Varkhal + Assistance Tech
Loathe as he is to admit it, Var uses assistance tech to make dealing with his conditions not only easier, but in some cases possible at all. It's only because he's from a family of old money (and inherited double what he was meant to, with the loss of his brother who was meant to get the other half) that he can afford the tech he needs. He likely wouldn't have survived in the Empire without it.
Rebreather
Breathing issues cause him to have a hard time getting enough air. He needs to wear a rebreather - or some other form of mask, tube, nasal canula etc. connected to a source of oxygen - at just about all times. He can take it off for short periods of time given he's somewhere with good air quality + ventilation, mostly to eat or shower, before putting it back on.
His usual rebreathers function almost as filter systems, filtering extra oxygen from the air to feed into the mask, while also filtering the CO2 out of the mask. The ones meant for daily use, wearing into combat or the field and such, tend to be clunky and somewhat uncomfortable, as they need the space to have the technology in them to work properly.
He has more "lightweight" ones, as he'd describe them, for when he's just hanging out, tinkering, not doing anything that makes his breathing pick up. These ones are more comfortable, but can't keep up with oxygen intake if he's doing anything remotely strenuous.
When he sleeps, he uses something similar to a CPAP. His breathing is worse when he's asleep, so he needs a stronger source of pure oxygen. The mask connects to a high-quality machine via a tube; the machine is like a large version of his usual rebreather, filtering larger volumes of oxygen at a time through itself and into the tube/mask, while dispelling the CO2.
While not fully intentional, the filters in his rebreather(s) also help to keep him from getting sick.
Koboh (no serial number)
Var uses she/it interchangeably for Koboh.
Outside of his rebreather, Koboh is his most obvious piece of assistance tech. She's a large droid, modelled after a varactyl (the flower of her namesake, Koboh spiker, is engraved on her head). Var has an implant in his skull connected to her, allowing him to communicate simple things with her, as well as allowing her to get certain types of readings from him. She can also get readings from his various rebreathers/other masks.
She's essentially an alert droid, with a number of other tasks also programmed into her.
Tasks and alerts include:
-Informing Var (or others) if his rebreather or components in it are nonfunctional. She carries both a spare, lightweight quality one, as well as one connected to her via a tube, similarly to his sleep setup. Lightweight one is for more mobility but can't give as much oxygen, one connected to her for if he's really struggling for air and being stuck at her side is fine. -Carries his immunosuppressants, and also gives him a ping when it's time to take them -Carries his AAC -Keeps track of his blood oxygen levels via the implant -Finds another person to aid Var if he's injured or unconscious -Able to use his AAC to communicate something Var might need if he's unable to -Meant to be a mount, he uses her if he's expected to travel long distances or over rough terrain by foot -Keeps track of air quality -Has an emergency combat setting, reserved only for if Var is in immediate danger and can't defend himself. Priority is to run, with him on her saddle -Emergency setting for if Var isn't breathing, is unconscious, and there's no one around to help -She scans most people she meets, then sends a ping to Var's implant. He can then set "tags" for the person, ping them back to Koboh, and the tags get saved to her databank. Different tags allow her to have different interactions with people (all of the polycule has friend tag, with priority tag be noted for some of them for certain circumstances, for example)
TZ-9947 - "Bluebell"
Var uses it/its for bluebell.
A very small droid that floats near Var. There's a bluebell squish bloom engraved over its compartment.
Since Koboh can't follow Var everywhere, Bluebell is intended to follow him places that she can't go. It's usually mounted over Koboh's chest, but when Koboh is told to stay put and to release Bluebell, it activates to follow Var.
Its compartment carries immunosuppressants, it alerts him if he needs to take them, and it also keeps track of his blood oxygen levels via the implant. He'll sometimes keep a pain stim or two in it, as well.
AAC Device
Var's voice is so quiet that his rebreather can easily drown it out if someone isn't giving him their full attention. Paired with the rasps from the rebreather itself, and general communicator interference, he can be anywhere from difficult to impossible to understand over comm. To make up for it, he has an AAC with pre-set words and phrases to each button.
There's general, military/command-type buttons, and medical ones on the device.
As it's physical buttons, Koboh can also use it to communicate if Var needs something.

The Role of Continuous Positive Airway Pressure (CPAP) Therapy in Sleep Apnea Treatment
Continuous Positive Airway Pressure (CPAP) therapy is widely regarded as the gold standard treatment for obstructive sleep apnea (OSA). At Sleep Center, we recognize the pivotal role that CPAP therapy plays in effectively managing sleep apnea and improving the quality of life for individuals affected by this condition.
