Jules McAllister is my Courier Six for all newcomers and long-time followers who may have forgotten! Yes I have other Couriers (hi Miri, hi Faye, hi Sabine). Yes Jules is the one with the massive AU. She's a 5'3" Aries menace and I cherish her so deeply. She's never made a good decision in her life.
Anyways the song I have always, always most strongly associated with her is "When You Were Young" by The Killers.
It's the religious imagery and sense of heartache. It's the man coming in that she thinks will save her-- in the song it's a romantic interest, for Jules it's Caesar, taking the place, in her mind, of the father she didn't have growing up. It's the appearance of salvation from her addiction and her depression and rage at a system that's left her behind, with smooth words that tell her she's special, she's different, she's fit to join the Legion because she's not like the other women. It's the sense in the song, and the knowing in Jules' story, that this ends poorly. It's the heartache and longing for something better, for an escape, for someone to love you, and finding all of that in exactly the wrong way, place and person.
(This is the part where I disclaim that while yes I play and cosplay Legion characters sometimes no I don't think they're right of course not oh my god writing bad guys ain't the same as agreeing with 'em aaaaaaaaaa can you tell how many times people on this hellsite have given me shit for this)
Jules ends up crucified for her efforts in the Legion's victory over New Vegas. My good friend @randamhajile and her OC Lexington took it upon themselves to a) pull her sorry ass off the cross b) try to Redeem Her (questionable results) and c) remember that massive AU I referenced? It's this one! They go on adventures and there's, like, whole fanfics for it, I've put it on my resume. The tag for your perusal!
Sylvie Caron is my new rogue I am playing in a D&D game! She is a grave digger by trade, in a world that heavily frowns upon it, as part of a group called the Tallymen. Post-mortem care, on Papatūānuku, consists of the deceased's body and spirit being taken to the River of Souls to be reincarnated-- the Tallymen disagree with this being the best course of action. They, and Sylvie in turn, believe that it's better to bury bodies far away from the river, and wouldn't hesitate to resurrect someone if asked/hired for it. It is... a bit of a taboo job.
So Sylvie's song is "Bottom of the River" by Delta Rae.
I'll admit, I don't know Sylvie extremely well yet; I just started playing her last Saturday. But the ominous atmosphere, the work-song rhythm, and the river imagery especially, with the River of Souls contrasting with the toxic River Aspic of Sylvie's home city, ring true for her.
Now Katya Fyodorova-- long-time viewers will remember her as Grief, my pandemic game scourge aasimar shadow sorcerer and the love of my life and my icon, literally, here and on Twitter. She's my estranged Russian mafia brat. My chiaroscuro hot mess. My nightmare child sweet cheese light of my life who's going to royally fuck up hers. I hold her so so gently. And then snap her like a glowstick. Long-time viewers may also recall seeing her referred to as Katya Vorakh; the name change is intentional and plot-relevant lol. I started playing her in a game right at the beginning of the pandemic that fizzled out, and me being unreasonably attached to her I continue to play her on my play-by-post Westmarches-style Discord server. She's doing great (?)! Has a cousin, a few friends, a cat, a hot goth jock elf boyfriend, aside from the fact that a devil recently tore apart her apartment and she has two nemesii and her shitty ex-patron she's just recently found out is haranguing a friend and is Fucking Vecna it's great! Don't look at the six-armed angel in the corner.
Anyways I have so, so many songs for Katya, my beloved, but the one that's hitting me hardest lately is "A House In Nebraska" by Ethel Cain.
Yes I've been listening to Preacher's Daughter on a solid loop for like two and a half months. No I'm not okay. Don't worry about it. But for Katya! For Katya, this calls back to a key part of her backstory, the sudden and violent death of her best friend Artemy Vorakh, almost ten years ago at this point in her life. She still blames herself for it-- if he wasn't her friend, he never would have been caught in the mafia crossfire that ended his life-- but at the same time... she's selfish. She's not a great person. She wouldn't change the fact that they were friends. Her mourning him has been a quieter part of herself lately, as she's sloooooowly let down her walls and let herself make friends and fall in love, but it's still very much there. Like, this is the woman who dressed in mourning blacks and a widow's veil and called herself Grief and didn't show her face for years after Artemy's death. They were never romantically involved. There's a lot, a lot to their relationship-- she thought she was in love with him for six or seven years and never said anything, he stuck with her despite how difficult she frankly is, he was the only person in her life she trusted not to manipulate her for a long, long time, and they had loose (very loose) plans to run away from Baldur's Gate together and live anonymously. (Katya would have hated this. Katya was not in love with him. Katya confused love with a whole lot of things and has been terrified, terrified to put any kind of label on how she feels re: aforementioned hot goth jock elf beyond "I'm very fond of him".)
