Endometriosis

Endometriosis—the struggle is real.  Killer cramps are NOT normal.  Periods that last longer than 7 days are NOT normal. Heavy bleeding that soaks through a tampon every 2 hours is NOT normal; pain during sex is NOT normal. Bouts of diarrhea and vomiting that accompany every menstrual cycle are NOT normal. No, no, and no!  For many people, this reality is just endometriosis at work.
Sad truth: Many of us are taught to downplay these symptoms. Our pain is diminished by parents, siblings, friends and even health care professionals who convince us that everyone goes through this.
Maybe that is why, according to the Endometriosis Foundation of America, it takes 10 years on average to receive an accurate endometriosis diagnosis. That’s a decade, people! That’s 130 periods of agony, 912 days of someone asking you to take Advil and suck it up.  That…is not okay.
Endometriosis is pervasive.  It affects 1 in 20 Americans of reproductive age and an estimated 176 million people worldwide. It occurs when tissue similar to the endometrium (the lining of the uterus) is found outside the uterus on other parts of the body.  
There are lots of symptoms that can vary among patients.  Pelvic pain is most common, as well as pain that coincides with menstruation.  Other symptoms include heavy cramps, long-lasting bleeding, nausea or vomiting, pain during sex and, unfortunately, infertility.  Some people may even experience symptoms throughout their entire cycle—a real drag.
In addition to these physical symptoms, endometriosis takes a toll on someone’s personal and professional life. Chronic pain can severely affect quality of life day-to-day; medical care can be extremely costly. Furthermore, absenteeism can alter relationships in the workplace and at home.
Despite the intense discomfort, many people do not realize they have endometriosis until they try to get pregnant. And because the disease tends to get progressively worse over time, approximately 30-40% of people who have endometriosis experience fertility challenges.
There is no simple diagnostic test for endometriosis—no blood, urine, or saliva testing can confirm the condition. The only way to verify endometriosis is to undergo a diagnostic laparoscopy with pathology confirmation of biopsy specimens.  
On the bright side, many endometriosis symptoms— including infertility—can be addressed after diagnosis. The gold standard for endometriosis treatment is laparoscopic excision surgery. This involves a careful removal of the entire endometrial lesion from wherever it grows.
The first step to getting there is recognizing that your pain is not normal and seeking timely intervention. The earlier endometriosis is detected and treated, the better the results. Tracking your symptoms will make you better informed for your next doctor’s visit, and set you on a path to better (and less painful!) menstrual health.

Hi Joy! I know this might be a difficult to answer question since people are different, but what IS a normal period? Is it really normal for so many women to suffer extreme pain, dizziness, and worse for basically a week every month? That just doesn't seem like a stable condition, what the hell nature? Also other mammals don't spend a week almost dying every time they don't procreate? Idk my period just ended and I'm a bit pissed off at it for ruining my week.
A thing a lot of people confuse when it comes to this kind of thing, is accepting something as normal, simply because it is common.
It used to be thought that conditions like endometriosis were super rare, and now it turns out something like 1 in 10 people have it, it just doesn’t always manifest badly enough for people to be concerned because we’ve been conditioned into thinking extreme pain/other debilitating symptoms from our menstrual cycle is normal because medical science hates anyone not reasobably healthy, white and male.
Sometimes these symtpoms can be the result of deficiencies, which are exasperated during the menses. Other times it’s a warning sign that something else ain’t quite right. Like endo or ovarian cysts. Or even a thyroid dysfunction. Sadly you’ll likely have to fight hard to find out.
As for a “normal” period, it’ll vary from person to person due to personal physiology, but things like bleeding through a tampon or pad in less than a few short hours is considered too much, and can cause other health issues to occur like anemia. Vomiting and or blacking out is definitely not normal either.
Some discomfort is to be expected, but if taking ibuprofen and a heat pack to your lower back/abdomen doesn’t help, there’s a chance your pain is too high to be normal. Not simply that you are too wimpy.
Some minor stomach upset like constipation, gas and slightly loose bowel movements are also somewhat within the normal range due to how hormones affect your stomach/intestines but it shouldn’t be debilitating, nor should it last your whole period. Same with migraines or dizziness, which can also be a sign of a hormone imbalance or an underlying deficiency, usually from the d and b groups.
Period blood that is dark red to almost purple in appearance can be completely normal every now and then, but if it’s persistent it can be a sign of too much estrogen. Just like a watery pink/orange period can indicate an imbalance with prestogen, I believe.
This is all just from personal experience, before anyone jumps on me. I’m not a doctor so I can’t diagnose or offer anything more solid than this, but I do deal with a lot of shit and had to fight to get help so hopefully this will help someone.

