#pain killers
Text
So, a lot of chronic pain patients are understandably wary to the suggestion that we might be addicted to our pain meds, but it is actually very possible and I really wish we talked about it more. Like addiction to pain meds is likely less common than ableds would have us believe, but that is still possible and we should address it more.
Like if you are taking more meds then you are supposed to then that is a problem. If you take your meds when you don't really need them, or mix them with other substances to increase the effects that's a problem. If you're lying to people to get more meds, or hiding the amount you're using, or stealing to get more meds that's a problem. If you're meds start effecting your mental health or your ability to function then that is again, a problem.
That doesn't mean you have to stop taking your meds, and effective healthcare should never force you to do this. But if you've noticed you have an unhealthy relationship with your meds, or if a loved one has expressed concerns about your use thrn it may be time to reassess your relationship with your prescription. Try to get in touch with harm reduction programs to say what steps you can take to keep yourself safe. I go to a SMART recovery meeting once a week, but you may find you have different services in your area.
Being an addict doesn't mean there's anything wrong with you, or that you've done something wrong. There are many factors that contribute to addiction and not one of them is that you weren't trying hard enough to use a specific substance responsibly. You are not alone in this. Stay safe.

Okay biopsy over. Despite all the pain meds it was still so painful I cried, but the meds made me relax so it went faster and it was less painful than last time. So, I made it. Now I'm feeling woozy and headed home soon once they're sure I wont pass out. Thanks everyone for the support. I love to all.
It's so hard to type right while medicated but I'm trying for you guys.

Medicine in Castlevania
Tumblr media
Had a talk about this topic with @autumnmobile12 and promised I was gonna write a bit about it.
There is the big question, what kind of medicine the vampires have available in the Netflix version of Castlevania. After all Lisa goes to Dracula to learn about medicine and does so. And we know that the knowledge available is far ahead of the time that the series is set (mainly 1475 and 1476).
We see a lot of chemical equipment in Dracula's laboratory, but of course we cannot say, what it does.
The one thing we know is that they know about antibiotics. This is based on Lisa giving the old woman in the village a medication for her cough made from mold. Which is fairly certainly penicillin.
Given how penicillin in the real world was discovered, we can assume two more things from this. In the real world it was discovered, by some spores from a certain kind of mold (one, that primarily grows on melons and sometime on citrus fruits) getting on a petri dish, where they were cultivating bacteria cultures, and then killing the bacteria. Hence, them knowing about penicillin does imply that they know about how to grow bacteria cultures - and hence know about bacteria. After all you would not give someone antibiotics without knowing about bacteria!
Another thing we know is, that Lisa in her own laboratory in Lupu has a centrifuge. Usually centrifuges are used either in some chemical processes, but more likely is, that it is used in blood tests. Which would make it clear that they have some knowledge of the components of blood. This also does imply that they can draw blood in a somewhat orderly manner.
So, in the real world penicillin was discovered in 1928, while blood tests go back to the 18th century.
Some other stuff that was discovered medically in the early 20th century would be insulin (which originally was made from the organs of animals, which I absolutely could see to be something that the vampires in Castlevania have figured out) and vitamins and their importance for the human body.
In the about 100 years before that, other important stuff was discovered. Aspirin being one such thing, as it was originally made from the bark of willow and I very much assume that they have already access to. X-Rays, too, being such a thing. Though I am not entirely certain about these, because while they do have electricity, I am not entirely certain how much they understand about electricity. Something I am rather certain about is general anesthesia, which was discovered in 1849 in the real world.
Vaccines were discovered technically in 1799, but it took until 1890s, until they figured out to prepare dead vaccines, that would just train your immune system, instead of giving your a minor infection. But yes, I am fairly certain that they do know about vaccines.
Some other discoveries, that would be around in the 1920s, are kinda dependend on how well the understanding of electricity is. Because the only electricity we see is in electric light. So, it is hard to say whether they have electro cardiography for example. Though obviously it would help with the survivability of surgery.
I am not entirely certain, whether they might have some other medications that need modern chemical equipment.
Another thing I am fairly certain they have, is proper microscopes, given that those go back really far back and even in the middle ages they had some good ones and they probably have access to much better ones in this world.
But just to imagine, how much the knowledge of bacteria, fungi and maybe viruses would help survivability. Just knowing to desinfect hands before a surgery (something we see Lisa do) and such would do a lot. Heck, antibiotics would greatly help the survivability of the black death and of course vaccines would do so much more. So, if they really end up sharing that knowledge in the end of the series, the next few centuries would have a very different outcome.
(I might also ramble a bit about technology in that world, if anyone is interested.)
Tagging @udaberriwrites and @lena-hills, too, because they might be interested.