"Family Tree" and "Ptolemaea" also make an appearance on a Katya/Grief-related playlist, but that one's a surprise tool that will help us later :)

Ophelia Filius [Aishwarya Rai]
A former Tevinter altus of the city of Ventus, and current acting-Warden-Commander of Ferelden. A powerful necromancer who studied with the Mortalitasi, Ophelia works to recruit Fereldan Wardens in order to boost the numbers from the deficit caused during the Blight. She takes over while Reyn Caron searches for the cure to the Taint. Headstrong and clever, Ophelia has no desire to see Tevinter succeed, and denounces the practice of slavery, having realized its despicable nature after her own brother was sold into slavery against his will and experimented on by a magister. Ophelia is currently attempting to redefine the between-Blight Grey Wardens as a force for assisting the common good, protecting roads and helping the oppressed, and is willing to march on Weisshaupt with her fellow Wardens if the Order needs to be... cleansed of Corypheus’ influence.
Oly Aeducan [Jason Momoa]
A dwarf prince, disgraced by his brother’s murder and then exalted as a Paragon for his role in ending the Fifth Blight. He takes advantage of the dwarven resilience to lyrium to grant himself Templar powers, carefully rationing himself to ensure he does not become addicted. Husband of Skadi Brosca. He sets up permanent residence in Ferelden, near his friend King Alistair. A surprisingly skilled painter. He is a total pushover when it comes to his son with Skadi, Duncan Aeducan.
Skadi Brosca [Shantel VanSanten]
A former fixer for an Orzammar crime lord and the sister of a noble hunter (and the current wife of King Bhelen Aeducan), Skadi is intelligent, humble, and a vicious fighter. She’s generally a nice person, considering her occupation, but has a worryingly lax attitude towards crime. Do NOT play her at chess. Or Wicked Grace. Or Diamondback. Don’t play games with her. You will not win. She is the only dwarf in history to reject the title of Paragon in favor of remaining Casteless.
Konrad Cadash [Chris Evans]
An ex-Carta bruiser, he also was once a boxer in an underground fighting ring (sometimes literally; the Carta would send him to fighting Orzammar dwarves in prize fights. Was technically adopted by Oly Aeducan and Skadi Brosca. Has been taught some Legion of the Dead fighting style. He actually prefers the surface to Orzammar, and once killed a troll on his own. Becomes Leliana’s aide during Inquisition, and stays on as part of the spy network after Tash steps down.
Riona Mac Tir [Rachel Weisz]
Bastard mage daughter of Loghain and older half-sister to Anora. Senior Enchanter of Kinloch Hold and current Regent of Gwaren Teyrnir. Riona adapted the physical focusing movements taught to mages in Kinloch into a martial art not requiring magic, and later developed its use with folding fans. Traveling with the Hero of Ferelden after Broken Circle, Riona became friends with Alistair due to them both being bastards and sharing a dislike of her father. She is Ferelden’s answer to Madame de Fer, and acts as Tash’s escort to Halamshiral. She loves dogs, like many Fereldans, and owns a Mabari hound named Aofie.
Basile and Sylvie de Serault [Chris Hemsworth and Kathryn Newton]
The former Marquis of Serault and his sister, Basile and Sylvie are Orlesian nobles of the disgraced Serault, home to the Shame, a maleficar of note who passed some of his powers down to them, imbuing them with latent magical abilities, although not enough to consider them mages. Basile is a bookish and slightly shy man who would rather not be a ruler, but dutifully accepts and excels in the role as keeper of Ylenn Basin when Duke Tash is elsewhere. He loves his studies, and is no slouch in combat, although he only ever fights as a last resort. Once had a fling with the Prince of the Forest and is actually well-versed in glass-making. Sylvie is an adventurous and slightly boisterous young woman who enjoys hunting and exploring. She hates having to conform at court and enjoys her role as Lady Protector of Ylenn Basin. She has a custom made Serault weapon, a knife made of Serault glass attached to a fine chain, that she can whirl through the air nigh-invisibly, with deadly accuracy.

