This is dependent on the situation. If it is role play, then many of them require the inmate to be in restraints at all times. Hampton Jail requires leg irons to be worn at all times and the inmate in additional restraints when not in the secure area of the cell block.
The real prison environment is quite different. Places like China and Thailand traditionally kept their inmates in leg irons for the duration of their sentences. This restriction is easing or has been eliminated in many countries. In the USA, I would highly doubt anywhere requires full time restraints. It would be a rights violation. You certainly will see inmates restrained during transport, court appearances, etc. but not during day to day incarceration.
In prison, an inmate will be restrained for disciplinary reasons, while on a chain gang work detail, during transportation to another facility or court and while moving from area to area. This is particularly true of maximum security, super max and death row inmates.

Kidnapping, Sexual Assault
In March 2022, Levi Austin Goss, was convicted and sentenced for aggravated kidnapping and aggravated sexual assault of a child from a 2013 incident. Authorities say Goss kidnapped a then 16-year-old who was walking along the side of a road, hit her in the head with the blunt side of a hatchet, dragged her into a wooded area, raped her then beat her again, leaving her to die.
The victim survived by pretending she was dead, then after the suspect left, she walked to a nearby movie theater for help, according to investigators. Although she was naked, witnesses said she was covered in so much blood, they thought she was initially wearing a red dress.
As Private First Class in the U.S. Army, Goss enlisted in 2016 and served as a parachute rigger with the 82nd Airborne Division. He spent most of his life under the radar of authorities, until the summer of 2017 when he was pulled over for a DUI in North Carolina and was also connected to another sexual assault.
Harris County investigators say Goss became a suspect in a 2017 sexual assault in North Carolina, which triggered his DNA being uploaded into a national database. In December 2017, his DNA matched DNA collected in the 2013 Harris Country rape.
The Department of Defense issued Goss with a Dishonorable Discharge.
