Have you heard the legend of Paul Bunyan? It’s not the kind of thing that they’d tell you in that fancy college-boy college, no siree. Paul Bunyan was a real American, seventeen feet tall and capable of killing a British soldier just by looking at them real hard. What kind of car did Paul Bunyan drive? A goddamn ox, that’s what. Oxen are like a cow, but all fucked up, as evidenced by the special plural form of their name.
That said, he probably only drove the ox because nobody had yet invented the car. Nobody wants to drive an ox; they get terrible fuel economy and you have to keep feeding them even when you’re not driving them, sort of like keeping oil in a BMW. Henry Ford wouldn’t kill several bystanders test-driving his exotic hot rods for another couple of decades, which meant that Mr. Bunyan was plumb out of luck when it came to automotive enjoyment. I like to think that, if he were alive now, he’d eschew the current trends towards crossovers and crossover-based pickup trucks, and go right for the gusto: the Isuzu NPR.
“Isn’t that a commercial vehicle?” I hear you whine through my kitchen floor furnace grates, and although I am thankful you have stopped crying for food so often, do not mistake our newfound conversational ease for camaraderie of any sort. You will remain chained up down there until Nissan pays the ransom, which is not gonna happen while you’re making all this noise and making me forget to put the note for the aforementioned ransom in the mail.
Yes, the Isuzu NPR is a commercial vehicle. So what? Paul Bunyan’s a lumberjack. He can get his air-brake endorsement no problem, if he needs to. Really, the only obstacle for him would be to cut the roof off the vehicle so he can fit inside it like an enormous Power Wheels, which probably won’t even impact its crash safety all that much.

Daily Life in the Phyrexian Spheres (Maze to Bays)

The Maze is a thick tangle of coppered greenery and savage beasts shielding loyalist Phyrexia from the outskirts of the plane, posing a significant challenge to attackers moving in or rebels moving out. In theory, the domain ruled by Vorinclex and Glissa is loyal to Elesh Norn and stands ready to serve the Orthodoxy. In practice, most Maze denizens don't discriminate between Mirran or Phyrexian, loyalist or rebel. All are prey unless they can prove their worthiness to survive, and many an Orthodoxy enforcer realizes too late that their authority provides no shield from the dangers of the wild.
Though the Maze's savage reputation is well-earned, areas of tranquility remain, shielded by dense growth. Bubbling ichor wellsprings, nexuses of mana, and meadows of rare and exotic flowers await those with the skill to find them. Pockets of relative civilization also exist, such as the living quarters of priests, aspirants, and commoners, woven or grown into the landscape's coppery scaffolding. Common roles include the upkeep of local plant life and the study and augmentation of predators, carried out by Phyrexians known as stewards.
Not all Maze residents subscribe to Vorinclex and Glissa's ideology of weeding out the weak, though, and many form communities and care for those close to them. They are reluctant to aid outsiders, usually more out of self-preservation than an idea of social Darwinism (though many justify it with the latter). Luckily, no one is enforcing their obedience or ideologies--all Maze-dwellers must do is survive. Though predators run rampant, actual policing and enforcer presence is almost nonexistent here, making the Maze a suitable location for canny rebels to hide. In particular, the border between the Maze and the Furnace is a hotspot for illegal activity--another region that praetors consider beneath their notice.

The Bays are the heart of Phyrexia's surveillance state. Every feature of the landscape, from buildings to "trees" and the great skylights themselves, is studded with ever-watching eyestalks. The cameras' ubiquity reminds residents of their duty to obey--after all, you've got nothing to fear if you've got nothing to hide. Mind magic is heavily used, and memory redaction is common. The infrastructure of the sphere itself is also carefully micromanaged, down to local climate patterns. Control centers are overseen by ranking officials but staffed by commoners, who desperately beat back the unseemly Tangle growth that reaches down here.
Religion takes a backseat here in favor of hard science, and some researchers brazenly deny the Orthodoxy and claim experimentation as Phyrexia's truest way ahead. After all, the Progress Engine's primary export is research, and its titanic labs demand a steady influx of talent, raw materials, and captive test subjects (both Mirran and Phyrexian). Compleat creatures that fall short of their duties are often "repurposed" this way. Vast amounts of information are stored in archives of glistening oil and quicksilver.
Even in this sphere of eyes, however, dissidents carve out bubbles of secrecy. Drones and cameras can be covertly hacked, and mental defenses are meticulously constructed to mislead. Spy networks infiltrate nearly every level of Gitaxian academia, pilfering research and weaponry to aid the rebellion's cause.
A quirk of Surgical Bay life, which bleeds over into the other spheres, is the tendency of tools and inanimate devices to be replaced by living Phyrexian creatures known as skites. From syringes to chests and telescopes, even the most mundane items skitter on their own accord, and railings for them to walk on are built into most rooms. These simple creatures are roughly equivalent to trained animals, and variants are employed by both loyalists and rebels to spy on their opposition.