Canadian figure skating team headed to PyeongChang for 2018 Olympic Winter Games
OTTAWA, ON: Canada will send the largest figure skating team to the 2018 Olympic Winter Games in PyeongChang, South Korea. The team comprised of 11 entries, for a total of 17 skaters, includes two entries in men and three entries per discipline in ladies, pairs and ice dance. Competition begins on Friday, February 9, with the team event, and runs through to Friday, February 23, with all events taking place at the Gangneung Ice Arena.
The Canadian team will be lead by Opening Ceremony flag bearers and figure skating team co-captains Tessa Virtue, 28, London, Ont., and Scott Moir, 30, Ilderton, Ont. The three-time world champions will be competing at their third Olympic Winter Games, having previously won gold in 2010 and two silver medals (ice dance and team event) in 2014. This season, they won gold at both Skate Canada International and the NHK Trophy, and silver at the ISU Grand Prix Final. The eight-time Canadian champions are coached by Marie-France Dubreuil, Patrice Lauzon and Romain Haguenauer in Montreal, Que.
Three-time world champion Patrick Chan, 27, Toronto, Ont., is the first of two Canadian men entries. Chan will also be competing at his third Olympic Winter Games, having placed fifth in 2010 and won two silver medals in 2014 (men’s singles and team event). This season he placed fourth at Skate Canada International and won his record 10th Canadian title. Chan is coached by Oleg Epstein and Ravi Walia.
Canadian silver medallist Keegan Messing, 26, Sherwood Park, Alta./Brampton, Ont., is the second Canadian men’s entry. This season, he placed eighth at Skate Canada International and fifth at the NHK Trophy. Messing is coached by Ralph Burghart in Anchorage, AK, USA.
World bronze medallist and 2018 Canadian champion Gabrielle Daleman, 20, Newmarket, Ont., is one of three Canadian entries in ladies. Daleman placed 17th in Sochi as the youngest Canadian athlete at the 2014 Olympic Winter Games. This season, Daleman placed sixth at both the Cup of China and Skate America. The two-time Canadian champion is coached by Lee Barkell and Brian Orser in Toronto, Ont.
World silver medallist Kaetlyn Osmond, 22, Marystown, Nfld./Edmonton, Alta., will also represent Canada in the ladies’ category. Osmond placed 13th at the 2014 Olympic Winter Games in ladies singles and won the silver medal in the team event. This season, she won gold at Skate Canada International, bronze at the Internationaux de France, and bronze at the ISU Grand Prix Final. The three-time Canadian champion is coached by Ravi Walia in Edmonton, Alta.
Canadian bronze medallist Larkyn Austman, 19, Coquitlam, B.C., will be the third Canadian entry in ladies. This will be her first Olympic Games. This season, Austman placed 12th at Skate Canada International. She is coached by Zdenek Pazdirek and Liz Putnam in Coquitlam, B.C.
Two-time world champions Meagan Duhamel, 32, Lively, Ont., and Eric Radford, 32, Balmertown, Ont., will be the first of three Canadian entries pairs. Duhamel and Radford placed seventh in the pairs event and won the silver medal in the team event at the 2014 Olympic Winter Games. This season, they won gold at Skate Canada International, bronze at Skate America and bronze at the ISU Grand Prix Final. The seven-time consecutive Canadian champions are coached by Bruno Marcotte in Montreal, Que.
Canadian silver medallists Julianne Séguin, 21, Longueuil, Que., and Charlie Bilodeau, 24, Trois-Pistoles, Que., will be the second Canadian pairs entry. This will be their first Olympic Games. This season, they placed fifth at the Rostelecom Cup and fourth at the NHK Trophy. Séguin and Bilodeau are coached by Josée Picard in Chambly, Que.
Canadian bronze medallists Kirsten Moore-Towers, 25, St. Catharines, Ont., and Michael Marinaro, 26, Sarnia, Ont., are the third Canadian entry in pairs. Moore-Towers will make her second appearance at the Games after wining a silver medal in the team event in 2014 with her previous partner. This will be Marinaro’s first Olympics. This season, Moore-Towers and Marinaro won the bronze medal at the Cup of China and placed sixth at Skate America. They are coached by Bruno Marcotte, Richard Gauthier and Sylvie Fullum in Montreal, Que.
