Why does krillin call 18 san and not something else? Isn't san more like for friends?

"San" is an honorific, it's more formal. However, it's also something one attaches to a name, not a number. "Juuhachigou" literally means "#18". It's not meant as a name, but as a designation. Attaching "san" to that is sort of a way for Krillin to say to her that he doesn't see that as a number. It is her name. She's a person, not a thing. Roughly, the closest Western equivalent I can think of would be calling her "Miss 18". It's kind of sweet, really.
