Robert Smith, 74, spent six years in terrible pain. He could barely walk and had stopped doing many of the things he loved, like playing golf. After what he calls “years of hell,” he was diagnosed with peripheral arterial disease (PAD), a condition caused by a build-up of plaque that blocks blood flow in the arteries of the legs and feet, preventing oxygen-rich blood from circulating throughout the extremities. An artery in Smith’s right leg was completely blocked.
“I was misdiagnosed for several years,” said Smith. “I did a lot of research and didn’t want to go through an invasive procedure, so I was so relieved to hear about new approaches at UC San Diego Health that could treat me, improve my quality of life and get me back on the golf course.”
One day after treatment, Smith was able to walk a mile pain-free.
“It was certainly worth the trip from Seattle to San Diego,” Smith said.  
UC San Diego Health is on the forefront of treating PAD with new technologies, including catheters with cameras the size of a grain of salt that allow cardiologists to see inside smaller arteries for the first time. We asked Mitul Patel, MD, interventional cardiologist at UC San Diego Health, about PAD and the latest treatment options for a disease that affects approximately 20 million people in the United States.  
Question: Who is at risk for PAD?
Answer: PAD affects more than 200 million people globally. Risk factors for the development of PAD include smoking, high blood pressure, diabetes, high cholesterol, obesity and advanced age, with 12 to 20 percent of individuals older than age 60 being affected. People with PAD have a higher risk of coronary artery disease, heart attack and stroke. Left untreated, PAD can lead to gangrene and amputation. You can take control and prevent PAD by pursuing a heart-healthy lifestyle with regular exercise and a balanced diet and by managing the relevant risk factors mentioned.  
Q: What are the symptoms of PAD and when should you see a doctor?
A: Robert was a typical patient with classic symptoms of PAD, which included cramping in the affected extremity that occurred with even minor exertion and improved to some degree with rest. Many people with PAD mistake the symptoms of PAD for something else, such as sciatica or “nerve pain”, or are misdiagnosed, like Robert was. Other common PAD symptoms include change in the color of the legs, coldness in the lower leg or foot, leg numbness or weakness and sore toes. Should these symptoms arise, you should inform your primary care physician who can refer you to a vascular specialist for further evaluation and treatment.  
Q: What are the latest treatment options for PAD?
A: The initial evaluation is fairly simple and easy and usually starts with measuring blood pressures in the limbs and determining any differences in the blood pressures (called ankle-brachial index or ABI). An ultrasound of the arteries may also be performed.  Depending on the results of these preliminary tests, further testing with computed tomography angiography, magnetic resonance angiography or invasive angiography may be done to determine the best treatment option.
A variety of treatment options are available for the management of intermittent blockages due to PAD.  The initial therapies include management of PAD risk factors, such as high blood pressure, high cholesterol and diabetes. In most cases, lifestyle changes and medications are enough to slow the progression or even reverse symptoms of PAD. If the patient remains symptomatic after these steps have been taken, the next approach is revascularization, or restoring blood flow. Revascularization can be accomplished with open bypass surgery, but more recently, the majority of peripheral revascularization involves endovascular therapy — treating blockages with devices that may remove or modify plaque followed by balloon angioplasty if needed, and in some instances, with stent placement.  
Perhaps the most advanced revascularization involves the technology we used to treat Robert, which involved the use of intravascular imaging with optical coherence tomography. This technology involves a catheter with a small fiber optic camera on the tip fed through a small incision in the groin and does not require full anesthesia. Once inside, the physician is able to see and extract the diseased areas or plaque without damaging the artery wall, which can cause recurrent narrowing of the blood vessels. Fluoroscopic technology is traditionally used during these procedures, but this newer technology reduces the use of fluoroscopy or X-ray radiation, which in high doses can have harmful effects.  
At UC San Diego Health, we prefer to remove the plaque and then perform balloon angioplasty with a drug-coated balloon to prevent scar tissue build up in the artery going forward. To remove the plaque, we have been using intravascular imaging, which is integrated in the Avinger’s PantherisTM atherectomy catheter and allows us to cut out only the plaque, leaving healthy tissue behind. We have found that this advancement has led to better results with less use of radiation.