How CPAP Therapy Works
CPAP therapy involves the use of a CPAP machine, which delivers a continuous stream of air through a mask worn over the nose or nose and mouth during sleep. This steady flow of air acts as a pneumatic splint to keep the airway open, preventing the collapse of soft tissues at the back of the throat that leads to obstructive sleep apnea episodes.
Key Benefits of CPAP Therapy
Improved Sleep Quality: CPAP therapy effectively eliminates or significantly reduces apnea events, allowing individuals to experience uninterrupted, restorative sleep throughout the night. By maintaining an open airway, CPAP therapy helps prevent snoring, gasping, and choking episodes that disrupt sleep.
Enhanced Daytime Functioning: Restorative sleep achieved through CPAP therapy promotes daytime alertness, cognitive function, and mood stability. Individuals who adhere to CPAP therapy often report improvements in energy levels, concentration, memory, and overall quality of life.
Reduced Health Risks: By mitigating the adverse effects of untreated sleep apnea, CPAP therapy helps reduce the risk of associated health complications, including cardiovascular disease, hypertension, stroke, diabetes, and cognitive impairment.
Partner Benefits: CPAP therapy not only benefits the individual receiving treatment but also their bed partner. By eliminating disruptive snoring and sleep apnea episodes, CPAP therapy can improve the sleep quality and overall well-being of both partners, fostering healthier relationships.
Optimizing CPAP Therapy
Mask Selection: Choosing the right CPAP mask is crucial for comfort and effectiveness. Options include nasal masks, nasal pillows, and full-face masks, each catering to different preferences and facial structures.
Pressure Settings: CPAP machines feature adjustable pressure settings to accommodate individual respiratory needs. Auto-adjusting CPAP machines (APAP) automatically titrate pressure levels based on real-time breathing patterns, optimizing therapy effectiveness.
Humidification: Integrated humidification systems help alleviate dryness and irritation commonly associated with CPAP therapy, enhancing comfort and compliance.
Routine Maintenance: Regular cleaning and replacement of CPAP equipment, including masks, tubing, and filters, are essential for optimal therapy performance and hygiene.
Conclusion
Continuous Positive Airway Pressure (CPAP) therapy stands as a cornerstone of sleep apnea treatment, offering significant benefits in terms of sleep quality, daytime functioning, and overall health. By embracing CPAP therapy and adhering to treatment recommendations, individuals can effectively manage sleep apnea and enjoy a better quality of life.
Contact Sleep Center today to learn more about CPAP therapy and other innovative sleep apnea treatment options. Our team of sleep specialists is dedicated to providing personalized care and support to help you achieve restful and rejuvenating sleep.

CPAP device: what is it, how does it work and what is it for?
Tumblr media
You've probably heard about obstructive sleep apnea (OSAS) and CPAP device therapy.
Your relative or friend may have sleep apnea and have talked about their experience using Philips CPAP Lawsuit. But what exactly is CPAP?
CPAP stands for “Continuous Positive Airway Pressure”. And CPAP therapy is considered the “gold standard” for the treatment of obstructive sleep apnea (OSAS) and has been available for 30 years.
Refers to pressurized air that is delivered through a unique delivery system that uses a mask. People with sleep apnea are given a prescription for their air pressure adjustment (from a sleep specialist), as well as a prescription for the machine and specialized equipment used to deliver it.
In this article, you will know what a CPAP machine is and how it works. Learn all the details for the best use of the device. Check out!
What is a CPAP device?
The CPAP machine is basically a small box that has a motorized fan inside. This fan very silently draws in ambient air, pressurizes it gently and delivers it to a device specific to your needs.
The air intake section of a CPAP machine has a filter to eliminate dust, smoke, or other impurities from entering the air. You will replace the filter as needed.
Another important part of the CPAP machine is the humidification chamber built into the housing. This is where the water is heated to humidify the pressurized air before it is delivered.
Warm, moist air soothes the nasal and upper airways and helps prevent swelling and discomfort that can sometimes occur while using the therapy.
While the use of humidification is optional, it brings relief to most patients using CPAP device therapy.
Attached to the CPAP machine is a hose that connects the case to the mask. This lightweight, flexible tube is normally heated to reduce any condensation that may accumulate inside it while the humidifier is in use.
The hose is long enough – around six feet – to allow full movement during sleep. Hoses can wear out over time and you should replace them as needed.
Because of this, it is often said that the success of CPAP therapy is often based on how comfortable and well-fitting the mask is.