A Terrible Brother and A Worse Doctor: The Elusive Vache

Disclaimer: Spoilers for Slices of Life Before the Furnace: Origin of the Medicine and Blizzard Immunity, Natasha's Companion Quest chain.
In Origin of Medicine, when Oleg tasks the Trailblazer with checking up on the overworked Natasha at her clinic in the Boulder Town, we learn that the doctor has patients who have been absent for their scheduled check-ups. In her stead, the Trailblazer tracks down the missing patients and attempt to persuade them to go to the clinic.
The patients are stubborn in their refusal: why go for paltry treatments at Natasha's when the Mutual Aid Society has an easy cure for their ailments?
If this sounds like snake oil, that's because it is. At the conclusion of the quest we learn from Natasha that the "wonder medicine" being sold is the leftover results of experimentation on residents of the Underground by a former Overworld doctor, Vache.

Natasha tells the Trailblazer this:
"There was once a brilliant Overworld doctor with unmatched pharmaceutical skill. The doctor was set on developing a medicine that would steel the human body against the cold. He named his research project 'Blizzard Immunity'. But his research met with obstruction time and time again, the biggest factor being a lack of test subjects. Tampering with the human immune system carries huge risk, and nobody in the Overworld was willing to submit themselves to such experiments. As such, the doctor got in touch with his sister in the Underworld, claiming that he wanted to support the medical effort below ground. Naturally, his sister was overjoyed. After all, as far as medicine was concerned, her brother had always been the standard she'd strive to attain. She brought her brother underground and entrusted him with her clinic and laboratory. Issuing free medicine to the needy was just Vache's way of conducting experiments on his test subjects. On some level, his reagents were able to alleviate the patients' suffering... but nothing more. They would forget their pain for a while, and neglect their regular treatment plans. The result... patients with mild illness would attribute their recovery to the so-called wonder medicine, while patients with serious illness would quickly deteriorate... and sometimes die."
The sister in question is, of course, Natasha herself.
Natasha is the adoptive daughter of a prominent medical family in the Overworld, Yevgeny and Vanessa Harrower. Following in their parent's footsteps, Vache and Natasha enrolled in Belobog's medical school. While they initially had a close, loving relationship as brother and sister, Vache began to change the further he got into his research.
"He was gentle and introverted. But when it came to work he would be absurdly obsessive... like a completely different person. Whenever I made a mistake on an experiment, he'd bite my head off... and then a while later, he'd apologize and comfort me. He was conflicted in that way."
The source of his obsession: Blizzard Immunity.
From a young age, Vache wanted to develop a cure for the Eternal Freeze that plagued Belobog that would allow humans to withstand the frigid cold and thus rebuild abandoned settlements in the Snow Plains. This ambition, when met with roadblocks in the Overworld, would become single-minded obsession that drove him to seek out vulnerable human test subjects in the Underground.
After observing differing reactions to hot and cold from subjects with Fragmentum Corrosion, Vache began using said corrosion in his Blizzard Immunity. The "wonder cure" he handed out in secret to patients was not a miracle medicine that could heal any illness, but mere reagents that were nothing but temporary relief for symptoms and a way to continue observations in his research.
When his wrong-doing was uncovered by Underground leaders, Oleg had him banished and permanently denied access to the Underworld.

In the second part of her companion quest, Blizzard Immunity, Natasha learns from her family's butler that Vache had been sending correspondence for up to two years after his banishment to the Snow Plains - letters addressed to Natasha.

"Natasha, thank you very much. You helped me find the missing piece in the development of Blizzard Immunity."
This reveal, along with the unexpected sincerity conveyed from the letters, spurs Natasha into heading into the Snow Plains to search for Vache herself. The Trailblazer joins her to help. While out in the Eternal Freeze, the two encounter a troop of patrolling Silvermane Guards. After explaining their purpose for being this far out in the wilderness, they learn that the soldiers received special medicine from a mysterious doctor out in the plains which helps them withstand the cold.
Natasha deduces that this could be no one else but her brother, Vache. The guards inform her that he is elusive, only ever being encountered in person once, and since then the medicine has been dropped off at a specific location for them to pick up. In fact, they haven't seen him in two or three years, and it's unclear if he is still alive and well.

