COMMON TYPES OF TREATMENTS FOR INFERTILITY

TI – Timed Intercourse – Egg is formed by the help of medication and the right time to try naturally is recommended. IUI – Intra-uterine Insemination – egg is formed & at the time of rupture, semen sample is prepared in lab & injected in uterus. IVF – In-vitro Fertilization – eggs are formed in more numbers & removed from ovaries, fertilized outside in a culture dish by keeping the eggs and sperms together. ICSI – Intra-cytoplastic Sperm Injection –when sperms are incapable of fertilizing the egg or in case of previously failed cases ICSI is done which is injecting a single sperm into the egg with the help of a very advanced machine called micromanipulator.

MALE SPERM PROBLEMS

Oligospermia – Reduced count of sperms Teratospermia – Abnormal morphology (under 4% of normal sperms) – structure of sperms is defective. Problem can be at the level of head, neck or tail of the sperm. Asthenospermia – Abnormal motility (movement) of sperms. Oligoasthenoteratozoospermia (OATS) – Problem at all levels of sperms. ICSI is indicated in these cases. Azoospermia – No sperms in the semen. Cryptozoospermia – No sperms in the semen but found after centrifugation of semen in lab. Hypospermia – Decreased volume of the semen.

FEMALE PROBLEMS

Tubal Factor – Tubes are found either blocked or non-functional & can be operated upon or bypassed by IVF.

Ovulatory Disorders – Polycystic ovaries needs egg formation which may be simple but if it fails it needs aggressive management in the hands of an expert. Ovarian failure needs donor egg.

Endometriosis – menstrual blood flow gets deposited in pelvis. Surgery / IVF or both can be done on case to case basis.

Fibroids – Only the ones pushing the cavity need surgery. So a detailed scan from an expert is required. Surgery can be done laparoscopically.

Adenomyosis – Uterus becomes bulky and enlarged. Mostly it is suppressed with medicines but sometimes very large uterus neees surgery.

Starting my day out right with some anti-inflammatory properties. I have been doing the paleo diet in order to help with Endometriosis and Interstitial Cystitis. Paleo pancakes with blueberries, can you say yum! This is something I struggle with daily and have to be super mindful of, or I can end up in bed for days. If I let my stress get out of control it can cause a bad flair and I'm out of comission, which is damaging for a student and I can fall behind fast, especially in a PhD program. I was truly saddened to hear that another #endosister has taken her life. I did not know Allison but hearing of her death & reading her final FB post brought me to tears. I know that darkness & despair that is brought on by #endopain I know the cruel reality of having medical professionals minimize or dismiss our pain. This is what an #endobelly looks like. I was cruising along, the picture on the right is 2 weeks ago. The picture on the left is what happened today during an #endometriosis flare up. Same nutrition, same workout regimen. It will go back to normal in a few days because I’m receiving treatment. . I spent years blaming myself. This is an bottle of CBD #cannabis oil that a patient brough in today. It has brought her significant symptomatic relief. And the side effects are not serious. Despite growing interest in the therapeutic use of cannabis to manage chronic pain, only limited data that address these issues are available. Today I did a Pubmed search on #cannabis and #endometriosis and did not find a single article. Given the fact that routes of administration and different combinations of cannabinoids are available, patients are left to do research, experiment on their own or seek reccomendations from dispensaries. The only other option are closed patient groups: Facebook groups are the living example of why patient sourced research is so relevant . Wherever legal, medical #marijuana should be regarded as an additional therapeutic option for the symptomatic management of #endometriosis related pain. #endometriosisisreal

I was going through my surgical photo library and found this . I excised this rectal nodule 19 years ago while still working at #columbiapresbyterian . This is how long I have been involved in this! A bit of background history:at the time I was working with endo pioneer Harry Reich. Columbia at the time did not support this type of surgery and we ended up all moving to the now defunct St. Vincent's Medical Center, in downtown New York. We created a partnership of 4 surgeons whose names you may recognize : Dr. Reich , Dr. CY Liu , Dr. Seckin and your Dr. Vidali . It was an amazing and exciting time in terms of work , research and friendship. Over the years egos and life experiences have gotten a bit in the way but there is much love. The patient had severe pain and rectal bleeding and no gastroenterologist was able to see the lesion. #endometriosis nodules are very insidious and difficul to see. She did very well and went on to have 2 children and has been #pain free ever since. If performed appropriately and in a timely fashion , surgery can really solve many endometriosis related problems.