How does the CPAP machine work?
Tumblr media
People who have sleep apnea have trouble keeping their airways open to breathe normally while they sleep.
Most commonly, the soft tissue in the throat and oral cavity (the tonsils or tongue, for example) relaxes or collapses, closing off the space needed for air to move through the nose and into the lungs.
This leads to pauses in breathing that can happen consistently and long enough to deprive the bloodstream of the oxygen it needs to deliver to all organs.
The brain senses this drop in blood oxygen and forces it to wake up. However, some people may experience over 100 of these pauses and wake-ups per hour for most of the night!
No wonder some people with severe sleep apnea feel so tired all the time.
If this disrupted breathing pattern is left untreated, it can create long-term health problems for the person afflicted with sleep apnea.
How to use CPAP at home?
The use of CPAP at home should be given according to medical advice. However, something is already right: the device should always be used as recommended by your doctor, with the appropriate air flow adjustments, at bedtime.
To use it, the process is quite simple. It is enough that you, in possession of the mask already sanitized, put it on your face, positioning it as comfortable as possible.
Once placed on your face, you just need to adjust the straps, so that the mask is snug, but not tight. Then, lie down on your bed and re-adjust all straps and mask straps.
Then just train a few breaths just through your nose and start your journey in search of your much-deserved sleep and rest.
However, it is always worth making an alert. If you are just starting to use CPAP, what always counts is persistence in using the equipment, no matter how uncomfortable or even unusual it may be to sleep with the mask on.
It is important that you always make the bedtime routine a moment of great pleasure, even if at that moment you have to use CPAP as a way to seek an even healthier and, above all, restful sleep.
Therefore, always incorporate the necessary adjustment of the parameters for using the equipment into your sleep hygiene routine, as recommended by your doctor – or even by your pulmonary physiotherapist, as in some cases it is prescribed.
Then, make the time to put on the mask also a time to start preparing for bed, such as rubbing cream on your hands or feet.

Why N95s Work for Medical Professionals, But Not For You:
First, I just want to point out there that this is take #2 because tumblr ate the first one and I’m pissed. The first one was really good, but now I don’t have 3 hours to dedicate to this one, so screw it. This is the speedrun. I’m gonna try to do it in an hour. If it doesn’t make a ton of sense, that’s probably why.
You may have come across someone on social media pointing out “why do medical professionals ‘need’ N95s specifically if we’ve all been told they’re no more effective than surgical masks/cloth masks? Isn’t that a bit of a double standard?”
The short answer is that N95s are really helpful and really important if they fit, are worn correctly, are worn in environments where they actually need to be worn, and are worn with other equally important pieces of PPE (how medical professionals wear them), but they don’t do as much for you personally if all of these conditions aren’t met, and you can ultimately accomplish close to the same thing (not getting sick) if everyone wears a cloth mask and washes their hands in public.
Let me explain:
1- N95s that aren’t fitted don’t do the N95 thing:
N95s are masks* with filters that can stop 95% of airborne particles 0.3 microns in size or larger. There are a lot of sizes, brands, and models to choose from. In order for them to actually do this level of filtering, there has to be a near-perfect seal around the outside of the mask, which forces the air through the filter (which would otherwise be a path of most resistance).
To be sure the air is actually going through the filter, the mask needs to be professionally “fit tested” (see link for more on the actual process, which is pretty extensive). It’s not uncommon for people to have to try two or three different kinds/sizes of masks during a fit test before they find one that actually fits their face. Others have a face shape that just can’t ever get a good enough seal, no matter what mask they try. If you’ve never been fit tested, the mask you’re wearing probably isn’t a whole lot more effective than a surgical mask or cloth mask- good for stopping droplets of other people’s saliva from hitting your mouth and nose, and from your saliva droplets from hitting other people (still really good things, obviously, but nothing that actually requires an N95-rated filter).
2- You genuinely don’t need an N95 to protect you in a community environment, but we actually do in a hospital environment:
The good news is you, as someone in the grocery store, is not in a situation where you need all the real protection a fit-tested N95 can give you. A truly fit-tested and properly worn N95 is designed to stop you from breathing infectious, aerosolized particles, which is a whole lot of overkill in most situations. Aerosolized particles are particles of (usually saliva in this context) that are so tiny and light that they can hang in the air and get breathed in even after an infected person leaves the area (unlike the larger saliva droplets that tend to fell out of the air pretty quickly).