My Period Journey
I got my period completely unexpectedly. I hadn’t even been told a thing about it and was absolutely convinced something was wrong with me when I saw blood in my panties. I had to go, crying and scared, to my teacher. I had to sit, embarrassed and bloody, in the office and wait for my stepmom.
I was 9
The blood was thick, heavy, and I felt like I was going to throw up. My stomach rejected food, the part of my body I wasn’t even fully aware of yet was now always sticky and wet and gross and I was told it was completely and totally natural. No one told me it was okay to be afraid. No one prepared me.
“It’ll be over in a few days,” they said.
“It won’t come back until next month,” they said.
I was 10
Sleep started to elude me almost completely, and then I’d get so tired that my father had to literally drag me into a standing position so I’d start to become conscious. My stepmom didn’t explain that if my pad got full I could change it. She yelled at me because pads are expensive. I ruined almost all of my underwear because I didn’t want her to yell at me again. My dad refused to acknowledge it had happened at all. He has four daughters.
I was 11
A sharp pain gripped my side and I could barely breathe. I didn’t feel good. I begged to stay home from school. I was crying and clutching my side. Something wasn’t right.
“It’s normal,” they said.
“Don’t be so dramatic,” they said.
I passed out in science class. Woke up in the doctor’s office because my small town didn’t have a hospital and was told I’d had a ruptured ovarian cyst. I had four more cysts on my left one and at least three on my right. I needed to be on birth control and tested for PCOS.
I didn’t even know what an ovary was.
I was 12
My dad flat out refused to get me birth control. Said I didn’t need it. That there was no reason for a twelve year old to get on the pill. I’d just start having sex and who knows what else and that was that.
He’s a nurse.
I was 13
“What do you mean you’ve gone through the entire box of pads already?” my stepmother demanded, loud, shrieking. “There were 20 pads in there! How many days do you bleed?”
I didn’t know I was supposed to count.
“When does your period start? How many days between stop and start?”
I didn’t know I could count.
I started marking it all on my calendar. Some months there was nothing. Some months over half the days were filled in. I stole an entire box of pads from under the sink to hide in my room for my very own.
I was 14
New year, new calendar. I give my period tracking one to my stepmom and take her through it page by page. My periods last 10 days at the least. There is no consistent day my period begins and I show her.
“You just counted wrong,” she says.
I was 15
My legs swell. My back aches. I’ve got a headache. I puke up my dinner and shit out my breakfast five minutes after I ate it. I’ve bled all over my bed.
“You’re overreacting,” they said
“Don’t be such a baby,” they said
I was 16
I can’t eat for two straight days because if I do I will throw up. I’m not sick. I’m on my period.
It’s normal, I think.
I’m 17
I go through 40 pads this time.
It’s normal, I think.
I’m 18
I gained three pant sizes right before the blood shows up. I lay in bed all day with a heating pad across my shoulder blades, on my lower back, and one across my stomach. It doesn’t really help.
It’s normal, I think.
I’m 19
My own money. No health insurance. I moved away. Saw a doctor. I’m on birth control pills.
I’m 20
The pills have stopped working at easing my blood flow. The doctor tries a new pill. It does nothing. The doctor tries another pill. I can’t afford it. I don’t go to the doctor for four more years.
I’m 24
My girlfriend drags me to the doctor with my state health insurance. She tells the doctor about my symptoms. The doctor’s mouth opens slightly.
“That’s not normal,” she says.
I bleed for 28 days straight.
I’m diagnosed with Polycystic Ovarian Syndrome. The doctor asks if I want an IUD. I’ve never even heard of that.
My insurance pays for it. It’s free.
“Okay,” I say.
“It’s worth a try,” I say.
I haven’t had a period in months.
I am 25
My oncologist examines my medication list. “IUD? Birth control?” he asks. “You’re married. Don’t you want kids?” No. “What about your husband.” Wife. “Oh.”
My GP is out of town. I see a new doctor. We’re discussing surgeries. Is a hysterectomy an option?
“No,” he says. “You might marry a man who wants kids.” I’m married to a woman and I don’t want kids.
My dad is a nurse. He has four daughters.
“You’re married to a woman. Why are you on birth control?”
“Because I need to be,” I say. Finally. I say. “Because I want to be.”
Because it’s my body. Period.