Natasha and the Trailblazer hurry to what was known as the location of his research base. It's a dilapidated shake nearly consumed by the snow, and appears to be long abandoned.
And it's guarded by a Fragmentum monster.
After defeating the creature, they are able to investigate the house. There, they find some of Vache's research notes. His last experiment recorded was two years ago, leaving off on a positive note of progress. Then, there's nothing. With no other clues, leads, or indication of what may have become of her brother, Natasha concludes that he finally succumbed to the cold he had spent his life trying to conquer. With the Trailblazer's help, she builds a grave for him off in the snow and says her final words to him.
"I can't forgive him for what he's done. But I nor can I say that I don't miss him... Maybe I should be glad. The medicine might not have been what he wanted it to be, but it came to something. Maybe he found some inner peace. They'll remember him as a murderer... as someone whose experiments brought great harm to the innocent. Vache doesn't deserve to be remembered as a "doctor". I know that very well. But maybe this Blizzard Immunity can now bring benefit to others... even if it was made by a monster."
"One day, the world will see a perfect Blizzard Immunity."
And that's seemingly the end of Vache's story.

A rule of thumb in story-telling is this: If there's no body, death cannot be confirmed.
There is no definite conclusion about Vache's fate. It is entirely likely that after years in the Snow Plains, the Eternal Freeze killed him, with a lack of resources and appropriate shelter. If not that, he just as easily could have fallen victim to the Fragmentum and turned into a monster - perhaps the very one lurking outside his laboratory that Natasha and the Trailblazer defeated.
Or, maybe, Vache had another breakthrough in his research into Blizzard Immunity. The medicine had already progressed far enough that the Silvermane Guards are able to wander the Snow Plains without danger, and Belobog in uncomfortably hot for them.
With a lack of test subjects in the wilderness, who else did Vache have to experiment on but himself?
Say he was taking every new version of the medicine. Like the soldiers, Vache would become more and more immune to the freezing temperatures. The closer to the city, the hotter it would be for him. His initial purpose was to help humanity venture deep into the Snow Plains and reclaim the lands they lost.
What else does Vache have to lose, now that he is banished and nearly forgotten?
I believe Vache is still alive and has wandered further into the Eternal Freeze. I believe he has perfected Blizzard Immunity, and with the Fragmentum at his fingertips, has probably enhanced his body to something beyond human, but not quite monster.
When we left Belobog in the main story quest, the civilization has been freed from the Stellaron and worsening conditions, but given the centuries of its influence, Jarilo-VI's ecosystem and climate has been drastically altered. It probably won't ever return to how it was long ago, and it will take many years of hard work to reclaim even a fraction of what humanity once had.
But there is hope. When we return to Jarilo-VI in the main story, I believe it will feature the people of Belobog starting to venture deeper into the Snow Plains. And who might we run into there?

In fact, a large reason why I have this theory is because I think Vache is not only alive, but will be a prominent character in Jarilo-VI's future storyline.
And I think he'll look a little something like this:

That's right. I believe Vache is a potential Il Dottore expy from Genshin Impact.
His Fatui Harbinger title says it all - he is The Doctor.
Thus far all the expies in Honkai: Star Rail have been from its sister game, Honkai: Impact 3rd. Himeko, Bronya, Seele, even Natasha, plus many more. Welt is actually the same Welt from HI3. While there haven't been expies from Genshin Impact (yet) it has been established that the world of Genshin does exist within the greater universe of HSR.
In Herta's Storage Unit within the space station, you can find Windgliders from Mondstadt, Wings of Flights.

There techincally already is a Genshin Impact expy in game, though he hasn't been seen yet. It's none other than our pigeon-loving friend, Timmy, mentioned by Natasha herself.