Canadian silver medallists Piper Gilles, 26, Toronto, Ont., and Paul Poirier, 26, Unionville, Ont., will represent Canada in ice dance. Poirier will compete for the second time at an Olympic Games, he placed 14th in 2010 with his previous partner. This season, they placed fourth at both the Rostelecom Cup and Skate America. They are coached by Carol Lane and Juris Razgulajevs in Scarborough, Ont.
Two-time world medallists Kaitlyn Weaver, 28, Toronto, Ont., and Andrew Poje, 30, Waterloo, Ont., will also represent Canada in ice dance. Weaver and Poje placed seventh at the 2014 Olympic Winter Games in Sochi. This season, they won silver at Skate Canada International and placed fourth at the Internationaux de France. The two-time Canadian champions are coached by Nikolai Morozov and train in Hackensack, NJ, USA.
Mike Slipchuk, Skate Canada High Performance Director, and Manon Perron of Boucherville, Que., will be the Canadian team leaders. Dr. Erika Persson of Edmonton, Alta., and physiotherapists Agnes Makowski of Toronto, Ont., and Meghan Buttle of Toronto, Ont., will be the Canadian medical staff onsite. Officials from Canada at the event include Leanna Caron of Timmins, Ont., Janice Hunter of West Vancouver, B.C., Nicole Leblanc-Richard of Dieppe, N.B., and Jeff Lukasik of Calgary, Alta.

SECRET RADIO | Sept.5.20

As there’s still very much a pandemic going on, we really didn't want to stop overnight if we didn’t have to on our drive from New York to Illinois, so after we exhausted the fantastic Saturday NPR lineup we figured a great way to stay awake was compiling the second issue of WBFF. You’ll hear a lot of the African and tropical vibe from last week, plus more French yé-yé and Europe in general. Here’s the track list, and of course, detailed notes below:

1. Michel Polnareff - La lezione del capellone
Le Beatnik is one of Paige’s all-time favorite songs — the guitar is just such a rad riff, and it was a real debate on which version to put on, the French version of the Italian, and we figured we didn’t have much Italian yet… plus check out how he rolls his Rs! That’s one thing about the major hitters of French pop in the ‘60s — they all sing in different languages. (Keep an ear out for France Gall later!)
2. Antoine Dougbé - Honton Soukpo Gnos
Dougbé just hit us like a ton of bricks — these arrangements are completely perfect to our ears. So detailed, and with super funk and super Velvet Underground tones at the same time (check out the freakin guitar solo, and guitar outro), plus of course a Beninese pulse that is just unstoppable. This one was produced, uncredited, by Melomé Clement, and Dougbé isn’t the singer, he’s the composer.
3. The Velvet Underground - Cool It Down
The guitar tones here come so naturally out of the T.P. Orchestre sound. There’s plenty of stuff he’s saying that I don’t understand, and he’s saying it in English.
4. Ranil - Ranil y Su Conjunto Tropical - Licenciado
This is another Analog Africa release, with a fantastic album cover. Paige dove into it and soon found that she was checking every day to see if the vinyl was out, cos it was under 300 copies and falling. It’s got a parrot on the cover that looks like the Greenwood parakeets. This song is how she learned that she loves cumbia music.
5. T.P. Orchestre Poly Rythmo - Trop Parler C’est Maladie
The guitar launches into these endless patterns that wind around an unknown number of percussionists. Everyone feels so fluid together. “Trop parler c’est maladie”: “Too much talking is bad,” literally, which Paige imagines as essentially meaning, “Let’s stop talking and make out.” But we don’t really know.
The way the voices layer together when they’re singing, and then the guitar rises into the spaces between them, twisting and winding and droning and melodizing, is just completely mesmerizing. This incredible guitarist is Papillon, and he died in 1982. What a huge loss — he plays so eloquently and so conversationally.
6. Janka Nabay & the Bubu Gang - Somebody
I learned about Janka Nabay by hearing that he had just died. Listening to KDHX, the DJ (Caron House?) said she was sorry to hear about his passing and put on this song and I had to stop driving so I could just listen to all of the patterns crossing. I sure would have loved to see this band perform live — I feel like this is a specific reason we want to be in NYC, to meet people with capabilities I hadn’t previously known existed and work together.
7. Anna Karina - Roller Girl
Our friends Phil and Archie are an intercontinental love affair. When Phil tries to express an American accent, it’s always dripping with RRRRRs. This song feels like that, a French singer dramatically unrolling her Rs like she’s messing with her English teacher.