Someone with covid-19 does not naturally shed aerosolized particles full of virus. They would have to actually do something to artificially aerosolize that infectious saliva. Something like:
An aerosol (nebulizer) breathing treatment
Artificial respiration/intubation/mechanical ventilation/CPR
Using a BiPAP or CPAP machine
Suctioning a tracheostomy
Notice that these things should not be happening at the grocery store, and if for some reason they are, it would be pretty easy to avoid that aisle. But those things happen a lot in a hospital, and a lot of times the medical professionals involved can’t avoid being in the area where they’re happening, which is why they do, unlike you, need a fitted N95 mask.
3- Yes, we do follow all these same guidelines in a hospital setting:
In a hospital setting we’re not just wearing N95s willy-nilly. We wear cloth masks when we’re not in patient rooms to prevent ourselves from spreading the virus! We wear surgical masks (or sometimes non-fitted N95s depending on the number of surgical masks we have) when we go into patient rooms for things other than aerosolized procedures (they can be worn a lot longer than surgical masks in situations where we’re running out of all PPE). We essentially only wear our one personal, assigned, fitted N95 in situations where we absolutely have to, or when its in the best interest of making what we have last as long as possible (its better for the masks to continuously wear them than to take them off and put them back on a bunch of times).
4- In summary
In summary, medical professionals are wearing professionally fitted N95s in environments where they are likely to need the protection that only N95s can provide. Similarly, people who aren’t working in hospitals right now both will likely not be in a situation where they actually need an N95, and if they’re wearing one, they’re probably not wearing it correctly anyway. That’s why you’ve probably heard both sides of that argument.
Hopefully that decreased some anxiety for people.
*Yeah, I know they’re actually called respirators, I’m using the colloquial “mask” in this post because that’s a different discussion and I’m on a time crunch.

Green Cleansing Vacation Rentals Looks Like New Cleansing And Janitorial
Cleaning workers must also perceive the importance and definition of green cleansing. This can add a “why” part that motivates teams to do their finest work. To discover merchandise compatible with a green cleaning program, make use of free assets just like the Safer Chemical Ingredients List from the U.S. Seek out third-party certified options with a stamp of approval from respected organizations like National Floor Safety Institute, Green Seal, and UL. “Any good white vinegar will do,” agrees TikTok #CleanTok star Ruby Cleans Things. But what if you don’t have the time to do kitchen chemistry right now? Chemical cleansing merchandise include harsh ingredients that may lead to coughing, sneezing, or eye or skin irritation in some individuals. Small youngsters may be delicate to chemicals utilized in laundry soaps and chemical cleaners. There are additionally numerous excellent books about selfmade alternatives to commercial cleaners, although not all of them concentrate on the difficulty of environmental safety. I use Cheaper and Better, by Nancy Birnes, The Non-Toxic Home, by Debra Lynn Dadd, and Blueprint for a Green Planet, by John Seymour and Herbert Girardet. Having to mix a new batch of this laundry formula from time to time is, I admit, a bit inconvenient. In fact, I wasn’t sure it was value it till I noticed, about a month into the experiment, that after 15 years of low-level zits my face had cleared up. When some houseguests arrived shortly thereafter I decided to switch back to detergent—rather than undergo the process of washing away the detergent residue from their clothes—and I instantly broke out. We've put together an open source decontamination system based mostly on Nebraska Medicine's publicly published protocol. It uses widespread components to build relatively robust UV mild sources for decontamination. Examples of relevant papers on fabric facemasks that present the serious shortcomings of this kind of protection are given under. “In addition to the T-piece adaptor, modifications to the BiPAP machine embody the addition of two high-efficiency particulate air filters at each ends of the oxygen hose to alleviate fears of spreading the virus. For making your personal air filters the Cpap and Bi-Pap machines often use a small rectangle of Tyvek to screen out a lot of the small particles. Conversion of raw material from agriculture and meals residue offers useful bioproducts. At the same time, it's also important to notice that their purported antiviral activities may not be directly relevant to the purposes of sanitization or disinfection. Antiviral actions such as the inhibition of viral replication or gene expressions, while potentially efficient within the treatment of viral infections, could not essentially be what the user is anticipating in terms of a topical or surface disinfectant. The log unit reduction and % (%) reduction are used interchangeably in literature to describe disinfectant efficacy. It's essential to disinfect surfaces where many people contact things throughout the day. Public transportation techniques like busses, trains and subways experience great influxes of people at peak times, which suggests the spread of dangerous germs is especially relevant in these places. Disinfecting seats, railings, doors, home windows and poles can kill the harmful germs that seem in these high-traffic areas. Disinfecting wipes from Wipes.com are bleach-free, so that they're safer than other disinfecting wipes and won't hurt surfaces delicate to bleach. Note that disinfecting wipes do include chemical substances that can be harmful if ingested. The shortage of HEPA filters can be supplemented by utilizing a painters full face mud mask and changing mud filters with HEPA filter material . I even have an thought to create PAPR's by utilizing portable CPAPs and the same HEPA filter. How to Take Care of a CPAP Machine
Tumblr media
A continuous positive air pressure(CPAP) is a device that is prescribed by the doctor to the person who suffers from a sleeping apnea disorder. Sleeping apnea causes a pause in your breathing often because of some temporary blockage in the throat. A CPAP machine to help in a steady flow of oxygen to your nose and mouth while you are asleep. Care oxy provides a CPAP Machine On Rent in Delhi. Care oxy is one of the leading manufacturers of medical equipment in Delhi. They produce sophisticated medical equipment which is easy to use. Moreover, care oxy has a full team of experienced doctors and nurses whose main aim is to treat the patient at their home. CPAP needs to be cleaned on a daily and weekly basis for its proper function. Some of the tips are-