Dottore's past in Sumeru mirrors Vache's. He was a young, promising scholar who entered a prestigious school to pursue his research, which stemmed from an obsession with creating "enhanced humans" that could surpass even the gods. Like Vache, the pursuit of his research led him to using unethical and horrifying means to achieve progress.
It's implied that Dottore, then known as Zandik, conducted medical experiments on patients and was investigated by the Akademiya for an incident during field research that resulted in the slaughter of one of his classmates. [1][2]

In the cases of medical experimentation, Zandik studied the effects of Eleazar on patients, not in an attempt to cure, but utilize its effects to enhance humanity. After investigation, Zandik was eventually expelled from the Sumeru Akademiya.
Some time after then, Zandik is approached by Pierro, leader of the Fatui Harbingers, and is offered membership. [3]

"Will you call me a monster, a madman?"
Two researchers who fell into obsession to achieve their goal, desiring to find a way for humanity to adapt, overcome, and become something greater than the forces of the world.
Both are banished due to their unethical research.
One joins a military and political organization of a foreign country where he is given free reign to conduct his experiments.
The other is presumed dead.
For now.
EDIT - Added after original posting.
After completing Natasha's companion quests, the player recieves Blizzard Immunity Lab Records - the last of Vache's research notes.
The notes describe a sad and horrific fate for Vache after he tests an experimental agent on himself. The implication is that Vache is able to give the Silvermane Guards a more stable version of Blizzard Immunity before succumbing to the deleterious side effects of the earlier version he injected.

It's extremely likely that Vache is well and truly dead.
On the off chance he isn't, however, I think he has a lot of story potential.

Several hours later, Urabrask sat, reclining in Lab 1, replaying the past hours events over and over again in his head.
None of it made a Phyrexian Curse Word ounce of sense.
He grinned biting his knuckle; he loved it.
Confusion, chaos, questions; it fed his spirit each time something made him shake his head. Living people hated it because their lives were confusing enough, and there far more questions than answers already.
But... there was something insanely entertaining feeling your brain running around in circles as questions flew by.
He laughed and felt his brain relax; the Living worried about the future because it came so fast and yet worrying about it made even less sense than magical gold oil flowing down his spine into a stinger that wasn't there that morning.
The future was coming regardless, so why not enjoy every second you have with people and things that might not always be there.
He dropped his feet down from the one empty area on the console before spinning around in his chair.
He'd learned the sensation of your head feeling unstable and your stomach turning over on itself was called Dizzy.
Despite feeling ill for a few minutes afterwards, he found something enjoyable about the feeling too.
"idiot."
He glanced over at Jin Gataxias, "What's wrong Gaxy?"
"Your antics make no sense. You learn nothing from spinning in the same area again and again, besides your frail, imperfect body grows ill."
Urabrask laughed, "That's the point. It's called 'Fun', you giant stick in the mud."
"Negative. I am Jin-gataxias, not a Stick, and my current location is a holding cell, not mud."
Urabrask grinned broadly; turns of phrase made Jin not only confused as hell, but infinitely frustrated.
"It means you're a party pooper." Urabrask said, using another one Amelia had explained to him.
Jin growled, "One does not defecate a celebration."
As Urabrask burst out laughing Sheoldred sighed, "He means you are ruining his fun by being your normal self."
"Why did he not just say that to begin with?" Jin asked.
"Because it's funnier watching your brain blow a gasket." Urabrask said.
"My brain does not contain a gasket!" Jin Gataxias said.
Sheoldred groaned, "By the Oil, you are dense."
"My alloy density has nothing to do with our current conversation." Jin gitaxias said.
"BE QUIET! ALL OF YOU!" Elesh Norn shouted.
Urabrask's laughter abruptly stopped, "What's wrong? Not enjoying the show?"
Elesh Norn turned her eyeless face towards him, "No. You assist the imperfect in continue to exist in ignorance, you mock your fellow Praetors, you HELPED our enemies capture us and reduce us to torsos in glass tubes! What part of this is enjoyable?"
Urabrask got to his feet, and walked around the console towards Elesh Norn's tank, "Says the entity who took pleasure in destroying others individuality and free will. I'm sure becoming a test subject isn't something the great Elesh Norn would ever have thought of when she imagined her future... but let's be honest," he stepped close to the tank, "I would have burned you in my furnace before i ever bowed to you, you narcissistic zealot."
As he turned to walk back to the console, a sound above him caught his attention; hidden within the complex canopy of wires, tubes and ductwork were two crouching figures.

