I love your meta about the nations doing stuff, but it's canon in both manga and anime that nations are known by the common people. Like, North italy literally greets his common citizens daily
hey! thanks for the ask and i'm glad you like my meta. to explain; i do take a really loose approach to canon. while i definitely follow some aspects of canon, i'll completely diverge from it if it suits my tastes—whether it's to do with the character's personalities or this sort of in-universe background about how nations are understood. 
so in this case, i feel like having nations always being "known" or understood to their people would be a missed opportunity to imagine some fascinating interactions that could occur if they were generally unknown but then "unveiled" at a particular moment. that isn't to say there aren't scenarios where nations being "known" at least to some people isn't an opportunity for interesting character work— imo, kiku being perceived as a kami by the shogun for example, is an interesting way to integrate their immortality and unearthly nature into their people's cultural cosmologies.
but on other occasions, i personally find it's more intriguing if the nation's existence isn't quite so known and accepted because that set-up is an opportunity to explore all kinds of themes. since in real life, ideas of nationalism and patriotism are heavily contested and people dispute the goodness of loyalty to nations or their very existence all the time. so the way i see it, nations being known and accepted all the time would be missed opportunity to explore them in relation to world history. for example; one interesting set-up might be someone finding the idea of an immortal, personified nation ridiculous because they believe they're living in an era where "science" and "rationality" has driven out anything supernatural or magical, and then having to reckon with the reality of the eldritch hetalias actually existing. or in other cases, like my personal headcanon about young Alfred— him not being accepted but being treated with suspicion by superstitious English settlers in 1600s colonial america because he does not seem entirely English + seems too intelligent and knowing for a young child. i also have similar thoughts about Arthur being perceived as a fae or changeling as a young boy. so generally, i think a nation being "known" and accepted can sometimes be a good background set-up for character exploration, but not always. so yeah, hope that explains my personal approach towards the r/ship between canon and my fanworks.
