του δείκτη μάζας σώματος ήταν οι παράγοντες που αύξαναν την πιθανότητα ανταπόκρισης στην βηματοδότηση του υπογλωσσίου νεύρου. Παρόμοια είναι τα αποτελέσματα που προκύπτουν από τους ασθενείς που έχουν εμφυτευθεί στο Weill Cornell Medical College της Νέας Υόρκης στο οποίο εργάζεται ο ιατρός ΩΡΛ Κουτσουρελάκης Ιωάννης.
Το κόστος του βηματοδότη παρότι πολύ υψηλό (~20000 USD) καλύπτεται από τα ασφαλιστικά ταμεία στην Αμερική (Medicaire, Blue Cross Blue Shield, Aetna, κ.α) αλλά και στην Ευρώπη (Γερμανία, Ελβετία, Ολλανδία). Σύμφωνα με τον ιατρό ΩΡΛ Κουτσουρελάκη Ιωάννη αναμένεται να ξεκινήσει η εφαρμογή του στην Ελλάδα εντός του 2019.