SO I am having a bad pain day, and I am reminded that for people who suffers from pain, addiction to pain killers can be pretty much unavoidable, because between the pain and no pain I know what my choice is.

Salve/ Pain Killers/ Bad Coping Mechanisms
@whumpril day 6
CW: swearing, drug abuse, implied slavery, suggestion of alcohol abuse
Caretaker watched as Whumpee held their hand in front of their face and spread their fingers, examining the damage of their latest fight. Caretaker had managed to get the bleeding to stop before Whumpee insisted they stop toughing them. They were surprised Whumpee had let them do that much since they had a bad habit of insisting they could do everything on their own.
There was a nasty cut on Whumpee's palm and a bite mark below their thumb. Their knuckles were bruised and cut, and Caretaker could now see how their wrist was swelling and the way they winced when they rotated their hand. No wonder they had wanted Caretaker to stop touching them, they had been holding them by the wrist.
"Here" Caretaker pulled some salve from their first aid kit and held it out to Whumpee who looked at it suspiciously. "Put it on the cuts it should help them heal."
"What kind of bullshit is that?" Whumpee huffed but took the salve anyway.
"Modern medicine. Something you'd know a little bit about if you ever went to a hospital."
"I hate doctors" Whumpee told Caretaker flatly for about the 500th time since they'd met. Caretaker had never met anyone who consistently needed a doctor more, or had seen one less. They supposed that was why they kept them around. Caretaker was by no means a medical professional, but they knew the basics. Including how not to ask too many questions.
Caretaker watched as Whumpee applied the salve, then passed over the gauze and bandages.
"You should really get your wrist x-rayed though"
"It's just a sprain." Whumpee insisted. "I've broken it before, I know what it feels like."
"Of course you've broken it before" Caretaker mumbled to themselves, then raised their voice "At least buy a brace for it so you don't hurt it more."
"A brace won't fit it with the bandages."
"It would if you did an actual medical wrap."
Both Caretaker and Whumpee looked down at Whumpee's hand. They had used up almost the entire roll of gauze bandaging it. Only instead of bandaging their hand Whumpee had absentmindedly wrapped it as though they were going to be boxing.
"Oh," Whumpee muttered uncomfortably. Their normally stoic exterior cracked a bit as they began to unwrap the excess gauze. "Sorry."
By the time Whumpee finished rerolling the unneeded gauze Caretaker had had enough of watching them wince at the pain of trying to hold their wrist still.
Caretaker got up and poured Whumpee a glass of water, handing it to them as they began to rummage through the first aid kit again.
"Got anything stronger?"
"Yep" Caretaker pulled the bottle of painkillers they had been looking for out of the first aid kit and held them out to Whumpee. Their reaction was instantious, but no where near the reaction Caretaker had been expecting.
Whumpee jumped out of their seat as though Caretaker had just brandished a gun at them and took several steps backwards.
"What the hell Caretaker!" They cried. "Put those away!"
"Seriously!" Caretaker cried back "They're just pain killers. I'm not trying to poison you!"
"Just put them away Caretaker. Please."
"Fine!" Caretaker relented only because they weren't sure they had ever seen that look on Whumpee's face before. "What were you planning on taking for your hand then?"
"Nothing" Whumpee insisted sitting back down, though they still looked oddly uncomfortable.
"You can't be serious! That's a bad sprain! And the cut on you palm has got to be killing you."
"It's not that bad."
"You always say that. But I can see you flinching. Are you some kind of sadist or something?"
"First of all, the word you're looking for is masochist. And secondly, don't be gross." Whumpee huffed. They looked back down at their hand, winced for the umpteenth time then looked back up at Caretaker who raised an expectant eyebrow. Caretaker was a patient person. But their patience only went so far, and Whumpee couldn't really expect them not want an explanation.
"You know how I used to work for Whumper right?" Whumpee said after a long silence.
"Yes" Caretaker answered not able to totally keep the distain out of their voice. They hadn't known Whumpee then. Nor had they ever met Whumper. And Whumpee had only ever brought them up a few times. But they had heard through the grape vine that saying Whumpee had "worked" for Whumper was a bit of a stretch. It implied they had had a choice in the matter.
Whumpee ignored Caretaker's tone and carded their good hand through their hair.
"They like fighting. They liked when I fought. They'd make me do it a lot. It didn't matter if I was still injured from the last one. Concussions, broken hands, cracked ribs. If I could still swing, I could fight."
"Luckily, Whumper had a friend... A doctor, as long as we're putting everything on the table. They used to get me free pain killers to manage all my injuries. They didn't seem to care about what or how much I was taking as long as I was keeping up with Whumper's demands."
"Pretty quickly I figured out that they worked just as well for the mental pain as they did the physical. Even after I got out, it took me a long time before I stopped taking them. And if I'm being honest the only reason I did was because I couldn't afford the habit. It wasn't until I was clean that I realized how fucked up the whole thing was. Anyway, I'd rather just deal with the pain than go back to that."
The two sat in silence for awhile while Caretaker tried to process everything Whumpee had just told them. It explained a lot. Like why they hated doctors. And why they liked picking fights.
"I'm sorry." Caretaker finally said "I didn't know."
"Well I don't try and advertise it." Whumpee said with a shrug and a half smile.
"Understood" Caretaker responded "Can I at least get you some ice though? For the swelling?"
"Yeah, I don't have a problem with ice. And a beer if you've got any."
"Sure thing" Caretaker said over their shoulder on their way to the freezer. "But you know alcohol is a slippery slope too."
"Let me deal with one problem at a time." Whumpee called back.