Daily basis cleaning tips-
1. Unplug your CPAP machine.
2.Disconnect the mask and air tubing from CPAP machines.
3. Disassemble your mask into three parts, i.e., headgear, cushion, and frame.
4. In a tub take clean water and clean your headgear, cushion, and frame to remove any dust. Gently rub it with water, probably drinking water. Don't use any harsh things to wash the parts.
5. Rinse the parts thoroughly with clean water.
6.Place these parts in a clean towel. Avoid using direct sunlight to them.
Weekly cleaning tips-
Unplug your CPAP machine.
Disconnect the air tubing from your mask and CPAP machines.
Take a tub and fill it with clean water, and rinse the inside and outside of the tube with mild and gentle soap.
Rinse again thoroughly with clean water.
Place the air tubing on the flat surface on the top of the towel. Avoid placing it in direct sunlight.
The other tips for taking care of CPAP may include:
The outer cap of CPAP does not require special maintenance. If necessary unplug your machine and wipe it with clean and moist clothes. Dry the unit thoroughly, never dip the part of CPAP into the water.
Be very cautious, don't split any liquid into the machine. This may damage the entire circuit and void the warranty. Most of the manufacturers suggest using distilled water. Every morning clean the water chamber with a 10 min soak in gentle and mild soap, rinse well, and air dry after each use.
Choose a gentle and mild soap to clean the mask and tube. Never use any kind of harsh soap or bleach. To cut soap residual use 1 part of vinegar and 3 parts of water after cleaning.
Tubing should be replaced whenever needed or at least annually.  Tiny holes that are not visible to the necked eyes comprise the proper delivery of adequate air pressure. You can use a tiny cover for these holes. Try cleaning the tube weekly, by using a mild soap. Rinse well and allow the tubing to air dry. Avoid direct sunlight to it.
CPAP machine filters are inexpensive. Routine replacement will add more life to your machines. CPAP machine filter simply needs a weekly hand cleaning with a mild soap, clean water rinse, and air dry.
CPAP mash comprises different components being the outer frame, cushion, and headgear. To give a long life to your mask, remove makeup, and put the mash on the clean face. Try to clean the cushion daily with a mild soap mix and air dry. Weekly clean your mask frame and headgear.

What's the Difference Between CPAP and BiPAP Machines?
Difference between CPAP and BiPAP Machine
When breathing is interrupted due to low levels of oxygen in the blood, sleepers experience "apnoea" known as OSA. OSA (obstructive sleep apnea) is a type of disorder that affects the ability to breathe normally during sleep. In such a situation, the sleeper person wakes up coughing or gasping for air.
Disruptions in breathing make it difficult for people with OSA to get deep, restful sleep. Healthcare providers recommend the use of CPAP or BiPAP machines for a person with OS disorders. These devices make breathing more comfortable, allowing you to experience a good night's sleep.
Throughout this article, we explain the difference between a CPAP and a BiPAP machine and what these devices might be right for.
About CPAP Machine
CPAP machines provide a continuous stream of air to the throat to make breathing easier during sleep. They feature a small water tank and filter to power the machine. There is a CPAP masked tube attached to the motor of the machine. This mask is applied over the mouth and nose and secured with a chin strap. The constant air pressure from the machine keeps the airway open to improve breathing and allow for a deeper sleep. CPAP stands for Continuous Positive Airway Pressure.
