1x15: Sam is trapped behind iron bars, and Dean tells him, “Must be getting a little rusty there, kiddo.”
5x22: While possessing Sam, Lucifer looks into the mirror to tell him, “You got me all wrong, kiddo.”
7x02: Dean looks at Sam and says, “Oh, you think I’m Dean. Right.” He grins and then the hallucination changes, morphing into Lucifer.
9x01: In Sam’s mind, Death watches as Dean puts a hand on Sam’s shoulder, saying, “Come on.” Then he begins to glow with light, and Sam looks confused as his face morphs into that of Gadreel’s vessel.
5x11: A psychologist opens Dean’s file, and says, “You’re my paranoid schizophrenic with narcissistic personality disorder and religious psychosis.” Dean looks rattled listening to this diagnosis.
7x17: Sam is on his bed at a hospital, and the hallucination of Lucifer sits behind him reading from a large book, saying, “Narcissistic personality disorder. Okay, now, this one I could have.”
5x03: Dreaming, Sam turns over in bed and jolts, surprised to see Jessica. She smiles at him and says, “Hey, baby. I missed you.”
8x09: Sam sits in a bar and takes out his phone, considering the text message he believed was sent by Amelia. The text reads: [Sam, I need your help. Come quick]
2x05: Dean glances at Sam and away while driving the Impala. He slaps a hand on Sam’s leg and offers a weak smile, saying, “You’ve always been a freak.”
13x23: Lucifer tries to avoid bringing Maggie back to life at Jack’s request, saying, “Right, well, Sam’s always been… sorta different.” He smirks.

Sam definitely took a photo of the initials before he left for Stamford.
He didn't know why, and for the first 6 months he'd look at it often. If it was 2010 or later, and any other sibling pair on TV it would have been his phone background or something, and that probably would have been enough.
But this is the irrationally erotically codependent Winchester brothers and as time goes on and he doesn't hear from Dean, thinking his Dean agrees with their absentee dad (that Sam should stay gone, that Sam had abandoned their family), he has to do something to make this symbol of home/love/belonging/family/Dean an indelible, permanent part of him. So he goes to a tattoo parlour and asks for Dean's initials. And when he's asked where, he immediately knows he wants it on his hip, somewhere no one's going to see, because this is too personal and sacred for people to know about.
Jess asks of course. Why, who, were you drunk or something? And Sam brushes it off. She jokes about him getting something for her and the rage that boils up in him is only seen on his face but they never discussed it again.
Then when Dean breaks in, and she hears "this is my brother, Dean" she knows, before Sam does, he'll never make it back to her, not in the ways that matter, because it isn't her initials tattooed on his hip.

i feel like the reason why sam makes such a good christ figure is that he's not just the chosen sacrifice of the story, he's also shunned and hated for it. the angels don't just see him as an abomination and unworthy of free will or empathy, they hate him because they know that when it comes down to it they also rely on him. after everything they've done, no matter how horribly they've treated him and convinced him that he doesn't deserve redemption, they still expect him to lie down and sacrifice himself. and he in the end he does. but not because they expect him to, but to prove once and for all that they were wrong about him and he can still be good. god what a character

after dean threw away the amulet and they are barely talking outside of cases and the world keeps ending and dean keeps getting quieter. sam watches and watches and watches until he can't and he gets drunk for once before dean does and when dean tries to push him onto his bed he grabs deans arm, giant hands closing around his brothers flesh like a cage. he let go before dean pushes him away, feeling like a stranger too big too monstrous in his skin and not the kid brother that dean deserves to have, used to have, but dean stays still. and sam is drunk and he should be slurring but his voice comes out small and clear and he asks Do you ever regret selling your soul for me?
