Quantum Computing’s Sputnik Moment

General Mark A. Milley, chairman of the Joint Chiefs of Staff, recently expressed grave concern about China’s reported test of a hypersonic missile: “I don’t know if it’s quite a Sputnik moment, but I think it’s very close to that. It has all of our attention.”  Maybe it should be, but General Milley may have missed the real 21st-century version of a Sputnik moment: China has claimed huge breakthroughs in quantum computing.  
It’s inside baseball to those of us who are neither computer experts nor quantum physicists, but let’s put it this way: the countries/companies that dominate quantum computing will dominate, full stop.  Healthcare included.  
I won’t pretend to understand quantum computers or try to explain how they work, but they’re to “traditional” computers as those computers are to, say, a calculator, or to an abacus.  They’re much faster – like a quantum leap faster – and can quickly do computations that would take even traditional supercomputers centuries to complete, if ever.  For example, think you’ve got an unbreakable code?  Unless you’ve got the fastest quantum computer, think again.  
The Global Times reported that a Chinese research team had achieved a 66-qubit programmable superconducting quantum computing system that is 10 million times faster than the fastest supercomputer and more than a million times faster than the previous fastest quantum computer, Google’s Sycamore.  And, oh-by-the-way, China has a second quantum computer that uses photons (light) instead of electricity, and that is “1 septillion times faster than the world’s fastest existing supercomputer,” according to the Xinhua News Agency.  
Hey, look, there’s a hypersonic missile over there!
Skeptics quibble over the speed comparisons – much depends on the problem and how the computers are programmed – but, as IFLScience wrote:” …with regards to sampling problems, it appears quantum computers are finally significantly better than conventional options.”   Bill Fefferman, a University of Chicago computer science professor, told ZDNet: “Early experiments hint that this technology will hold the promise to solve very interesting problems that cannot be solved classically.”
It’s not like Google is resting on its laurels.  This summer it used Sycamore to create “time crystals,” a new form of matter that appears to not obey the second law of thermodynamics (i.e. entropy always wins).  What this means for quantum computing, or for physics more generally, isn’t yet clear, but it’s a big breakthrough.  
Meanwhile, Amazon and Caltech are partnering to establish a new branch of the AWS Center for Quantum Computing on the Caltech campus.  The shared goal is “is to create quantum computers and related technologies that have the potential to revolutionize data security, machine learning, medicine development, sustainability practices, and more.”  It’s the first such corporate partnership for Caltech.  
Caltech professor Oskar Painter, who also heads quantum hardware at AWS, stresses that these are still early days for quantum computers: “If we were to just take today’s ideas and go forward with them, we would create a dinosaur of a quantum computer.”  Fellow Caltech professor Fernando Brandão, who also heads up quantum algorithms at AWS, says: “There’s a new paradigm in computing.  It’s not just about making our current computers a little bit faster or a little bit better as we have been seeing in the last 50 years at least. It’s about building a completely new kind of computer.”
Nadia Carlsten, head of Product for the AWS Center, is looking for, shall I say, quantum breakthroughs: “The quantum algorithms that have the most potential for significant impact, for example in industries like manufacturing or pharmaceuticals, can’t be solved by simply expanding today’s quantum technologies.”
IBM has also made quantum computing a major focus, stating that quantum computing “is on the verge of sparking a paradigm shift.”  Microsoft and Intel are also betting big on quantum computing, as are more specialized companies like Cambridge Quantum Computing, D-Wave, and Rigetti Computing.   
Healthcare is a big reason why.  Read any article about the potential impact of quantum computing and healthcare is always listed as one of the top two or three sectors it will impact (the military, of course, is another common one).  For example, the AWS announcement referenced “medicine development,” ZDNet just published “Eight ways quantum computing is going to change the world” that listed “Discovering new drugs” as #1.  
The article notes: “Quantum computers, however, have the potential to one day resolve the molecular simulation problem in minutes… This would mean that life-saving drugs, which currently take an average 10 years to reach the market, could be designed faster — and much more cost-efficiently.”