NCPAP (nasal continuous positive airway pressure) machines are similar to CPAP machines, but they attach only to the nose. Instead of a full face mask, NCPAP machines have a nasal mask that is secured to the face by headgear and nasal plugs. Nasal pillow masks do not cover the mouth and are often more comfortable for those who prefer less weight on the face.
About BiPAP Machine
A BiPAP, or bilevel positive airway pressure machine, is a type of CPAP machine. It works the same way a CPAP machine does with one main difference. Bilevel machines produce different levels of air pressure, rather than just one single level. Like CPAP machines, BiPAP machines may feature either a full face mask or a nasal mask.
A Standard CPAP or BiPAP Machine
These BiPAP or CPAP machines can reduce the breathing problems of sleep apnea and make breathing more comfortable during sleep. However, BiPAP therapy provides variations in air pressure that may be preferable for some individuals.
CPAP machines create a consistent level of air pressure inside the mask. The constant pressure is strong enough to open the airways and improve breathing. However, with a single pressure level, sleepers have to exhale against the added pressure. This lack of variation can make it difficult for those with muscles and nerves affected.
BiPAP machines provide high pressure during inhalation (IPAP) and low-pressure exhalation (EPAP). High pressure during breathing is helpful for people with sleep apnea because it can force air into the lungs, even as the airway narrows. Different pressure settings can also make breathing work easier for people with muscle and nerve damage. The different pressure settings make it easier for these individuals to breathe, which helps them sleep soundly.
ATD Group is the best supplier of BIPAP and CPAP Machine
BIPAP stands for bilevel positive airway pressure that delivers mild airways during sleep. ATD Group is a leading name in this arena, known as the Best BiPAP machine manufacturers in India.  We promise to deliver the best inline product to the customers. Whenever required, reach us directly.

All you need to know about the functioning of a CPAP device
Tumblr media
A continuous positive airway pressure CPAP device is the most commonly prescribed device for treating sleep apnea disorders. Obstructive sleep apnea (OSA) causes interruptions or pauses in your breathing, often because your throat or airways briefly collapse or something temporarily blocks them. A CPAP machine sends a steady flow of oxygen into your nose and mouth as you sleep. This keeps your airways open and helps you breathe normally. Let’s take a closer look at how this machine works, the pros and cons of using one, and other options for sleep apnea. A CPAP machine’s compressor (motor) generates a continuous stream of pressurized air that travels through an air filter into a flexible tube. This tube delivers purified air into a mask that’s sealed around your nose or mouth.
As you sleep, the airstream from the CPAP machine pushes against any blockages, opening your airways so your lungs receive plenty of oxygen. Without anything obstructing this flow of oxygen, your breathing doesn’t pause. As a result, you don’t repeatedly wake up to resume breathing.
Are there different types of CPAP machines?
CPAP devices all have the same basic components:
A motor housed in a base unit
A cushioned mask
A tube that connects the motor to the mask
A headgear frame
Elbow pieces that act as joints
Adjustable straps that allow you to customize the fit of the device
Different mask types
Mask styles can vary with different CPAP machines. Which type you wear depends in part on your breathing habits, how comfortable the mask is for you to wear, and the kind of sleep apnea disorder you have.
The different types of CPAP masks include the following:
Nasal pillow mask- This type of mask has a small cushion that caps over your nostril area. It may also have prongs that fit into your nostrils. This mask allows you to wear your glasses easily. It also works well if you have lots of facial hair that may prevent a larger mask from fitting snugly.
Nasal mask- This type is a cushioned mask that covers your whole nose area. It may be a better option if you tend to move around in your sleep. It can deliver a high-pressure airstream.
Full mask- This type is shaped like a triangle and covers your mouth and nose. Your doctor might prescribe this kind of mask if you breathe through your mouth when you sleep or if you have a blockage of some kind in your nose.
What’s the difference between CPAP, APAP, and BiPAP machines?
Other types of breathing machines include APAP and BiPAP devices. Here’s how they differ:
CPAP device
This device is programmed to produce pressurized air at one steady air pressure level. To change the air pressure, you have to reset the device’s settings.
APAP (automatic positive airflow pressure)
This kind checks your breathing throughout the night. It automatically adjusts the air pressure to compensate for changes in your sleep position or medications that may have changed your breathing.
