Sims 4 CC Disability, Chronic Illness, and Mental Illness Masterpost
I was getting frustrated with how hard it was to find CC for disabled sims so I made a masterpost of everything I could find. I have deliberately cast the net pretty wide, as you can see by the title. Some of the mods could also be used for able-bodied and neurotypical sims.

1- Download.
2-unzip to electronic arts/ sims 4/ tray folder.
Should pop up in your sims 4 gallery! :)

Simply Click the file table under the picture on the page and then download the file how you would download any other mods!