David Cowan, one of the major investors in Rigetti, explained to Forbes’ Kevin Dowd why he was so keen on quantum computing: “I mean, simply put, curing cancer.”  
Mr. Dowd goes on to elaborate: 
Perhaps the most exciting applications of quantum computing are in medicine. There are trillions of atoms in each cell and trillions of cells in the human body, all interacting with each other in an unceasing biological dance. Current superconductors are seriously powerful machines, but unspooling that kind of choreography is beyond their reach.
This is not going to happen overnight.  Quantum computers are not quite stable, not quite flexible, not quite available.  Your local hospital or physician group probably has more immediate concerns.  One hospital CIO told Becker’s Hospital Review: “I expect it will likely take another 3 to 5 years of innovation in hardware and subsequent applications to develop a quantum computing ecosystem that can truly benefit the healthcare industry.”
That is probably a safe bet, but changing to new technology in 3 to 5 years means that most health organizations would have needed to start looking it 3 to 5 years ago.  It’s a safe bet that the big health insurers, and huge health systems like The Mayo Clinic or The Cleveland Clinic, are already thinking about it.  
If you believe that Big Data, artificial intelligence, synthetic biology, the internet of things, or the confidentiality of patient records are going to be important in healthcare – and I certainly do – then quantum computing is going to be important as well.  That “internet” you think you know and rely on now?  We may be in the early days of quantum computers, but people are already working on the “quantum internet.”
The world is going to change, and quantum computing is going to make possible many of the things that are going to change it.   Healthcare should take the latest developments as a Sputnik moment.

How to Get Back into Ketosis After a Carb Binge
A little planning and motivation will help you start a low-carb, keto, or Primal lifestyle, and under normal circumstances, keeping your carbs on the low side is easy. But let’s not create the illusion that it is easy all the time. From time to time, you may get stressed and eat mindlessly. Or, your aunt drops off her blue-ribbon cake that you’ve loved since you were in preschool, and you give in, just this once. Or, you had a jam-packed day and all you can muster to make for dinner is that package of gluten-free noodles in the back of your pantry. The next thing you know, you’ve eaten enough carbs for a week, and you wonder how you’ll get back into ketosis after a carb binge.
The short answer is, yes you will recover from a carb binge. Yes, you will get back into ketosis. As far as how long it will take to get back into ketosis – that depends on numerous factors, that we’ll dive into here. The important thing to remember is, you did not obliterate your goals with one misstep. Especially after you’ve spent some amount of time in ketosis, your body will allow for fluctuations in carb consumption here and there. That’s called metabolic flexibility, which we’ll go into shortly.
  Can You Have a Cheat Day on Keto or a Primal Diet?
Admittedly, I’m not a fan of calling them “cheat days,” for a few reasons:
“Cheating” implies that you did something wrong and should feel guilty about it.
Earmarking “cheat days” sends the message that you can eat whatever you want that day with abandon. You’d be surprised how much you can backpedal on your goals in a 24-hour period.
I prefer to frame higher carb meals or snacks as carb cycling or carb refeeding, which is an intentional higher carb meal to enhance your results; or, frame them as treats, which are planned. That way, the extra carbs are enjoyable, planned in advance, and come with limitations so you don’t go overboard. And, there’s no guilt involved.
So, can you have high-carb days on keto? If you are in ketosis and have a sudden surge in sugar or carbs, your body will burn glucose instead of producing ketones. In order to get back into ketosis, you have to use up the glucose you just consumed, and the glycogen your body just stored.
The concern is whether the transition back into ketosis will be as difficult as you remember from those first days cutting carbs. If you have been in and out of ketosis for a while, you may slip back into ketosis fairly easily because you’ve developed metabolic flexibility. If you’re just starting, you may go through some of the discomfort of transitioning between fueling with sugar vs. fueling with ketones. Your body “remembers” though, and most likely, it will not last as long or be as severe. This article contains some things you can do if you experience “low-carb flu.”
What Happens to Your Body After a Carb Binge?
So, you decided to give in. First, don’t beat yourself up. It happens. What does your metabolism do with the surge of insulin and carbs? Even a few quick forkfuls can shift you from small doses of quality carbs wisely spread throughout the day to possibly 100 or more grams of pure sugar in one sitting. It’s likely you’ll experience some effects, but you can get past it.