BiPAP (Bi-level positive airflow pressure) This device has two pressure settings, one pressure for inhaling and a lower pressure for exhaling. It’s used for individuals who can’t tolerate CPAP device or have elevated carbon dioxide levels in their blood. BiPAP devices can also come with a backup respiratory rate for patients who have central sleep apnea. The backup respiratory rate ensures the person breathes, as the main problem with central sleep apnea is initiating breath.

Fears of Ventilator Shortage During Coronavirus Pandemic Unleash a Wave of Innovations
Tumblr media
As the coronavirus rages across the globe, ventilators that pump oxygen into the lungs of critically ill patients have been embraced as the best hope for saving lives. But fears of a ventilator shortage have unleashed a wave of experimentation at hospitals around the country that is leading to some promising alternatives to help sustain patients.Doctors at North Shore University Hospital on Long Island have been using machines designed for people with sleep apnea to keep scores of coronavirus patients breathing, and engineers at New York University have transformed hooded hair salon dryers into personal negative pressure chambers that deliver oxygen and limit the spread of aerosolized virus, lowering the infection risks for health care workers and other patients.Pulmonologists across the country have been turning to a remarkably simple intervention: flipping patients onto their stomach, which markedly improves oxygen levels for those in respiratory distress.Doctors say these and other ad hoc interventions have allowed many hospitals to weather the surge of desperately ill patients in recent weeks, and may have helped stave off the dire ventilator shortages and rationing that some had feared but have not come to pass.“Some of these are battlefield interventions that we would not normally use in hospitals, but this crisis has been an incredible spur for creativity and collaboration,” said Dr. Greg Martin, a pulmonologist in Atlanta and the president-elect of the Society of Critical Care Medicine. “The beauty of this is that we’re learning a lot and hopefully some of this will translate to things we can use in the future.”The innovations have also been fueled in part by the dismal survival rates for patients on ventilators; in New York City, 80 percent of ventilated coronavirus patients did not survive.Doctors have always seen ventilators as a last resort for patients in respiratory distress, but many physicians on the coronavirus front lines are finding that noninvasive interventions can keep patients off the machines and improve outcomes. The pandemic is also scrambling previously accepted wisdom about acute respiratory distress syndrome, or ARDS, a condition marked by perilously low oxygen levels. The condition, an alarming hallmark of severe Covid-19, occurs when fluid that floods tiny air sacs deep within the lung interferes with the body’s ability to deliver oxygen to vital organs. But over the past month, many doctors say they have been mystified by Covid-19 patients with low oxygen levels who are still breathing on their own. Some health workers have reported caring for patients with oxygen saturation rates in the 70s and 80s — anything below 93 percent is cause for concern — yet their heart rates and brain function were relatively normal.“It’s been a very humbling experience dealing with a new illness, where we’re not sure whether the treatments we’re using are beneficial,” said Dr. Anthony S. Lubinsky, medical director of respiratory care at NYU Langone Health. “The data just isn’t there yet, but what we’re realizing is that a ventilator alone is often not enough to save ARDS patients.”Intubation, which involves inserting a breathing tube into the lungs, carries myriad risks. Patients must be heavily sedated, and many remain tethered to the machine for weeks. They also need to be closely monitored 24 hours a day, a challenge for hospitals grappling with a flood of patients and a shortage of respiratory specialists.Weaning patients off a ventilator presents its own difficulties because prolonged ventilation can cause severe lung injury, but removing breathing support too early can prove deadly.“You never want to intubate a patient unless it’s absolutely necessary,” said Dr. Susan R. Wilcox, the chief of critical care in emergency medicine at Massachusetts General Hospital.Still, many doctors believe that intubation offers the best hope for saving the most seriously ill patients. In recent weeks, as Covid-19 patients inundated hospitals in New York, Dr. Hugh Cassiere, a pulmonologist at Northwell Health, realized that the hundreds of less sophisticated breathing machines known as CPAP and BiPAP devices gathering dust in hospital storage rooms might help doctors weather a ventilator shortfall. The lunchbox-size devices are familiar to the millions of Americans with sleep apnea, chronic obstructive pulmonary disease and other breathing disorders.Over several days, Dr. Cassiere and his colleagues figured out how to convert the machines into makeshift ventilators, in part by using 3-D-printed adapters to hook the machines up to endotracheal tubes and to add HEPA filters that capture virus emitted during exhalation.Roughly 100 patients at North Shore University Hospital have been successfully sustained on the machines, and Dr. Cassiere said he has also used them on patients with other illnesses in order to free up critical