God okay so I’m going to say this now; PLEASE PLEASE PLEASE do not risk your health or anyone’s in the form of getting a “cheap” binder of ebay or where ever. PLEASE PLEASE PLEASE save your $4-$10 dollars for those cheap ebay ones and put it towards a gc2b binder. Or if you want an Underworks go for that but I highly suggest gc2b. They’re about $40 (with shipping) and will last you WAY longer than any cheap ebay one. They are MEANT for lasting you. I wash mine many times week, wear it daily, Wear them daily. I have two. Had them for almost a year now. But I cannot urge you enough to PLEASE save up your pennies for a PROPER SAFE binder. Binders you can breathe in, binders you can move in, binders that don’t crush your ribs, binders that are comfortable, binders that are safe.

If you can’t afford a binder, don’t despair! Used binders are often passed on by post-op trans men or those whose binders may no longer fit. There are a few programs available that help distribute donated second-hand binders:

How to Choose a Chest Binder
If you still remember your old bra size, you can find out your binder size by using theBra to Chest Size Converter Tool. If you don’t know your old bra size, you can measure yourself the old-fashioned way:
Take a snug measurement of the fullest part of your chest using a tape measure (best if measured while clothed) and write that number down onto a sheet of paper.
Measure underneath your chest where the crease is and write that number down as well.
Add those numbers together and divide the sum by 2. This number will differentiate your size not only from brand to brand but from binder to binder as well.
Selecting a binder brand and style can be difficult: there are so many options that it can be overwhelming! Plus, there aren’t very many reviews of binders other than those about Underworks’ and T-Kingdom’s more popular models. After buying your binder, help make the experience easier for guys in the future by contributing your review to one of the review sites listed below.
Essentially, there are two types of binders: short ones and long ones. The short ones end right at your waist. The down side of these is that if you carry some extra weight, short binders tend to roll up and act more like a bra. The long ones can be pulled down past your waist by several inches, however it’s inevitable that it will still roll up. To reduce the chances of this, wear a belt. Choosing between a short and long binder has more to do with your body type, specifically your abdomen, and not your chest size.
Lastly, consider the location of the company you’re buying from. Buying from a company that’s closer to you can save you a significant amount of money on shipping costs.
How to Put On a Chest Binder
It might seem silly, but you’re probably going to need some help figuring out how to put on your new binder, particularly if you purchased one of the longer styles.
Put your binder inside out and upside down.
Step into your binder and pull the bottom of it up, ideally to your belt line. The binder should still be inside out and upside down.
Use the sleeves as handles to pull the top of the binder (the end closer to your feet) up to your shoulders.
Put your arms through the sleeve holes and adjust your chest to your needs. You may need to pull the bottom of the binder out from underneath itself if you don’t want it folded under. For others, leaving it folded under may help stop the binder from rolling up.
Don’t be disappointed if you look in the mirror and it looks like you have one big boob in the middle of your chest. You just need to adjust your chest. Reach in from the neck hole and push your chesticles down and out. You’re basically pushing your nipple toward your armpit to achieve the flattest looking chest possible.
FTM Chest Binding Tips
Very important: When binding, you should not by any means feel as though you can’t breathe or like you’re going to pass out from a lack of oxygen.
Binders aren’t the most comfortable things in the world. To make binding more comfortable, and to reduce the possibility of the binder moving around a bit, some guys wear a light shirt underneath.
Depending on the size of your chest, you may need to layer clothing on top of the binder to get optimal chest flattening. You’ll find that some of the shirts in your closet require you to layer more than shirts in your wardrobe.
You can swim in your binder. Just wear a sleeveless or sleeved T-shirt over it. Don’t worry if your binder seems less effective after a swim, this isn’t permanent. Simply wash it and it will go back to normal.
Your chest will look bigger than it really is when you look down at it. Check in the mirror for a more accurate side view.
Not all binders breathe well, and the reality is that you’re probably going to get hot. If you’ve already started testosterone, you’re definitely going to sweat. The build up of sweat can irritate your skin causing rashes and sores. Wearing a thin cotton shirt that breathes well underneath your binder may help prevent this. If you find this uncomfortable, try applying corn starch to your body before putting on your binder to help keep it from holding in moisture. If you’ve already experienced skin irritation of some sort, take care of it the same way you would an open wound. Washing the irritated area with anti-bacterial soap will keep it clean and help it heal faster.
Conclusion
Chest binding, as cumbersome as it may seem, can be very freeing for transgender men. There’s a plethora of quality FTM chest binding products available for body types of all shapes and sizes. Regardless of what you use for binding, please remember to put your health first. Now that you’re armed with all the information you need to find the right binder for you, go forth and feel more comfortable in your skin!

