İlk yardım eğitim merkezi açmak isteyen kişi ya da kurum, gerekli belgeleri tamamladıktan sonra Acil Sağlık Şube Müdürlüğü'ne başvuruda bulunur. Yetki belgesini alan ilk yardım merkezi, ilk yardım eğitimlerini yasal olarak vermeye başlayabilir.

Vakıf veya dernek ise yönetim kurulu tarafından imzalanmış tüzük ya da ana sözleşme
Merkezde görev yapacak tam zamanlı mesul müdürün TC kimlik fotokopisi ve ilk yardım eğitmenlik sertifikası örneği
Merkezde eğitim verecek eğitmenlerin TC kimlik fotokopileri ve ilk yardım eğitmenlik sertifikaları örneği
Merkezde eğitim verecek eğitmenlerin eğitmen olmak istediklerine dair çalışma dilekçeleri
İlk yardım merkezinin yönetmeliğe uygun bölünmüş odalarını (eğitim salonu, dinlenme alanı, mesul müdür odası vb.) gösteren yerleşim planı (planda m2ler gözükecek)
Merkezde bulunması gerekli araç, gereç ve malzemelerin eksiksiz olduğunu gösteren mesul müdür imzalı malzeme listesi
Sağlık Bakanlığı onaylı ilk yardım uygulama kitabi ve sunum cdsi
İtfaiyeden alınan yangın ruhsatı
Acil Sağlık Şube Müdürlüğü başvuru evraklarını inceler ve en geç 30 iş günü içerisinde sonuçlandırır. Evraklarda eksiği bulunmayan merkeze haberli denetime gidilerek, merkezin fiziki şartları sağlayıp sağlamadığı, gerekli araç gereçlerin tam olduğu yerinde incelenerek kontrol edilir. Fiziki eksilikler var ise verilen sürelerde eksiklikler tamamlandıktan sonra müdürlükçe ilk yardım eğitim merkezi yetki belgesi düzenlenir. Yetki belgesini alan ilk yardım merkezi, ilk yardım eğitimlerini yasal olarak vermeye başlayabilir.