care ventilators for Covid-19 patients. “I’d rather have more regular ol’ ventilators, but this could be a viable option if a hospital is running low,” he said.Enthusiasm for the machines has been spreading. Rhode Island health officials on Tuesday announced a campaign asking residents to drop off their unused devices at fire stations across the state. Many hospitals are using them to increase oxygen levels without resorting to intubation. The devices, doctors say, have been especially helpful for coronavirus patients with moderately impaired lung function. The positive air pressure they pump out dislodges fluid in damaged lungs, allowing the alveoli to better absorb oxygen, respiratory specialists say.The Mount Sinai Health System in New York is retrofitting 200 breathing machines donated by Tesla. Those devices, manufactured by ResMed and known as VPAP, or variable positive airway pressure, have been reconfigured to trap exhaled contagions. In a pinch, the revamped machines can also serve as air pumps for intubation.Dr. Charles A. Powell, director of the Mount Sinai-National Jewish Health Respiratory Institute, said the machines were not quite powerful enough to help patients in severe respiratory distress, though they do appear to provide relief for coronavirus patients with moderate breathing problems. He said the hospital has begun a randomized study to see whether the machines that use a simple face mask could be used at home by people not sick enough to be hospitalized. The downside, he said, is the risk of infecting others in the household because the masks do not filter out exhaled virus. “If possible, a person who is not sick should stay out of the room while the CPAP machine is working,” he said. Some doctors remain wary of the alternative devices. Dr. Wilcox of Mass General cited a 2017 study that found that up to 80 percent of patients with moderate respiratory distress who were attached to CPAP machines damaged their lungs by taking huge breaths over an extended period of time. The risk was especially pronounced in younger patients. Dr. Wilcox said she prefers to give coronavirus patients oxygen through a tube fitted beneath the nostrils.“There is probably a subset of patients who could benefit from CPAP machines, but the challenge is how to quickly determine who they are,” she said.Dr. Wilcox has also been encouraged by prone positioning, an intervention that involves turning patients in respiratory distress onto their stomachs. The practice, long used to increase oxygenation for mechanically ventilated patients, has been surprisingly beneficial for Covid-19 patients who are not intubated. The maneuver works by opening parts of the lungs that are compressed while lying on the back.“It’s all anecdotal at this point, but patients tell us they feel better,” Dr. Wilcox said. “It’s just fascinating.”Another intriguing alternative is a positive pressure hood that fits over a patient’s head to prevent the spread of pathogens while pushing air into the lungs. The hoods, which resemble something out of “2001: A Space Odyssey,” have been widely used by doctors in Italy and China in their battle against Covid-19.Over the past month, engineers and doctors in the United States have been rushing ahead with designs for similar devices, though they have not yet been approved by the F.D.A. The hoods can be hooked up to a CPAP machine or the central oxygen lines common in most American hospitals. As she tended to coronavirus patients last Tuesday at the University of Chicago Medical Center, Dr. Bhakti Patel noticed a man in his 50s struggling to breathe. His oxygen level had fallen to 85 percent and he was taking 30 breaths a minute; eight to 10 breaths a minute is considered normal. “You just can’t sustain that,” said Dr. Patel, a pulmonologist. Not long after placing a hood over his head, the patient’s oxygen level hit 100 percent, she said. For the past few weeks, a dozen patients who were on the verge of needing intubation have been using the ventilated helmets, and the early results have been promising. One patient has been discharged, another has left the I.C.U. and another two are expected to leave the I.C.U. this week. Only one patient who tried the helmet had to be intubated.“I am cautiously optimistic,” said Dr. Patel, a co-author of a 2016 study that found the helmets helped avoid intubation in 80 percent of ARDS patients and significantly reduced mortality.Over the past week, hospitals across the country have also begun experimenting with the helmets. At one, in Oakland, Calif., nurses have been assembling the helmets from parts they ordered on the internet.Dr. Giacomo Bellani, a critical care doctor at San Gerardo Hospital and University of Milan Bicocca in Italy who has studied the use of the helmets in ARDS patients, is also a longtime booster. He said they were widely adapted in the 1980s by Italian doctors grappling with the country’s chronic shortage in I.C.U. beds. The hoods have been a crucial stopgap measure for hospitals in northern Italy swamped by coronavirus patients struggling to breathe. Still, Dr. Bellani said they were not a panacea for the most desperately ill.“The hoods can buy you time when you’re short on I.C.U. beds,” he said. “But there is a drawback because in some patients you are just postponing intubation and the longer the delay, the greater the risk that some patients won’t make it.”Rukmini Callimachi, Tara Parker-Pope and Rachel Abrams contributed reporting.