vaginal hygiene & health - an introductory crash course

- the vagina is the tubular ‘internal’ sex organ that some people have. - the ‘vulva’ is the set of external sex organs that people with vaginas have. it includes (but not only):
pubic mound 
labia minora and majora (inner/outer ‘lips’)
clitoris and clitoral hood 
vulval vestibule (found in between your labia minora)
urethra (where you urinate from)
vaginal opening
-vaginas naturally contain a balance of different bacterias. this bacteria protects the vagina from external bacteria and fungus, provides natural disinfectants and maintains a healthy vaginal pH of 3.5-4.5.
-vaginal pH varies from person to person, day to day, depending on your cycle, your diet and many other external factors,
-all vaginas have a scent. all of them. no two people smell exactly the same, and you shouldn’t expect your vagina (or anyone else’s) to smell fruity or floral.
- if your worried about your scent, you can place perfume or an essential oil to an area around your vulva but not on it. some places like between your thighs or on your lower stomach, for example,
-your body secretes natural fluids called discharge. discharge is how your vagina self cleanses. it is completely normal and vital to healthy vaginal function.
-discharge can vary in amount, color, texture and scent depending on your cycle. each person is different. if your discharge is unusual or abnormal to you and your unique body - visit a doctor if possible.
-abnormal discharge can be green, grey, very yellow, smelly, itchy, chunky, etc.
-abundant discharge can be annoying. you can remove excess discharge in your shower by inserting one clean finger into your vagina, moving it from one side to another and ‘scooping’ the discharge out. repeat as needed. this reduces the amount of discharge found on your panties and in turn, reduces vaginal scent.
- dead skin cells and oil can build up between your clitoris and your clitoral hood, so it’s important to lift your hood up and rinse thoroughly so it doesnt harden. 
-washing your “external” vulvar areas such as your public mound and outer lips is okay, though some may find skin here more sensitive.
-don’t douche. you may think it helps but it only masks any problems while causing more. douching throws of your flora, alters your PH and forces bacteria into your cervix.
-water and a soft, clean wash cloth is all you need to clean your vulvar areas that contain a mucous membrane. you do not need to wash these areas. applying soaps or cleaners to these areas removes of natural necessary lubricants and bacteria. this may cause irritation, inflammation, major discomfort and dryness. some amount of moistness should always be present.
-a vast majority of vaginal washes, even those touted as hypoallergenic, still contain dyes and fragrances which are known irritants. read the label.
-while Summer’s eve is a lesser of evils with a PH of 3.0 - 3.5 (still not quite correct), it is still a cleanser. vaginal washes are used to cleanse “bad” bacteria from your vulva. they also wash away the “good” bacteria needed to maintain a healthy balance.
-castile soaps (like Dr. Bronner’s) has a pH of about 8.9. these are not suggested.
-never insert any cleansing agents into your vagina.
-the information above applies to any gels, deodorants, perfumed products and wipes, too. these can all disturb your natural balance.
-while they still pose a possible risk of irritation, unscented baby wipes are the lesser of evils regarding “freshening up”.
-pay attention to your irritants and triggers. different people, different reactions
-everyone has different stances and different methods that work for them. if you aren’t experiencing vaginal discomfort or odd changes, you don’t have to discontinue certain product use. i do, however, recommend a trial period of leaving these products alone due to their effects on your vagina.
-allow your vagina to breathe. keep too-tight clothing to a minimum. cotton panties are the best option next to not wearing any at all. rotate the washcloth and towels you use to clean and dry your vagina.
-urinating after masturbation or sex reduces the amount of bacteria in your urethra wiping ‘front to back’ keeps rectal bacterial away from your vagina.
-naturally, food alters your vagina. balancing your diet in general and including things like pineapples, strawberries, yogurt, soy (the list goes on!) can influence your pH.
-beverages like water, cranberry juice and pineapple juice are also proven to improve vaginal health, and may even improve taste.
-adding a probiotic supplement can increase the amount of “good” bacteria called lactobacillus-your vagina.
-the less ideal your vaginal balance/pH is, the more vulnerable you are to infections and STDs.
-when it comes to internet home remedies, your mileage my vary. try everything with caution and research thoroughly.