I don't like taking pain killers. Mostly because I still haven't gotten great at swallowing those pesky pills, but also because I more so like to power through before resorting to ingesting anything.
It's a stupid thing really, because you should seek pain relief, if you're in pain but for me with a regular headache or period pains and I'll more likely just curl up in bed and rest while it sorts itself out. I know that's a very lucky position because for most parts that can handle it. The normal pain level I experience is none, and then on occasion I can have a mild pain, maybe a little annoying but rest and time can take it way.
Yesterday, I had a very minor "surgery" to remove two of my wisdom teeth. They did call it a surgery because it was the lower wisdom teeth in the jaw and those are always operated out, but it still feels a bit silly. I was advised to take a combination of paracetamol and ibuprofen about an hour later, as the anesthesia wore off.
My mouth was sore and cheeks swollen but the bleeding dried up quick and the pain killers kept me pain free, as long as I don't directly touch my cheeks or the area inside my mouth. It also made me google how pain killers worked for the first time.
I had a vague idea of how but it was still strange seeing it on the search page: when we are in pain or injured, a protein called COX2 releases chemicals called prostaglandins. These chemicals send a signal to your brain, telling you you're in pain. For some reason, it itched something in my (temporarily pain free) brain.
Because it's only really the brain that is pain free. It puts a damper on the experience of feeling the pain but the hurt obviously doesn't go away just by blocking those chemicals. You're not reacting like your body is programmed because of them. And it makes sense, you want to be comfortable, able to function, while healing takes place but it still feels strange.
Even now, nearing 36 hours after the surgery, I have been on pain killers. I'm due to take another round soon, if I feel the need, and I might because sleeping yesterday while the dose wore off wasn't exactly fun. Though I'm not sure mollifying the feel of pain will really help if I'm tossing and turning and bumping the area that's in pain.
Pain killers numb the feeling of pain. A clever trick we designed ourselves. Pretty freaking cool. I have no doubt that it's an actual life saver for those dealing with chronic illnesses/pain or having to heal from major injuries or illnesses. I love that they exist, so this is definitely not me trashing them or their use.
I am happy to use them right now, ease this already annoying period of healing and mandatory rest a little. But easing my reception of the healing wounds in my mouth have not magically sealed them back up. My body will though, another quite cool thing. Until it's further along, I'm happy to have the pain killers, even if the pills might cause me trouble.

Opioid Alternatives
Pain, whether intense or mild, brief or chronic, is detected by specialised sensory neurons that relay signals to the brain. When pain is bad or persistent enough to warrant painkillers, opioids are among the most effective at blocking this signal relay. But these drugs are highly addictive and, at high doses, can lower heart rate and breathing to a fatal degree. In looking for safer, non-addictive painkillers, scientists have screened thousands of existing drugs with various uses to find ones that act on sensory neurons (green) to reduce production of a known pain molecule, BH4 (magenta). Cells expressing BH4 appear magenta and/or white. In addition to some drugs with known analgesic effects, researchers found a schizophrenia drug that, at low doses, suppressed BH4 production. If this, or other BH4-lowering substances, can be further developed, it may be possible to have painkillers as powerful as opioids but without the devastating downsides.
