A classic scenario that could work in whump
Whumpee falls into a river that has fast currents.
Not only do they have to deal with drowning, but imagine that they hit rocks along the way, getting injured in the process. Plus exhaustion as they're struggling to keep their head above the water. They would even try to grab a rock but because they were so tired from all of the above, plus with how fast the currents are, they couldn't grab it. Maybe even chipping a nail or two from their attempt. Their struggles and screams for help become weaker as they lose strength to keep their head above the surface, before fully emerging in the waters.
Bonus points if the caretaker was running on the side, keeping an eye on the Whumpee as they find an opportunity to save the whumpee somehow. And seeing the whumpee going completely under, they were scared, maybe even jumping in the river themselves because oh shit they're drowning! BONUS BONUS points if there's a large waterfall/whirlpool/monster waiting at the end, which means the caretaker has to act fast or else the whumpee will die.

Some of my favorite words and phrases to describe a character in pain
coiling (up in a ball, in on themselves, against something, etc)
panting (there’s a slew of adjectives you can put after this, my favorites are shakily, weakly, etc)
keeling over (synonyms are words like collapsing, which is equally as good but overused in media)
trembling/shivering (additional adjectives could be violently, uncontrollably, etc)
sobbing (weeping is a synonym but i’ve never liked that word. also love using sob by itself, as a noun, like “he let out a quiet sob”)
whimpering (love hitting the wips with this word when a character is weak, especially when the pain is subsiding. also love using it for nightmares/attacks and things like that)
clinging (to someone or something, maybe even to themselves or their own clothes)
writhing/thrashing (maybe someone’s holding them down, or maybe they’re in bed alone)
crying (not actual tears. cry as in a shrill, sudden shout)
dazed (usually after the pain has subsided, or when adrenaline is still flowing)
wincing (probably overused but i love this word. synonym could be grimacing)
doubling-over (kinda close to keeling over but they don’t actually hit the ground, just kinda fold in on themselves)
heaving (i like to use it for describing the way someone’s breathing, ex. “heaving breaths” but can also be used for the nasty stuff like dry heaving or vomiting)
gasping/sucking/drawing in a breath (or any other words and phrases that mean a sharp intake of breath, that shite is gold)
murmuring/muttering/whispering (or other quiet forms of speaking after enduring intense pain)
hiccuping/spluttering/sniffling (words that generally imply crying without saying crying. the word crying is used so much it kinda loses its appeal, that’s why i like to mix other words like these in)
stuttering (or other general terms that show an impaired ability to speak — when someone’s in intense pain, it gets hard to talk)
staggering/stumbling (there is a difference between pain that makes you not want to stand, and pain that makes it impossible to stand. explore that!)
recoiling/shrinking away (from either the threat or someone trying to help)
pleading/begging (again, to the threat, someone trying to help, or just begging the pain to stop)

"Do you know what the French call it when you come, Whumpee?"
Whumper's whisper is warm and wet in Whumpee's ear. Whumpee squeezes their eyes shut, trying to ignore the rough hand slipping between their legs. They bite down hard on gag between their teeth, but still, their body jerks in response to Whumper's touch.
"They call it la petite mort, pet. A little death."
Whumpee's body coils like a spring as Whumper's fingers bring them closer to the edge.
"I wonder," Whumper purrs, "just how many times you'll die before I decide to kill you."

A drug-induced bedridden Whumpee.
They know the food and water Whumper offers them is laced with a sedative, but they don’t have the strength to refuse it. Their limbs are too heavy in their restraints. They can hardly string a sentence together, much less fight anyone off. Whumper guides Whumpee’s head, tilts the bowl to their lips, and warns them to swallow.

It's supposed to be for training.
That's what Vic tells them as he orders the team to strip, gag themselves, and hold still while he secures their wrists and ankles.
Training.
A mock captivity scenario, so they'll know what to expect if they're taken in on the mission. And maybe they're all a little anxious about what all that entails, but it can't be that bad, right? Not when they're a few days out from the mission, not when Vic needs them.
But as it turns out, it isn't themselves they should be worried about.
"Sometimes what you witness will be worse than what's done to you personally," Vic says as Sahota walks in, his eyes downcast. He strips in silence. Doesn't look at the team. Doesn't fight when Vic throws him onto the table.
"Sometimes, one of your comrades will get the brave, stupid idea that they'll take one for the team." Vic unbuckles his belt, and Sahota turns his face away from the others, the muscles in his shoulders taut. The team's shock turns to helpless horror as Vic forces his legs apart and suddenly thrusts into him.
"And all you can do is watch."