Shit No One Told Me About My Period
I knew the basics before I got it, but I had no clue…
* The blood wouldn’t necessarily be red. When I first got my period, I spent a few min looking at my underwear wondering how I shit myself. I didn’t know the blood could look brown, or be thick.
* That tampons weren’t a good idea yet. I was 10 or 11 when I got my first period and physically smaller than an adult woman. My first attempt at inserting a tampon was very painful and unsuccessful. I wouldn’t use them until I was around 14 or so.
* That when you use pads the blood can get on your bottom and I’d have to occasionally clean off the toilet seat after using it.
* That getting your first period DOES NOT mean you’re fully developed and fully able to bear children. I could have technically gotten pregnant at that age, but I was still a child and pregnancy would have put my life in danger because I was still physically immature.
* That it wouldn’t be regular for another few years.
* That very painful cramping is NOT NORMAL once you reach your 20s and is cause for concern.
* That the blood and tissue you pass can look chunky or stringy and not like blood from a cut.
* That stress can halt your period for months BUT
* That doesn’t mean you can’t get pregnant

Gender and pleasure
So much of the Euro-American understanding of being trans (or anything other than 100% constantly identified with your assigned gender) focuses on discomfort. 
Some people take this idea to an extreme and claim you can’t be trans unless you hate your body and want every surgery available to you. As many other writers have said before, that’s not true. It’s perfectly possible to be trans with only mild dysphoria or none at all. It’s perfectly possible to be trans and have a mental map of your body that looks just like the one you already have. 
But I’d like to push even harder against the idea that trans=discomfort. I’d like to offer this: sometimes the exploration of one’s gender can be motivated by pleasure rather than discomfort. 
Let me give an example. Let’s say there’s a person named Cal. Most people think of Cal as a boy, and Cal’s all right with that. So far as Cal’s concerned, a boy isn’t a bad thing to be. But sometimes, Cal likes to imagine being a girl and being treated as a girl. Those fantasies are always accompanied by feelings of pleasure, satisfaction, anticipation, and warmth. Eventually, having had these thoughts for years, Cal asks people to use ‘she’ pronouns in private and to refer to her as a girl. Cal does this for another year before claiming the label “trans”. 
Some people would say a person like Cal can’t be trans because there’s no dysphoria, self-hatred, distress, or even discomfort. There’s just a pleasure-based preference. But why is distress necessary? Why are trans people supposed to be defined solely by our pain and self-hatred?
It’s my opinion that defining trans people solely by discomfort is an aspect of transphobia. The idea behind trans=discomfort is that being anything other than 100% cis is so awful that no one would do it unless the alternative were unlivable. Think about that: defining trans people solely by their experiences of discomfort means believing that being trans is so awful that only misery could drive us to it. And to me, that sounds like the thinking of someone who really hates trans people.
So I’ll come out and say it: sometimes transition or self-exploration of gender is not just about lessening discomfort, but is about improving and deepening the pleasure we take in our lives.

Are old training bras a safe method for binding? I found an old one in my drawer that I think might work, but I’m worried about potential risks.
Hmmmm, could be. While nothing’s as good as an actual binder, a training bra that’s tight enough is pretty similar and has much the same effect if you have small breasts.

Remember, just because it isn't binder doesn't mean the standard rules don't apply!
Wear it for <b>short</b> amount of time, definitely shorter than 8 hours a time!
If you feel any pain or have problems with breathing, or you're just super uncomfortable, <b>take it off instantly</b>! [If you wear it to school, remember to take with you an alternative [another bra] for that very scenario.]
Following a tip shared by someone who was told it by a doctor - make sure your nipples are placed upwards :)
Remember also, that a sport bra will have lower level of compression than a binder, so it won't make your breast disappear completely. It will make them somewhat smaller though.
If you don't have access to an actual binder, this will be a good alternative. Though the best thing would be to get a binder as soon as it will be possible for you :)

As long at it’s comfortable and fits you.
If it’s uncomfortably tight don’t wear it. I’m not sure if too small training bras would damage your chest and ribs as much as a too small binder, but even if it doesn’t, your comfort is most important.