8. Wells Fargo - Watch Out
We believe we first heard about Wells Fargo from a Snap Judgment episode. It’s really great, starting with how they picked the name from the side of a stagecoach in a Western movie as a completely random phrase.
9. Patrick Coutin - J’aime regarder les filles
This song — this pretty freakin radical recording — became a huge hit in France in 1981 and apparently served as the soundtrack of that period. It was his big hit.
That main phrase he keeps repeating translates to “I like to watch girls on the beach,” which gets weirder and sometimes creepier with each pass.
10. Jacques Dutronc - Les cactus
Credit to Pandora for bringing us Jacques Dutronc! He sounds like Bob Dylan on a lot of songs, but he doesn’t seem to be the Dylan of France — that would be more like Serge Gainsbourg. But we could be wrong about that too. He’s also longtime partners with Francoise Hardy, which made them two of the hottest and hippest and most talented musicians of the ’60s.

11. El Rego - Zon Dede
One of our main appreciations now is how deeply and broadly and internationally James Brown hit the ’60s and ’70s. People seemed to be struck by him and his approach planetwide, and it shows up like a shockwave through the whole era. This is Benin’s El Rego (thanks to Analog Africa once more for pointing him out), and his whole character seems to be in conversation with James Brown, shouting at each other from opposite sides of the world.
12. T.P. Poly-Rythmo, Bentho Gustave - Iya Me Dji Ki Bi Ni
The level of simultaneous groove and drone happening here is just unstoppable. This song has a sound I feel like I’ve always been looking for, not to play but just to dance and freak out to. Bentho Gustave is T.P. Poly-Rythmo’s bassist. We just got his headlining album on vinyl, so amazing!
13. Cambodian Rocks 1 - Yol Aularong - Jeas Cyclo (Ride Cyclo)
It’s amazing to think about how many sudden collisions of American and Cambodian experiences there were in the ’60s — so many disastrous ones, but also whatever combination of factors lead to the existence of this music. It’s so incredibly skilled and smooth, and I want to know more about who made this and how.

Don’t miss the videotape version of the live TV version via YouTube. Celentano is an Italian singer, but this is him tweaking Americans with a songful of non-English English.
15. France Gall - Der Computer Nr. 3
Another example of French pop multilingualism. One of Paige’s favorite voices, just throwing herself into the melodies all casual and easy breezy. We found out about this song because Paige was listening to a bunch of Quebecois radio, and they were talking about her, and they were playing a bunch of clips of her performing, including this one. It turned out that she had just passed on. We had to find it find this track.
16. Saigon Rock and Soul - Thành Mái - Tóc Mai Sợi Vắn Sợi Dài (Long, Uneven Hair)
What a vocal performance! Everything about the tones and melody of this recording — voice, organ, guitar — make it feel like a dream of hardcore hippie rock, minus even one shred of San Francisco. Every time I hear this song hit its crescendo I am completely in its thrall.
17. Ofege - It’s Not Easy
So these guys are from Lagos, Nigeria, and were in high school when they recorded this. Which is just freakin perfect. It sounds like enthusiasm and bad gear and good voices and that singular feeling of being sixteen and having a stone blast.
18. Avolonto Honore et l’Orchestre Poly-Rythmo de Cotonou - Sètché Wêda
It took some time for us to find this song. This version is from an album we found online, but it also exists on an Analog Africa album. The hand drums locked in with the guitar lick for the verse get so deep when the other voices come in. And the cascading guitar riff creates a sudden electric waterfall, while the horns point straight over to jazz and western rock and soul.
19. Ranil y su Conjunto Tropical - Marlenita
Paige became completely smitten with this album, which we found after getting caught up in an album promising Peruvian chichidelica.
20. National Wake - Everybody Loves Freedom
When I started looking for African punk, I found this set of videos of National Wake and pretty much all of them are this good. They’re a Black and white band (the first?) from South Africa in apartheid ’70s and the rock and punk that they make overflows with feeling. There’s a documentary called Punk in Africa that clearly needs watching.
21. English Beat - Mirror in the Bathroom
The live version of this song really amps up the energy from the studio version I’m used to. This song always makes me think of my friend Audrey, who made me a mixtape when I was just a wee twig, and this was one of the songs that broke through and made a lasting impression. I think of her every time.