First off, the good news. There’s no carb police coming to take away your keto card. Nor is there any other permanent fate awaiting you. You’ll go about your day a live, generally functional human being. There is no truly long-term risk elevation for that matter. Nonetheless, you’ll likely experience a fair amount of regret for cheating on keto.
Your Pancreas Kicks Into Overdrive. Within a few minutes, your pancreas starts pumping out a flood of insulin to try to sop up all the excess glucose that’s suddenly rushing through your bloodstream. Remember, while glucose is muscle fuel when it’s in the muscles, it’s toxic sludge when it stays in your bloodstream. Your body knows that and does everything it can to get it out of there. Perhaps you’re feeling flushed, a little high, spastic, anxious, or nauseous depending on how much you ate, how big you are, what your normal carb load is, and how acutely you tend to “feel” the effects of sugar and other substances. Ironically, if you were insulin resistant, you might not even notice these sensations.
Excess glucose converts into body fat. The gush of insulin now creates a see-saw effect. If your glycogen stores have room, some of the sugar goes into muscles. If there’s no more room, the excess goes into fat cells, where it is stored as fat. In reaction to this quasi-emergency that your brain perceives as a life-threatening stress, the body steps up its efforts to achieve homeostasis by releasing both epinephrine (adrenaline) and cortisol from your adrenals. Your heart starts racing, and you’re starting to feel uncomfortable, maybe even sweating. And we’re still likely within the first hour after you finished off that cake!
Sugar crash. After a bit more time passes, burnout settles in. That’s called a sugar crash – when all the glucose is gone from the bloodstream and you start to feel sluggish, off-kilter, like the internal circuits are all fried after sparking in a heap of now smoldering wires.
Your immune system slows down. The havoc that sugar rush set off – the swing of glucose and insulin, the cortisol and adrenaline – they’ve sent your immune system into a tailspin. Research1 has shown that the function of immunity-related phagocytes, the cells that surround and engulf pathogens, is impaired for at least five hours after intake of simple sugars. Free radicals, or damaging oxygen atoms, have their heyday as well within the first few hours after sugar increases oxidative stress2 on the body. Your blood even thickens as a response to the stressors. A hefty dose of sugar can compromise the immune system3 for more than 24 hours.
Your sleep is disrupted. At the end of the day, you try to sleep it off, but you toss and turn as your heart continues to beat faster than normal. Little surprise there – the old hormonal system is confounding in its interconnectedness. You lay there cursing not just that cake but the entire cultural custom of birthday celebration. As the sun comes up and you roll out of bed, you think you should be done with this sugar business by now. Maybe, maybe not.
How to Recover From a Carb Binge
As bad as this sounds, it could be worse. If you follow a Primal or keto lifestyle and the carb overload was just a detour, you’ll come out of this generally as healthy as you were before the flub. You’ll experience the effects, and you may feel them more acutely than you did before you chose the low-carb path. This isn’t a bad thing. Nonetheless, after the dust settles, the worst thing you can end up with is maybe a cold you wouldn’t have gotten otherwise. Your system will realign itself pretty readily. After spending a couple days back on your regularly scheduled program, you’ll be as good as new.
How to Get Back Into Ketosis After Cheating on Keto
So, you want to get back into fighting shape as soon as possible. Here’s what to do:
Scale back your carbs to where you were before you found yourself off-track.
Make sure you are getting the correct balance of electrolytes. Read this article to understand why electrolytes are important while transitioning to ketosis and how to make sure you are getting adequate electrolytes.
Consume sufficient high-quality fats, especially at first.
Don’t overdo the cardio. You can ease back into more intense aerobic exercise once you’re fully transitioned.
Consider intermittent fasting. You may have an easier time getting into ketosis for the long haul if you time-restrict food intake, which gets your body used to producing ketones.
How Long Does it Take to Get Back Into Ketosis?
