You wake up in a dark void...
May I present The Defect, one of the main antagonists of my Scarlet Owl Tavern plotline in Toonkind D&D.
This peck neck is trapped in a plane in between our world and the one that we play D&D in; but he can project himself into characters' ideas and dreams. He didn't even have a physical form to appear as for a long time (which I used to meticulously torture the heck out of my poor players, who wanted nothing more than to see what he canonically looked like), but the more players that joined the story, the more connected our worlds grew. When he finally accumulated enough power to project an image of him in addition to his iconic voice, he took this form.
He's connected to Chloe the Tavernkeeper due to them being intertwined during creation, and was able to speak to her and appear physically much quicker than he could do so for other people. He can even take control of her briefly if he wants to interject something into a conversation or stop her from saying too much. Most of the time, he's just making annoying comments in her head.