22. Fela Kuti - Roforofo Fight
“Translation from the original English hrm hrm hrm”
This song structure is so very cool, where he’s kind of playing around, setting the stage, warming up his story, and then the horns crash in with a shocker part, which gives away to this cartoonish, endlessly repeating horn phrase, like a piece of early Disney footage on repeat. Also like loop-based recordings that were still more than a decade in the future. This is one of my favorite versions of Fela, where he’s exploding with a new self and a new mode of music.
23. Fela Ransome Kuti and His Koola Lobitos - Ololufe
Now some time travel - this is from a collection of earliest Fela Kuti, when he’s in a completely different mode, more in the high life and samba form. He and his band are playing in a crowd-pleasing style, and it’s really very beautiful, with gorgeous vocals and horn passages. But this is also all prelude to Fela Anikulapo Kuti, a character we haven’t even glimpsed yet when this song was recorded.
24. William Onyeabor - Atomic Bomb
We really don’t know much about this song except that it seems to always show up at just the right time — or maybe its presence just makes the time right.
25. Assa Cica - Tinma Sa Le
We were down a different rabbithole when we saw this video appear that we couldn’t pass up. (https://www.youtube.com/watch?v=2rpI09vbq_Q) It’s a long and winding song that was one of Assa Cica’s most notable hits in Benin.
I love the three slapped notes in the middle of the song that never come back — that’s the way to slap. The bass throughout this song, but especially in the choruses, is exceptional, telling its own separate story but staying in relation with the vocals. And Paige’s favorite part of the melody is the bridge.
We just learned that Assa Cica died on May 22 of this year. It feels so strange to fall hard for a person’s music just as they leave the stage for good. (See also Janka Nabay.) It also feels good to realize you’ve already become part of a musician’s music living and giving beyond their own lives.

Procès Mediator*: l'ancien président de la commission d'AMM Daniel Vittecoq a la mémoire qui flanche
L'ancien président de la commission d'autorisation de mise sur le marché (AMM) Daniel Vittecoq a soutenu ne pas se souvenir qu'en 2007, des membres de la commission nationale de pharmacovigilance (CNPV) avaient considéré le rapport bénéfice/risque de Mediator* (benfluorex, Servier) comme défavorable, jeudi devant la 31ème chambre correctionnelle du tribunal de grande instance (TGI) de Paris.
Alors que le tribunal a commencé à entendre les victimes et les experts pour le volet "homicides et blessures involontaires" depuis le début du mois, une audience sur le volet "tromperie et obtention indue d'AMM" s'est intercalée avec le témoignage du Pr Daniel Vittecoq, qui a été président de la commission d'AMM de 2003 à 2012 de l'ex-Afssaps devenue ANSM qui est sur le banc des prévenus en tant que personne morale.
Le Pr Vittecoq est d'abord revenu sur son parcours personnel comme médecin spécialiste des médecins infectieuses et du VIH en particulier et l'organisation de la commission d'AMM. Il a notamment affirmé lors de sa déclaration liminaire avoir découvert Mediator* en 2007. "Je ne connaissais pas grand-chose de ce médicament. C'était un vieux médicament qui avait probablement une petite efficacité mais qui ne semblait pas avoir un profil de risque particulier. Parmi tous les médicaments, il n'y avait rien de spécifique avec Mediator*."
Selon lui, dans le dossier qui lui avait été transmis pour la séance d'avril 2007 de la commission d'AMM, rien ne ressortait, en particulier sur "la parenté pharmacologique" de Mediator* avec les fenfluramines, en dehors de l'interdiction du benfluorex dans les préparations magistrales comme les anorexigènes en 1995.
Il a déclaré se souvenir que la commission avait examiné les risques de troubles neuropsychiatriques et d'hypertension artérielle pulmonaire (HTAP), mais qui ne constituaient "pas un signal significatif de toxicité dans le dossier". Il a évoqué un diaporama de Catherine Rey-Quinio, ancien médecin évaluateur et chef d'unité à l'Afssaps, mais qui ne comportait "rien sur la pharmacovigilance, pas un seul mot sur la valvulopathie".
Les résultats de l'étude Moulin, menée par Servier, ont aussi été présentés lors de cette réunion. "Vraiment, la commission d'AMM a été surprise par la qualité de cette étude qui révélait l'impact de ce vieux médicament sur le diabète mais pas sur les triglycérides. Oserai-je le dire? Cette commission est sortie avec le sentiment d'avoir fait son travail: on a dégagé l'indication triglycérides et on a demandé une inspection de l'étude!"