All Parts
Bones.
Stacks upon stacks of bones. Warped, mottled skeletons, ribs and spines that didn’t seem as if they could have belonged to a real, breathing creature.
Big ones, small ones, femurs like trees and trailing twisted tails that didn’t seem to have a beginning or an end.
All of them brittle and dry and old. Some even had begun to meld with the stone walls, slowly vanishing under new layers of rock.
Or perhaps being created from the walls.
I felt like a mouse in the den of a long-dead cat. Whatever used to live here had been huge, and been here a very long time.
Ancient, perhaps. Or Ancients.
Rrrrrrrrrt.
Already on edge, the sound of something rolling over the uneven ground nearly made me jump out of my skin. It came around the corner at speed, and then stopped unnaturally quickly.
As wide around as I was tall, it appeared to be almost entirely made of a single, enormous blue sphere, the insides swirling and spinning in unnatural ways. Large metal plates floated around the sphere, sitting in a way that was vaguely reminiscent of legs and a head. Except the head was just a four-plated pyramid, no eyes, no teeth.
It moved like it was alive, though, feet settling on the ground, looking for purchase, tilting its ‘head’ toward me in a way that made me feel watched.
Regardless, whether it was beast or machine, it radiated hostile intent like a furnace.
I hefted my staff, and the unknown creature responded in kind, lowering heavy feet to the floor and bracing itself.
By now, my preparations were taking a frustratingly long time. I didn’t dare enter wrath to attack, not when returning to calm took time I didn’t have. Beginning my battle hymn and mastering my reality was all I managed before it’s face opened like a flower.
Four large petals curled outward, revealing its glowing, swirling center. With a flash, a gout of blue flame shot out, aimed for my center of mass.
Even managing to throw myself out of the way of the main stream, particles of the liquid flame still grazed against me. They stuck like oil to my skin and clothes.
Then the pyramid began to take effect.
I’d actually bothered to test the strange relic before getting into a fight this time. It had been a subtle effect, more noticeable over time.
And here, just as it had in the lonely tunnels below, my mind shifted slightly.
When an idea came into focus, a path I could take, and attack I could make, it was a fleeting feeling. An instinct, a consideration of circumstance and opportunity and drive to survive. Before, that thought would have entered my mind and either been used or discarded without even the conscious recognition that a choice had been made.
Now, it… lingered. It was hardly there at all, but when I turned my thoughts back to before, the idea returned easily.
It wasn’t just an improved memory, there was something else to it. As if my mind continued thinking, even while I wasn’t paying attention, analyzing my surroundings. I wasn’t sure how useful it would be, but it wouldn’t be too difficult to find out.
My staff cracked into its outer shell, and bounced off. Quickly realizing the futility of attacking from the front, I instead hurled myself over it in a short, efficient vault. The rush of air as I flew sparked the scattering of burns on my skin, and I clenched my teeth as I landed to keep from hissing in pain.
But then I was behind it, facing a soft, blue, vulnerable core.
I didn’t even bother to speak the Word. Instead I just struck with fury, letting my battle hymn flow through my limbs to empower my smites, driving the butt of my staff into it several times. Whatever it was made of cracked like glass, and then shattered under my assault. I leapt out of the way as a thick glowing liquid gushed out and spread over the floor. The heat from that substance was intense, hotter than a fire, and I was forced to back up a few steps.
The creature—machine?—juddered, and then ground to a halt. The various pieces dropped to the ground, lifeless.
Hurriedly, I swept the spatters of liquid flame off my skin, snatching a bandage from my satchel to help. Thankfully, the pain subsided quickly.
In fact, the whole thing appeared to be cooling rather rapidly. The spilled ichor on the ground was darkening, hardening at the edges. Quickly, remembering the instructions, I snatched the White Beast from my bag and carefully dipped its face in the thick liquid.
The statue drank up the stuff, and then started to bleed from the eyes. Shocked, I nearly dropped it, and several bright red drops landed on the ground.
Where they hit, flashes of flame sparked, and then died.
Quickly, before I could lose any more, I grabbed another of the empty vials the Merchant had given me and gently tipped the dripping liquid into the round glass container. It didn’t spark on the smooth surface, and soon it was full, and the Beast had ceased its bloody tears. I corked the volatile mixture, and then tucked it very carefully away.
I very nearly forgot my meditation, which troubled me. It was a habit I’d deemed important, but the events as of late had shaken me up.
And, though I hated to admit it to myself, I was struggling to decide what I needed next. All my ruminations were leaving me with no tangible results.
Again, when I rose from my meditation, I had nothing of value to add to my kit. I did my best to accept the frustration, to shape it towards future growth.
But it was hard. Discouragement ran through my veins like a sludge, weighing down my limbs as I trudged ever higher through the terrible, bone-lined tunnels.
It did not help when I turned the corner and found someone all too familiar sitting there under a cube-shaped ribcage, with his wares spread temptingly before him on a colorful rug. It was a splash of vibrance in this dark, damp beyond.
“Hello, Watcher!” The Merchant said, cheerily.
Somehow, I felt like I should have expected this.
“Hello, Merchant.” I said, wearily.
