LORE UNDER THE CUT
Undergore is an au where humans who are pushed or fall into the underground succumb to its magic and die; transforming and reviving into monsters afterwards.
The first monsters were Toriel and Asgore, and Sans came a few years later, with some less important monsters appearing in between.
Undyne was next, then Alphys.
After that, a child named Papillon fell, and he took to Sans like a younger brother, and so they started calling him “Papyrus”
Eventually everyone had arrived, with a few humans surviving and escaping to the surface; though not without leaving a mark. Frisk was the most recent; having killed Flowey, Toriel, as well as a few “unimportant” monsters, and nearly killing Asgore, too.
Sans has lost friends, family, and even nearly papyrus in the past from these 8 humans who survived, and now, a 9th human (Violet) has fallen, and she seems determined to find a way for monsters to come back to life, or rejoin the world of the living. He does not trust her, and he makes it very known to her. But time wears on her, and she starts growing flowers— A sign that the underground is trying as hard as it can to kill her and make her join them.
Sans can’t help but feel bad for her. He knows what it feels like to wrestle with the powers at hand, what it feels like to fight against the power that’s trying so hard to kill you. He urges her to give in, to let the underground take her, not wanting to see another bright young soul suffer for years after what he saw Papillon Papyrus go through.
On top of that, there are a good amount of monsters — undyne included — who are eager to see what her monster form would look like. These monsters are willing to kill her themselves if they must, and she always has to have a monster who supports her journey with her if she wants to survive.
The further she goes into the underground, the more monsters she meets, and the more determined she becomes. This means the underground has less influence over her, so she’s becoming less fragile and weak as the journey progresses.
Eventually, she reaches asgore, and while he expresses his reluctance to take a life, he explains to her that his goal is to return his kind to their human selves. Which, with current knowledge and understanding, requires determination… And boy, she has a lot of it.
It’s her choice now; Sacrifice herself for her friends, or come up with a better solution that everyone will be happy with.

Sans is not the only monster with growths. Every monster has various “gore” aesthetics; flower gore being reserved for transitional stages, while other gores pertain to the monster’s state of mind and personality upon transformation.
Sans was “executed” by being shoved underground, he isn’t the only monster who was, though. Many of the older monsters are victims of the old human tradition of shoving their “criminals” and outcasts into the hole that is the underground, though a notable “young” victim of this is Monster kid, who was shoved to his death by his bullies.
Many monsters had rough surface lives, thus why they don’t want to return “home,” and have instead made their home here.
The 8 prior surviving humans had no choice but to leave after their journeys due to the nature of the underground. All humans who spend too long there die, and the length of time it takes depends on how determined they are.
Frisk was the most determined, but had the shortest journey, as they kind of blasted through their final reset and didn’t look back.
Violet cannot reset, but she can come back from any wounds that would not instakill a regular human. If she has multiple wounds she is less likely to come back fast. If she gets too hurt, she won’t come back at all.
Violet’s soul is in a transitional phase, as traits can change in this au. She is transitioning from kindness to determination, but was born as perseverance.
Most monsters have human names, which basically work the same as dead names — you can use them, but you’re a jackass if you do because they no longer represent the person you’re referring to, and it’s highly disrespectful to do so. Monsters who don’t have human names either forgot them or just never changed their name.

here's a big OC concept dump! these characters are from my original world/setting, called Vivacia. I'd love to share more about them/the world if anyone's interested!
"Vivacia is a world of caverns and soil and stone. She has no sky, for all that exists does so within her substance. In her empty spaces, mortals have built civilizations, sustained cultures, and found faith in the foundations of their reality–foundations that quake and fold and shift at a moments notice. Many attempt to understand Vivacia and explain–perhaps even cure–her ailments, though none so far have succeeded."
Some details starting top left:
1. Flynt (he/him, human), about 30 years old, devoted member of the Lanternkeepers faith, kind hearted but strong and stubborn
2. The High Surgeon (???, ???), mysterious leader of the Cautery, a violent sect of the church of Vivacia devoted to "curing" her at all costs. Has never been seen uncovered, by anyone, including senior members of the Cautery itself
3. Not a character, just a concept for a Beastfolk Pilgrim that belongs to the Lunarium, a faith surrounding the light, song, and the singing crystals that naturally form throughout Vivacia
4. Lukas Dietrich (he/they, Drakon), about 25 years old, born and raised in upper Incus Malleus. Last living heir to the noble Dietrich family, though their fortune is long gone.
5. Selevis Ichtar (he/him, Drakon), about 40 years old, born to the Ichtar family in upper Incus Malleus with an uncanny intelligence and ability to communicate fluently at just a few months old. He quickly fled the city in his adolescence and has spent his life traveling and studying through Vivacia, eventually writing his masterwork "Selevis Ichtar's Guide to the Undergore, how to identify where you are by what you see!", an essential text to anyone wishing to travel Vivacia's expanse
6. Unya (she/ve, Beastfolk), unknown age, mysteriously appeared, as a fully formed adult, deep within Vivacia. Ve does not know how she ended up here, but has vague memories of a time "before" and is now devoted to finding a way back to this unknown home.