You may wonder how long it will take to get back into ketosis after falling off. The answer is, it varies. It depends on how metabolically flexible you were before you started, how insulin-sensitive you are currently, how many carbs you were accustomed to consuming before you increased your carb intake… there are a lot of factors. The vague answer is, it won’t take long to get back. Start now, and you’ll get to where you want to be before you know it.

We are Asking Our Physicians to Do Too Much. It’s No Wonder They are Burnt Out.

We have heard the phrases “physician fatigue” and “burnout” too often in the last year – and for good reason. Covid-19 has placed an incredible burden on our healthcare providers. However, as healthcare professionals, the stats representing physician burnout are not new for us.  
We have seen similar trends and stats for years. Covid-19 did not cause the current state of physician burnout, it has just exacerbated it and further exposed critical issues with the expectations placed on physicians in today’s healthcare system. Ludi conducted a survey of physicians across the country confirming that exact theory:
68 percent of physicians feel pessimistic or indifferent about their occupation
48 percent describe the relationship with their hospital partners/employers as combative or transactional at best
We need to ask ourselves: What is actually causing this dissatisfaction and how can hospitals better align with their physician partners?  
According to the physicians surveyed, 68 percent agreed they have too much administrative burden placed on them. More often than not, our industry blames EHRs for dominating administrative time, but from the physicians we surveyed, EHRs are just part of the problem. In fact, 54 percent of physicians indicated they spend 1-3 hours per day on administrative work outside of EHR time, with another 35 percent spending more than 4 hours per day on similar tasks.
Let’s put that into perspective. On top of seeing patients, charting in EHRs, and all the other things physicians are expected to do to take care of patients, physicians are also spending at least another 1-3 hours per day on “everything else.” This everything else includes meetings, training, compliance, policy, etc.  
We are asking our physicians to do too much. It’s no wonder they are burnt out.
SOURCE OF DATA: Ludi Inc. compiled data on administrative work performed by more than 1,200 physicians between May 2020 and May 2021 that was logged through Ludi’s DocTime Suite. The notes entered directly by physicians represent how physicians spent their time for the last year when they weren’t seeing patients – as depicted in the word cloud. 
To better align with physicians, hospitals need to empower physicians to focus on what they do best and appropriately pay them for that work.
SOURCE OF DATA: Ludi, Inc. surveyed 100 random physicians in the United States currently employed by a hospital or a medical group or independent physicians contracted with a hospital.
The numbers speak for themselves and the ball is in the hospitals’ court to work with physicians to help them focus on patient care. In response, we are already seeing unprecedented levels of compensation redesign across the industry to better align physician and hospital priorities.  But these comp redesigns are just part of the solution. There’s plenty of low-hanging fruit that hospitals can leverage to improve their physician relationships right now – and it all starts with reducing administrative burden. 
First, hospitals should consider how and where technology can be used effectively within the nuances of their operations. They can start by asking themselves a few key questions, such as: Are there tasks or processes physicians are doing on paper right now that could be automated? What does this manual process involve and how does it impact different departments? What are the most time-consuming tasks for physicians each day, each month?
Of course, hospitals should also make sure the reason they’re implementing new automation in the first place is clear to them. That is, they should be solving for specific pain points (and agree on the priority of those pain points), and with direct feedback from the stakeholders themselves, in this case, physicians. If a hospital implements a solution that doesn’t take into account the physician’s daily routine or workflow, then it’s likely the new automation will fail. And when you’re dealing with doctors, you want to make the solution is as easy and quick as possible, given everything else they have to deal with throughout the day. 
The fact of the matter is though, when it comes to automation technology, more than 83 percent of physicians we surveyed agreed automation technology focused on reducing administrative workloads will improve their productivity and job satisfaction. That’s good news for hospitals.
Why? Because the less time physicians are spending on administrative tasks means more time spent with patients and other more meaningful tasks. And that’s a win-win for everyone. 
At the end of the day, hospitals should always be focused on what else they can do to support their employees from a morale perspective. I worked for health systems where we had an option to drop off dry-cleaning in the lobby – a simple task but something that physicians may not have time for given their demanding schedules. Now, take that example, apply it to the big picture, and go for it.
