How?

Inoichi is actually trans.

Before Minato and Kushina were dating, he and Kushina had a friends with benefits thing. He never showed off his body but he could pleasure Kushina fine so it worked.

He slept with Minato during his bachelor’s party when they were both blackout drunk.

He got pregnant with Ino.

Ino was born.

Minato and Kushina died and Inoichi swore to never tell anyone except his older brother (who figured it out instantly) and his mother (because moms’ can fix anything).

Canon is normal but Ino and Naruto are half-siblings.