Mais la présidente du tribunal, Sylvie Daunis, a rappelé qu'un mois plus tôt, les membres de la CNPV avaient demandé que plusieurs éléments soient pris en compte lors de la réévaluation du rapport bénéfice/risque de Mediator*, notamment "une efficacité du produit jugée modeste par certains", son métabolisme qui conduit à la formation d'un dérivé fenfluraminique, ses effets indésirables neuropsychiatriques, des rares cas d'HTAP et de valvulopathie.
Dans le compte rendu de cette réunion, il a en outre été mentionné que "certains membres ont tenu à faire connaître leur opinion en se prononçant pour un rapport bénéfice/risque défavorable de Mediator*", allant au-delà de leur mission.
Or, après avoir examiné le dossier de Mediator* en avril 2007, la commission d'AMM s'est déclarée favorable au maintien de l'indication dans le diabète comme adjuvant et a demandé l'ajout des troubles neuropsychiatriques dans le résumé des caractéristiques du produit (RCP) ainsi qu'une inspection de l'étude Moulin et une communication sur l'usage hors AMM. Mais les cas de valvulopathie ne sont pas mentionnés dans le compte-rendu.
Interrogé par Sylvie Daunis sur "la discordance" entre les conclusions de la CNPV et celles de la commission d'AMM, le Pr Vittecoq n'a pu qu'acquiescer. Mais il n'avait pas eu le compte rendu de la CNPV ni celui du centre régional de pharmacovigilance (CRPV) de Besançon, chargé de l'enquête officieuse en 1995 sur Mediator* devenue officielle en 1998. "On peut demander un sursis à statuer, ce qui permet d'avoir accès à ces documents", a expliqué l'ancien président.
Il a réaffirmé n'avoir découvert Mediator* qu'en 2007 alors que, déjà en 2005, la CNPV avait demandé une réévaluation du rapport bénéfice/risque du médicament, rappelant que c'était la direction de l'évaluation qui préparait l'ordre du jour des séances. Il a également déclaré "ne pas avoir été au courant à l'époque" de l'enquête européenne lancée en 1999 à la suite d'une alerte italienne.
Aucun souvenir de la présentation du président de la CNPV
La projection d'un diaporama sur le benfluorex contenant à la fois des éléments sur cette CNPV de 2005 puis de 2007, sur les cas de valvulopathie, la pharmacovigilance européenne n'a pas non plus réveillé sa mémoire. Il s'est déclaré "très surpris" par les conclusions de la CNPV qui n'ont pas été à "sa connaissance en ces termes" et par ce diaporama dont la date de présentation, 2007 ou 2009, a été discutée puis vérifiée tandis que les débats se poursuivaient.
Alors que le Pr Vittecoq répondait à la procureure Aude Le Guilcher qu'un rapport bénéfice/risque devenait défavorable quand le premier devenait moindre et le second plus important, il a répété que les éléments n'avaient pas été portés à l'attention des membres de la commission d'AMM.
La présidente du tribunal a bondi, ayant visiblement vérifié la date du diaporama. "Mais Jacques Caron est venu en personne! Il est venu expliquer les conclusions de la CNPV", faisant référence au président de cette commission lors de cette séance d'avril 2007. Sans que son nom soit précisé, le président de la CNPV est bien indiqué parmi les personnes présentes, note-ton dans le procès-verbal de cette réunion.
"Je n'ai pas de souvenir de ça. Peut-être ai-je été trop marqué par l'étude Moulin?", a avancé le médecin. Il a même acquiescé à demi-mot lorsque la procureure lui a demandé s'il avait été "ébloui" par le bénéfice de Mediator*.
"On a l'impression que l'ensemble de la commission d'AMM s'intéresse plus à la question du bénéfice de Mediator* qu'au risque ou à l'évaluation du bénéfice/risque, et ce malgré le président de la CNPV qui se déplace! De manière symptomatique, vous ne vous rappelez pas sa présence, ce qui est pourtant exceptionnel!", a commenté la procureure.
Se déclarant "sidéré", le Pr Vittecoq a fait observer que la commission d'AMM n'aurait "pas forcément décidé d'une suspension", mais aurait pu proposer une surveillance échocardiographique. Mais elle ne l'a pas fait non plus, a rétorqué Aude Le Guilcher.
La suspension d'AMM de Mediator* n'est intervenue qu'en novembre 2009, rappelle-t-on.
