As this picture shows, it's definitely possible to twist your other ankle while you're on crutches. I can't remember that this has ever happened to me, rolling my ankle while on crutches, but I'm sure it's happened to others. What does happen more often is that your good leg just gets worn out from picking up all the slack. Hopping around in a cast, without using crutches, will definitely result in a sprain if you do it enough.
